Leave Report. (a) An employee absent from work through illness or injury shall, within two days of returning to work from the initial absence, submit a completed leave report form. This form is available on the Human Resource Services website. (b) The Employer may request a report from a qualified medical practitioner in one or more of the following circumstances: (1) where it appears that a pattern of consistent or frequent absence from work is developing; (2) where the employee has been absent for five consecutive scheduled days of work; (3) where at least 15 days have elapsed since the last statement was obtained and the employee has been in receipt of plan benefits throughout the period. Benefits will cease to be paid when an employee fails to provide satisfactory evidence of medical disability during the benefit period. Where the Employer requests a report from a medical practitioner, the Employer will bear the costs of the report.

Leave Report. Prior to the salary adjustment review immediately following the completion of a Study Leave, a Faculty Member must submit a Study Leave Report to the Faculty Member’s Xxxx that provides an account of: (a) The activities undertaken during the Study Leave, including whether these activities varied from the approved Study Leave application and, if so, the nature of the variation; (b) How the Faculty Member expects the activities undertaken during the Study Leave will contribute to or enhance the Faculty Member’s effective teaching and the advancement of learning at the University.
Leave Report. It shall be the responsibility of each faculty employee to notify his or her Division Xxxx or appropriate administrator when a leave is needed. Within 10 school days after returning from an absence each faculty employee shall sign and file a leave report for the absence, either via paper copy (provided by and submitted to the Division Xxxx or appropriate administrator) or through the District Portal, in accordance with whichever procedure is required by the District.
Leave Report. Within 30 days following his or her return from the completed leave, each supervisor shall submit a report to the Staff Development Leave Committee, with a copy to the appropriate administrator, that identifies the manner in which the supervisor accomplished the objectives of the leave and planned activities as described in the application and any amendments as provided in Section 4. It is the responsibility of the supervisor to deliver the report to the Office of Human Resources. Unless otherwise agreed, if the supervisor fails to provide the required leave report by the deadline, the Committee shall refer the matter to the appropriate administrator for action. The Staff Development Leave Committee is also responsible for collecting, reviewing and approving the leave reports. After reviewing the report, the Staff Development Leave Committee shall either approve the report and submit it to the Board for acceptance, or request further information or refinement of the report from the supervisor. If the Committee is unable to establish that the conditions under which the leave was granted have been satisfied, the report shall be referred to the appropriate administrator. The administrator, after consultation with those deemed appropriate, and after considering possible mitigating circumstances, may recommend that the supervisor be required to compensate the District for the expense of the leave.

Within 10 school days after returning from an absence each faculty employee shall sign and file a leave report for the absence, either via paper copy (provided by and submitted to the Division Xxxx or appropriate administrator) or through the District Portal, in accordance with whichever procedure is required by the District. In some cases, the Division office may initiate the leave report in order to facilitate timely payment for a substitute, if any, but such a leave report shall be subject to review by the faculty employee who may revise and re-submit the report before District action is taken on the leave report.
Leave Report. Within three (3) months following return from Sabbatical or Professional Development Leave, the unit member must submit a report to the Administrative Leave Committee. The committee will review the report and may request further refinement or revision of the report prior to submitting it to the Superintendent/President for review and submission to the Board. If the leave report does not satisfy the committee, after the requested revision has been addressed, it may recommend that the unit member repay the leave funds and/or be denied future leaves. The format of leave report will be negotiated between the College District and SCCDAA.
Leave Report. Faculty Members who take an Administrative Leave are under an obligation to return to the University for a period of time that is equal to the length of the Administrative Leave.
