Medical Policies. A. Employees will pay the lessor of:
1. Premiums calculated by the insurance committee to represent 10% of the total cost of providing health insurance to participation employees.
2. Insurance premiums capped at:
a. Family - $287 b. Employee/Spouse - $181 c. Employee/Child - $181 d. Employee - $110
B. At the Insurance Committee’s discretion, if total District expenses fall below 90% of the total cost to the District in providing health care, plus reasonable claims, reserve may be spent to stabilize the employees’ percentage of insurance premiums in any of the following ways:
1. Offsetting increasing insurance costs
2. Reducing the employees’ monthly payroll deductions for insurance
3. Increasing employees’ insurance benefits
C. Current out of pocket costs and deductibles will not increase during the duration of the contract.
D. In the event the insurance reserve fund falls below $400,000 the insurance committee’s discretion will be assumed by the District until the fund balance returns to a level above $500,000. Recommendations made by the insurance committee will be considered by the District when making its decision.
E. An optional $50,000 group term-life insurance policy will be offered to each permanent full and part-time employee, the District paying 100% of the premium.
F. Individual teachers may opt to not participate in the group health insurance plan.
G. Teachers may change insurance carriers for subsequent school years if a majority of all bargaining unit members elect to make such a change in a secret ballot election conducted by a neutral third party. If the teachers opt for a new carrier, the District’s premiums will be the capped amount in A above. The teacher’s proportionate share of money remaining in the premium stabilization fund after all claims are paid will be distributed to the teachers.
H. The District will provide each teacher with the option to have his/her medical insurance premium payment as a pretax deduction. The District will also provide the option of flexible spending accounts for medical and childcare expenses in compliance with Federal and State law.

Medical Policies. A. As appropriate, during the term of this Contract, the Team may direct the Player to undergo medical examinations and treatments from a health care provider selected and provided by the Team. The Team pays for such examinations and treatments. The health care provider shall furnish all relevant information pertaining to the Player to any independent health care provider designated by the Team.
B. The Player must notify the Team through the trainer and head coach of the Player's Team immediately if the player is injured during the term of this Contract. The notice must include the time, place, cause and nature of the injury. The Team reserves the right and the Player agrees to submit to an independent physical examination at any time during the term of this contract by a physician chosen by the Team.

During the term of this Contract, the League reserves the right to require, and the Player agrees to submit to, medical examinations and treatments from a health care provider selected by the League or the Player’s assigned Team. The assigned team shall assume all costs for such examinations and treatments. The Player hereby authorizes any such health care provider to furnish to the League all
B. If the Player is injured while performing the services of this Contract, he must immediately report such injury to the League via the trainer and head coach or physician of the Player’s assigned team no later than twenty-four (24) hours after the Player’s first reasonably becomes aware of the injury. To the extent reasonably known or practicable, the notice must include the time, place, cause and nature of the injury. The Player will receive such medical and hospital care during the term of this Contract as the assigned team’s physician(s) deems necessary and appropriate in his professional judgment.
C. If the Player is unable to perform the services required of him as a player in the Arena Football League due to a work-related injury, the Player will receive such care and compensation as provided by the League pursuant to the CBA and applicable state laws. In accordance with the CBA, the Player must utilize the physician(s) and trainer(s) provided by the League and/or Player’s assigned team. The Player must make reasonable efforts to follow the instructions of the assigned physician(s) and trainer(s) and keep all appointments for treatment and/or rehabilitation (including traveling to meet with the physician(s) and trainer(s), if required). Except as provided by the CBA, if the Player fails to keep appointments or follow the physician’s and/or trainer’s instructions without good cause, it will be grounds for the termination of this Contract for cause by the League.
Medical Policies. A. The Player agrees to: (1) report in good physical condition at the time and place fixed by the Club, and (2) keep himself in good physical condition during the entire season. Player further agrees not ingest either orally, by injection or otherwise, any narcotic, habit-forming, illegal or hallucinatory drug except when prescribed by the Club's physician for medically-related illnesses or medical emergency. To monitor this provision, the CBA may administer urinalysis tests to the Player at any time, and without notice, during the term of this Agreement.
B. The Club and the CBA may furnish relevant medical information about the Player to any interested CBA or NBA team if it contemplates selling, exchanging or assigning this Agreement to another team.
C. If the Player, in the Judgment of the Club's physician, is not in good physical condition on the date of his first scheduled game for the Club or if, at the beginning of or during any season (regular and playoffs), he fails to remain in good physical condition (unless such condition results directly from an injury sustained by the Player as a direct result of participating in any basketball practice or game played for the Club during such season), so as to render the Player, in the sole judgment of the Club's physician, unfit to play skilled professional basketball, the Club shall have the right to suspend such Player until the Club's physician determines that the Player is in sufficiently good physical condition to play skilled professional basketball. In the event of such suspension, the annual sum payable to the Player for the regular season shall be reduced proportionate (compared to the length of the regular season) to the length of time that the Club's physician determines the Player is unfit to play skilled professional basketball.
D. The Player agrees to provide the Club's coach, trainer or physician with immediate notice of any injury suffered by him which may impact, impair or otherwise affect, either immediately or over time, his ability to play skilled professional basketball. Such notice shall include the time, place, cause and nature of such injury.
E. Should the Player suffer an injury covered in subsection D above, he shall submit himself to a medical examination and treatment by a physician designated by the Club. Such examination, when made at the request of the Club, shall be at its expense, unless made necessary by some act or conduct of the Player contrary to the terms of this Agreemen...
Medical Policies. A. Total contribution to the health insurance program will be based on the annual negotiated renewal provided to the DISTRICT by the health insurance program brokers in the spring of each year for the following year's rates.
1. The DISTRICT will contribute 90% of the total cost of the Health Insurance program each year.
2. Employee premiums will be set annually based on 10% of the 104% of Expected Cost, which will be provided in the negotiated renewal for the following year.
3. Employee premiums will not fluctuate year to year by more than 2% after the first year of this agreement.
B. Unless by mutual agreement of KEA and the DISTRICT, the health insurance plan offerings will not change. KEA will have the option of changing plans should the DISTRICT or another bargaining group agree to a change of plan or benefits that is mutually beneficial.
C. At the conclusion of this agreement, the DISTRICT and KEA will convene a Health Insurance Task Force to review plan benefits and costs, and compare with other plan options. The Task Force will make recommendations to the BOARD at the conclusion of their work.
D. Once the DISTRICT'S Health Insurance Reserve Fund achieves a surplus of $600,000, the DISTRICT will make a good faith effort to maintain a minimum balance of $600,000 in order to stabilize employee premium costs. Funds in the DISTRICT'S Health Insurance Reserve Fund can only be utilized for health insurance program costs.
E. At the Insurance Committee's recommendation, if total District expenses fall below 90% of the total cost to the District in providing health care, plus reasonable claims, reserve may be spent to stabilize the employees' percentage of insurance premiums in any of the following ways:
1. Offsetting increasing insurance costs
2. Reducing the employees' monthly payroll deductions for insurance
3. Increasing employees' insurance benefits

G. The DISITRICT shall provide a $50,000 group term-life insurance policy to each permanent full and part- time employee, with the District paying 100% of the premium.

Medical Policies. For children’s comfort and to reduce the risk of contagion, parents/guardians must pick up their child 1 hour after notification. Children absent due to reportable disease may return only with a physician’s note indicating that the child is no longer contagious.
