Information Supplied to the Union.
A. The Employer shall provide the Union with a current list of bargaining unit members at the end of each month at no cost to the Union. This list shall include the Employee’s name, Employee identification number, department, location, job title, work email address, and mailing address.
B. At the end of each month, the Employer shall furnish to the Union (without cost) a report showing all personnel transactions adding to, or deleting Employees from the bargaining unit.
C. The Union specifically agrees that all information provided shall be used only for purposes related to the execution of the Agreement, that the Union shall be responsible for the protection and security of information provided, and that the Union shall assume liability which may result from any improper disclosure or use by the Union of information provided.
Information Supplied to the Union. A. The Employer shall provide the Union with a current list of bargaining unit members once per pay period at no cost to the Union. This list shall include the bargaining unit member's name, social security number, position control number (PCN), organizational routing code, department, location, strike class and termination date or last date in pay status, if applicable. The list will also itemize and show any regular deductions made and forwarded to the Union. Past practice will continue regarding the furnishing of bargaining unit member information each pay period, including PEOPLE contributions.
B. Once each month, the Employer shall furnish to the Union without cost a report showing all personnel transactions adding to or deleting bargaining unit members from the bargaining unit.
C. The Union specifically agrees that all information provided shall be used only for purposes related to the execution of the Agreement, that the Union shall be responsible for the protection and security of information provided, and that the Union shall assume liability which may result from any improper disclosure or use by the Union of information provided.
Information Supplied to the Union a. The County agrees to provide at no charge to the Union:
(1) Minutes and agenda of the Board of Supervisors, Civil Service Commission and Retirement Board.
(2) Alphabetical and numeric salary range tables.
(3) Upon request, but no more than quarterly, a computer listing of members in the bargaining unit represented by the Union.
(4) Data regarding new persons entering the bargaining unit (date of entrance into unit, name, classification, department, and employment code).
(5) Data regarding persons leaving the bargaining unit (date of exit, name, classification, department, and employment code). If the exit is due to a termination, the termination employment code will be provided; if the exit is due to movement into another bargaining unit, the active employment code will be provided.
b. The County will make a good faith effort to deliver these items in a timely fashion.
Information Supplied to the Union. 4.01 The Company will provide to the Local Union, and the Union Chairperson on a monthly basis, the following information;
(a) Name, [first, last and nick name (if known)], employee number, address (including postal code and unit number), telephone number(s), seniority date, classification, and wage rate for each employee;
(b) Names of employees transferred out of the bargaining unit;
(c) Names of employees who have been terminated, for any reason, along with the termination date;
4.02 If any of the above information is immediately required in order to administer the Collective Agreement, the Company will provide it to the Union Chairperson when requested.
4.03 An employee must provide the Company with his/her address and telephone number(s) immediately upon employment. Thereafter, an employee must provide the Company with any change to his/her address or telephone number(s). This information will be provided on Company forms. Employees will retain a signed copy of this form.

(d) Names of employees laid off or recalled to work;
