Notification and Documentation Requirements. (a) The team member will provide the Company with notice as soon as is reasonably practicable of their intention to take carer’s leave, a satisfactory explanation of the reason for the leave and the estimated duration of absence.
(b) Reasonable proof of absence may be required by the Company to grant payment for xxxxx’s leave.

For any absence of 2 consecutive shifts or more or a single day absence on the day before or the day after a public holiday, a medical certificate shall be required from a duly qualified medical practitioner or statutory declaration to receive payment. Wherever practicable the team member shall notify their Manager, or if their Manager is not present, the Manager on duty, prior to the commencement of their shift of their inability to attend for duty and, as far as reasonable, the nature of the illness or injury and the estimated duration of the absence.

27.2.2 A Team Member may take up to 2 days’ personal leave in any one year without a medical certificate or a statutory declaration.
27.2.3 For absences in excess of 2 days in any one year, a Team Member will provide to Officeworks such evidence as would satisfy a reasonable person such as a medical certificate or statutory declaration.

The employee must give Xxxxxxx Care prior notice of any absence or of the intention to take leave as soon as reasonably practicable that he or she needs leave to provide care for any person as described in clause 17.4 and that the person requires care or support by another person because of a personal illness or injury or an unexpected emergency affecting them. If it is not practicable for the employee to give prior notice of absence, the employee will notify his/her supervisor of such absence at the first opportunity on the day of absence. Supporting documentation - as soon as reasonably practicable The employee must provide a medical certificate from a registered medical practitioner or a statutory declaration by the employee. The medical certificate or statutory declaration must state: • the name of the person requiring care and that person’s relationship with the employee; and • the reasons for taking such leave and the estimated length of absence; and • that the person has (or will have) an illness or injury during the period or that the person is or was affected by an unexpected emergency and that the illness is such as to require care by another person, in accordance with the requirements of the Workplace Relations Act. The employee is also required to complete and submit a (personal) Leave Form Request for approval by their Manager.

Notification and Documentation Requirements. (i) An employee taking Xxxxx’s Leave is required to provide notification to Scenic World in accordance with clause 9.2.1(b) of this Agreement.
(ii) The employee must, if required by Scenic World, provide a medical certificate, statutory declaration or other satisfactory evidence following 2 consecutive day absences on Xxxxx’s leave. Scenic World may also require an employee to provide medical certificates, statutory declaration, or other satisfactory evidence for any absences on carer’s leave when the employee has taken more than 5 single days absences in one year, excluding absences taken for sick leave.

Notice - as soon as reasonably practicable The employee must inform Xxxxxxx Care as soon as reasonably practicable, where possible no less than 2 hours before the commencement of their rostered shift, that he or she is (or will be) absent from his or her employment due to a personal illness or injury. The employee will inform Xxxxxxx Care of the expected duration of the absence and the nature of the illness. Xxxxxxx Care requires an employee to notify his or her Manager by telephone (not by text message/SMS). Supporting documentation - as soon as reasonably practicable For any absence: ▪ Of more than 2 consecutive days; ▪ Taken either side of a rostered day off or public holiday; ▪ Taken immediately before or after a period of annual leave; ▪ Where an employee agrees to work an additional shift and fails to work that shift; ▪ the employee must provide one of the following documents supporting the employee’s illness or injury which required the employee to be absent from work: ▪ if it is reasonably practicable to do so - a medical certificate from a registered health practitioner and a statement that, in the practitioner’s opinion, the employee was, is or will be unfit for work during a period because of a personal illness or injury; or ▪ if it is not reasonably practicable for the employee to give Xxxxxxx Care a medical certificate - a statutory declaration made by the employee which says that the employee was, is or will be unfit for work during the period because of a personal illness or injury. A statutory declaration will only be accepted where an employee is able to demonstrate that they were unable to make an appointment with their medical practitioner on a particular day. Xxxxxxx Care reserves the right to confirm with the employees medical practitioner their inability to secure an appointment. Please note Xxxxxxx Care may require an employee to provide a medical certificate or statutory declaration for any absence, including a single day absences where deemed necessary. Where possible, on the day of return to work, the employee is also required to complete and submit a (personal) Leave Form Request for approval by their Manager.
