Long-Term Unpaid Leave. 1. Requests for long-term leaves (more than 10 workdays) shall be granted for a full year or may be granted for a semester or the balance of the school year to tenured members. Long-term unpaid leave will not be granted to a member on a Plan of Improvement, or to a member who has taken long-term unpaid leave which extended to the end of a school year within the last five years. Requests for unpaid medical leave must be accompanied by a physician’s statement of inability to perform duties which is acceptable to the District.
2. Applications for long-term unpaid leave must be received by July 15; other requests will be reviewed on a case by case basis.
3. Upon request, the leave may be extended for up to an additional year.
Long-Term Unpaid Leave. An employee with three (3) years of consecutive employment with the District may request leave for a period of one (1) year. For a school term employee the request must be made at least sixty (60) days before the requested start date for the leave and before the employee's last day of work for the school term. For all other employees, the request must be made at least sixty (60) days in advance of the requested start date for the leave. The decision to grant such leave shall be at the discretion of the Superintendent whose decision shall be final.
Long-Term Unpaid Leave. 1. Employees who have a contract of employment for an indefinite period and who have been in employment with the employer for at least five years may request long-term unpaid leave, for the purpose of long-term employability. Such a request may be made a maximum of once every five years.

2. When possible, a 30-workday notice shall be given to the District of intent to take a long-term leave.
3. Upon request, the leave may be extended for up to an additional year.
Long-Term Unpaid Leave. The Board shall grant unpaid leave of absences in keeping with the following:
a. Unpaid leave of absence shall be granted not to exceed one (1) year; normally such leave shall be granted according to one-half (1/2) year periods for the following:
(1) Personal injury
Long-Term Unpaid Leave. When an employee has exhausted his/her accumulated sick leave, and is still unable to return to work due to illness or other disability, the employee may request and shall be granted unpaid leave for a period of no more than two (2) school years as provided by Section 3319.13 O.R.
Long-Term Unpaid Leave a. A leave of absence without pay for up to one (1) year may be granted for the purpose of caring for a sick member of the teacher’s family upon the teacher’s request.
b. The employee must inform the Employer, in writing to the Superintendent, by March 31 regarding his/her intention to return or not to the employment of the Eastwood Local School District.
c. If no such written notice is provided by the employee to the employer by March 31, the employee will be declared as resigned from such employment.
Long-Term Unpaid Leave. A teacher with 4 consecutive years in the District may be granted unpaid leave for either or both semesters of a school year.
Long-Term Unpaid Leave. (a) Any employee that has worked for the Employer for a cumulative period of twelve (12) months and averaged at least one-half the full- job classification during those twelve months is eligible for up to a total of 12 work weeks of unpaid leave in a twelve (12) month period for one or more of the following reasons: birth and care of a newborn child of the employee; placement with the employee of a son or daughter for adoption or xxxxxx care; care for an immediate family member (spouse, child, or parent) with a serious health condition; or medical leave when the employee is unable to work because of a serious health condition.
Long-Term Unpaid Leave. (a) Any employee that has worked for the Employer for a cumulative period of twelve (12) months and averaged at least one-half the full-time equivalent position in the employees’ job classification during those twelve months is eligible for up to a total of 12 workweeks of unpaid leave in a twelve (12) month period for one or more of the following reasons:
(b) Time spent on leaves of 12 weeks or less shall be considered service time with the Employer in computing severance pay, experience rating, length of vacation, and all other benefits which depend in whole or in part upon the length of service with the Employer.
(c) Xxxxxxx employed by the same employer are jointly entitled to a combined total of 12 work- weeks of family leave under this provision for the birth and care of the newborn child, for placement of a child for adoption or xxxxxx care, and to care for a parent who has a serious health condition. Leave for birth and care, or placement for adoption or xxxxxx care must conclude within 12 months of the birth or placement.
(d) An employee that takes leave pursuant to FMLA or Minnesota’s Parental Leave Act shall be guaranteed a return to their previous position or an equivalent position, including number of hours per week and rate of pay.
(e) An employee returning from a leave of absence under this section of more than one month shall notify his or her supervisor at least two weeks prior to return from leave, unless otherwise approved by the Employer.
