Employees' Files. 20.1 Each employee shall have an official personnel file located in Human Resources, the contents of which are limited to employment rather than academic matters. 20.2 Employees may view their official file in company with an employee from Human Resources, by arranging a suitable time during regular office hours. 20.3 The employee may have copies made of items in the file, provided the employee pays the cost of such copying. 20.4 An employee shall have the right to have the file supplemented and may add to the file in response to any documents contained therein. 20.5 Except as required by statute, no information contained in an employee’s file shall be released to anyone except the University and its agents without the employee’s written consent. 20.6 A Xxxx, Department Head or equivalent may, during the semester an employee is employed, have a current working file in the office containing documents destined for the official file in Human Resources. Employees have the right of access to such files on the same basis as outlined above. 20.7 Information in Human Resources in the files of former employees may be examined by the appropriate former employees under the same conditions as set out in 20.2. Files of former employees will be stripped or deleted entirely under the general University policy respecting this.

Employees' Files. D.2.1 An Employee will be allowed to review and receive copies of the Employee’s personnel file during normal business hours, upon prior written request to the person designated by the Employer. Such review must be made in the presence of a member of the Employee Services’ staff at a time that is mutually arranged between the Employee Services’ staff and the Employee concerned. D.2.2 The Employee may be accompanied by a Bargaining Unit or OSSTF representative. D.2.3 If the Employee disputes the accuracy or completeness of any such information, other than a performance appraisal, the Employer shall, where possible within fifteen (15) days from receipt of a request by the Employee stating the alleged inaccuracy, either confirm or amend the information on record.
Employees' Files. 10.01 Provided an employee has an eighteen (18) month discipline free record, any disciplinary records (letters of reprimand, suspensions or other sanctions) shall be removed from the employee’s personnel file. Copies of any disciplinary notations which are put on an employee's record shall be provided to the employee and the Union. 10.02 When any type of evaluation, performance review, progress report, assessment related to job performance, or nursing practice is completed with respect to any employee, it is understood and agreed that such employee shall be given an opportunity to sign the document, indicate any area of disagreement, and she/he shall also be provided with a copy of the document. 10.03 Upon request, an employee may review her/his personnel file at a time mutually convenient to the employee and the Manager of Nursing Services.
Employees' Files. 14.1 Except for initial employment documents (resume, references, etc.), nothing shall be added to any technology employee’s personnel file(s) until and unless the employee has seen the item and has acknowledged such by signing it. If the employee refuses to sign a document, it will be shown to an officer of the LEA, who will sign it. 14.2 Any employee may attach an addendum to information in her/his Personnel File. An employee may have information removed from her/his file by use of the grievance procedure, on the grounds that information in her/his file is incorrect, or irrelevant to the employment relationship. 14.3 The employee shall have the right upon request at reasonable times to examine her/his personnel file at a location designated by the Employer and to have a copy of any material in it. 14.4 The employer will not reveal information in employees' files without the consent of the employee to anyone outside the management of the Town, except when required to do so in legal proceedings. 14.5 In order for a document to constitute a reprimand, it must contain the designation RE: Letter of Reprimand; otherwise, it may not be used as evidence to establish a pattern of progressive discipline.

Adverse appraisals and discipline entries are subject to grievance by the employee. Upon request of the employee, any disciplinary document, other than discipline relating to client abuse or financial exploitation and performance appraisals, shall be removed from the employee's file eighteen (18) months from the date it was issued, provided there have not been any further disciplinary actions against the employee. Record of suspensions shall remain in the employee's file for a period of eighteen (18) months following the expiry of the suspension. Employees shall be entitled, upon reasonable notice, in writing, to access their personnel file in the office in which it is normally kept.

An employee, accompanied by a Union Xxxxxxx if requested, shall have the right to examine their personnel file in the presence of a management representative during normal business hours. Within five (5) business days after written notice, the employee may read and initial any document concerning their work performance which has been placed in their personnel file. An employee may comment in writing on any document or upon any report concerning the employee’s own performance and may request such comment to be placed in this file. Employees shall have the right to a copy of any materials in their file at no cost to the employee.
