Leave Form. A permanent employee claiming an entitlement to paid or unpaid leave shall complete a Leave form and attached to such form will be evidence of the reason for the absence (as described in Clauses 14.1 and 14.4 above.) This Leave form will be presented to the Shop Manager on return from the absence for authorisation.
