Enrollment Services. Representations and Indemnity. If you request that we contact any prospective student, Authorized User, or other person on your behalf (“Recipient”), you represent and warrant that: (a) you will comply with all Laws and contracts in connection with use of contact information for Recipients, the Enrollment Services, and with respect to the content and transmission of calls, texts, and other messages (“Messages”) sent, including, without limitation, all federal and state telemarketing- related laws, rules and regulations, the Telephone Consumer Protection Act (47 U.S.C. § 227) and the FCC’s implementing regulations (47 C.F.R. § 64.1200) (such laws, rules and regulations, as amended from time-to-time, collectively, the “Telemarketing Laws”); (b) as to each Recipient to be contacted by us on your behalf, you have obtained all consents that may be required by the Telemarketing Laws and your privacy policies; (c) you will retain documentary proof of such consents for at least five (5) years from the date the Recipient’s contact information is provided by you to us; (d) you will suppress and will not provide to us contact information for any Recipient who has registered his or her telephone number on the national Do-Not-Call Registry, any similar state registries or has otherwise indicated that he or she does not wish to be contacted by you or us; and (e) you will provide a reasonable means for Recipients to rescind consent to receive Messages and will not request us to send Messages to Recipients who have opted out of receiving Messages from you. We shall have the right to audit your compliance with subsections (a) - (e) above. Failure to comply with any provision of this section is a material breach of the Agreement. Except to the extent prohibited by Law, including Laws providing for the sovereign immunity of government entities, you agree to indemnify, defend and hold us harmless from and against all claims, lawsuits, proceedings, causes of action, damages, liabilities, losses, judgments, fines, penalties, costs, and expenses (including attorneys’ fees) relating to or arising out of your breach of the foregoing representations and warranties, or in connection with any claim or action from a third party that arises from the sending (or inability to send or receive), content, or effects of any Messages you distribute using, or your failure to use, the Product and Service. Enrollment Services. Although the DEPARTMENT is responsible for eligibility determination and enrollment, WPS shall maintain an enrollment/eligibility system to support the BOARD'S HEALTH BENEFIT PLAN. WPS shall not process any premium transactions related to enrollment, except for those direct pay SUBSCRIBERS from whom WPS will collect premiums and credit same to DEPARTMENT. Monthly, or upon request by the DEPARTMENT, WPS shall submit a data file (or audit listing, if requested by the DEPARTMENT) to update the DEPARTMENT’S membership files in a file format as identified by the DEPARTMENT after seeking input from WPS. WPS shall submit these files using the SUBSCRIBER identifiers (currently Social Security Number) determined by the DEPARTMENT. WPS shall create separate files for SUBSCRIBERS and DEPENDENTS in a format and timeframe specified by the DEPARTMENT, and submit them to the DEPARTMENT or its designated database administrator. When the DEPARTMENT sends WPS error reports showing SUBSCRIBER and DEPENDENT records failing one or more edits, WPS shall correct and resubmit the failed records with its next update. WPS shall collect from SUBSCRIBERS coordination of benefits information necessary to coordinate BENEFITS under the Wisconsin Administrative Code and report this information to the DEPARTMENT at least annually. WPS must follow the DEPARTMENT’S file transfer protocols (FTP), such as using the DEPARTMENT’S secured FTP site to submit and retrieve files.
Enrollment Services. The Customer Enrollment Form shall be completed and signed by Customer, and approved by Entrust, prior to Entrust commencing any activities relating to Customer’s KCaaS deployment. Any changes to the Customer Enrollment Form initially completed and signed by Customer and confirmed by Entrust, shall be subject to a formal amendment which must be confirmed in writing by Entrust, and may be subject to additional fees. Upon completion of the Customer Enrollment Form, Entrust shall provide the Enrollment Services to Customer to perform Customer’s deployment of KCaaS in accordance with the information and specifications set out in the Customer Enrollment Form. Such activities shall be subject to the following:
3.11.1. Unless otherwise set out in an Order (and subject to Entrust applicable standard terms and conditions), any Customer-side Entrust or third-party components required in connection with KCaaS (including provision, installation, and/or configuration thereof), are out of scope the Enrollment Services.
3.11.2. KCaaS is subject to the following assumptions and limitations:
3.11.2.1. No visits to Customer premises or other travel are included with the Enrollment Services. Any on premises services would be subject to a separate professional services agreement or statement of work with Entrust;
3.11.2.2. Customer will provide its own network connectivity to the service environment; and
3.11.2.3. Customer will support, troubleshoot, or monitor its communications infrastructure and components, network (LAN or WAN) for the purposes of problem resolution.
3.11.3. Any deliverables provided by Entrust as part of the Enrollment Services are not “works for hire”. All right, title, and interest in, to and under any intellectual property rights conceived, created, embodied, developed, or reduced to practice by Entrust in the course of providing the Enrollment Services shall vest in and be owned by Entrust and shall be deemed to be the Confidential Information of Entrust. Except as explicitly provided herein, no other license is granted under any intellectual property rights.
3.11.4. Nothing in this Agreement shall prevent Entrust or its Affiliates from providing to a third party the same or similar services as those provided to the Customer as part of the Enrollment Services.

4.5.1 Utilize an electronic application system and paper application, as directed by ADMINISTRATOR, to enroll Individual(s)/Family(ies) into Medi-Cal and/or CalFresh.
4.5.2 Develop a tool kit with essential forms and resources to facilitate consistency in outreach and enrollment for CAAs/CECs.
4.5.3 Provide care coordination services that will include, but not be limited to, enrollment in other State and federal programs, such as Woman, Infant and DocuSign Envelope ID: D8453ACA-07AC-4C14-89F0-2382A56E685D Child; low-income housing; California Alternative Rates for Energy/Family Electric Rate Assistance (CARE/FERA); and referrals to FRCs, food banks, and other community resources.
4.5.4 Assist Individual(s)/Family(ies) with the selection of a health care provider and information on retaining benefits.
4.5.5 Provide staff at events as directed by ADMINISTRATOR.
4.5.6 Provide follow-up services to ensure Individual(s)/Family(ies) are connected to a medical and dental provider.
4.5.7 Conduct group workshops to educate newly enrolled Individual(s)/Family(ies) on how to utilize Medi-Cal and CalFresh benefits and navigate the healthcare system.
