Subrecipient Duties and Criteria. The Subrecipient shall: • Recruit and assign qualified personnel to be trained and certified as Navigators by the State; “qualified” includes, at minimum, being a high school graduate and having prior education or employment experience that is relevant to the Navigator role; • Evaluate and manage performance of Navigators; • Ensure that Navigator staff have the appropriate technological skills and equipment to complete on-line applications through VHC; • Obtain a State of Vermont criminal conviction report on each Navigator for whom one was not previously obtained by the Subrecipient organization and keeping on file throughout the term of this agreement. If the prospective Navigator has not resided in the state of Vermont for the past five consecutive years, then an FBI criminal background check is required. The Subrecipient shall make the criminal conviction report available to the State upon request; • Ensure that only certified Navigators are providing direct, in person enrollment assistance to consumers; • Identify distinct target populations and provide for additional outreach and education in conjunction with the State’s outreach efforts; • Provide monthly reports to the Project Director detailing outreach activities, enrollment activities, and work plans for the coming month per the specific requirements outlined in the standard reporting tool (see Appendix IRequired Documents); • Attend quarterly Navigator Organization summits to be held in July, October, January and April; • Manage grant funding and compensation of Navigators, if applicable; and • Notify the State immediately if a Navigator terminates employment with Navigator organization. The Subrecipient’s individual Navigators shall: • Maintain expertise in eligibility, enrollment, and program specifications; • Conduct public education activities to raise awareness about the Exchange; • Provide information and services to Vermonters in a fair, accurate and impartial manner; • Facilitate selection of a qualified health plan (QHP) and/or Medicaid, Dr. Dynasaur, and other public health benefit program for eligible individual applicants using the Vermont Health Connect website; • Provide referrals to any applicable office of health insurance consumer assistance or health insurance ombudsman established under section 2793 of the Public Health Service Act, or any other appropriate State agency or agencies, for any enrollee with a grievance, complaint, or question regarding their health plan, cove...

Participate in monthly check-in calls with and provide reports to the State upon request. • Attend conference calls and webinars when indicated as mandatory by the State. • Manage compensation of Navigators, if applicable. • Notify the Assister Program Manager immediately if a Navigator terminates employment with the Navigator organization. • Subrecipient shall maintain a physical office location in Vermont from which Navigators shall frequently provide face-to-face assistance to customers. • Submit documentation to State before beginning work under this Grant Agreement, and no later than July 30, 2022, attesting to being and remaining conflict of interest free pursuant to 45 CFR 155.215 for any period of this Grant Agreement. This includes performing relationship checks on the individual Navigators working for the Subrecipient. The State retains the right to require Subrecipient to prepare and perform a written remediation plan should there be any concerns or questions regarding the attestation or the relationship checks. • Ensure that individual Navigators are competent, professional and have the appropriate skills to adequately fulfill Navigator duties. The Subrecipient shall ensure its individual Navigators: • Maintain expertise in enrollment and program specifications. • Provide information and services to Vermonters in a fair, accurate and impartial manner. • Facilitate selection of a Qualified Health Plan (QHP) and/or Medicaid, Dr. Dynasaur, and other public health benefit program for eligible individual applicants using Agency of Human Services authorized enrollment methods, includin...
Subrecipient Duties and Criteria. The Subrecipient shall: • Identify, engage, and support those who identify as vulnerable or underserved populations. • Provision onsite, workplace-based health insurance education and coaching through Working Bridges Resource Coordinators across an existing employer network of 15 companies in Chittenden and Xxxxxxxx counties. • Develop and disseminate health insurance information and education to nonprofit employers through Common Good Vermont’s existing network. • Produce multilingual, workplace-based videos to be distributed throughout communities via United Way of Northwest Vermont communication channels, workplaces within the Working Bridge network, across the Common Good nonprofit employer network, and through Vermont Language Justice’s channels. • Promote health insurance information (using the Department of Vermont Health Access Health Insurance 101 document as a tool), educate, and support resources in diverse languages. The Subrecipient shall ensure its information used is: • Up to date with the most current policies. • Provided to Vermonters in a fair, accurate and impartial manner. • Clarifies the enhanced subsidies for qualified health plans as a result of the American Rescue Plan Act. • Provide information in a manner that is culturally and linguistically appropriate to the needs of the population being served by the Subrecipient, including individuals with limited English proficiency, and ensures accessibility and usability of presentation and informational tools and functions for individuals with disabilities in accordance with the Americans with Disabilities Act and section 504 of the Rehabilitation Act.
Subrecipient Duties and Criteria. Assign qualified personnel to be trained and certified as Navigators by the State; • Obtain a State of Vermont criminal conviction report on each selected Navigator and keep on file. If the prospective Navigator has not resided in the state of Vermont for the past five consecutive years, then an FBI criminal background check is required. Navigators may initiate employment with the Subrecipient prior to the receipt of background check results by the Navigator organization. Continued employment of Navigators is contingent upon satisfactory results of the required criminal history reports; • Manage and oversee Navigators; • Ensure that only State certified Navigators are providing enrollment assistance; • Leverage connections with key target populations to boost the State’s outreach efforts; • Provide monthly status reports detailing engagement and enrollment efforts; • Meet with the state on a quarterly basis; attend additional meetings either in person or by phone as requested by the state; and • Manage State grant funding and compensation of Navigators, if applicable.
Subrecipient Duties and Criteria. The Subrecipient shall: • Identify, engage, and support those who identify as vulnerable or underserved populations by: o Distributing at least 500 pieces of printed material in the form of flyers, rack cards, and other types to targeted populations in an effort to raise awareness and advertise available services and programs. Printed material will be distributed to local organizations such as: medical facilities, churches, general stores, and others; reaching up to 50 different locations. o Coordinating with other local organizations to arrange distribution of printed materials and other information to target populations (for example, meals on wheels) o Disseminating information via newsletter up to 3 times during the grant period and reaching approximately 8,000 households. o Outreaching these target populations via social media, Constant Contact (weekly to approx. 523 recipients), email, press releases, phone calls, and paper-free contact. • Host at a minimum two (2) community education sessions based on connections made during outreach and education initiatives. • Conduct home and community visits in which Neighborhood Connections representatives will reach out to identified uninsured/underinsured individuals residing within their catchment area to offer a one-on-one appointment to determine eligibility, assist with application, and provide follow-up services. • Participate in monthly check-in calls with and provide reports to the DVHA Communications and Outreach team as requested by the State. The Subrecipient shall ensure its information used is: • Up to date with the most current policies. • Provided to Vermonters in a fair, accurate and impartial manner. • Clarifies the enhanced subsidies for qualified health plans as a result of the American Rescue Plan Act. • Provide information in a manner that is culturally and linguistically appropriate to the needs of the population being served by the Subrecipient, including individuals with limited English proficiency, and ensures accessibility and usability of presentation and informational tools and functions for individuals with disabilities in accordance with the Americans with Disabilities Act and section 504 of the Rehabilitation Act.

The training should be replicable and ideally could be shared beyond the organization and shared with the Department of Vermont Health Access (DVHA) for future use. • Engage an audience who identify as a vulnerable population or support vulnerable populations. • Over the course of the grant, capture an audience of approximately 35 people or more. • Evaluate attendee satisfaction of presentation. • Ensure that presenters have the appropriate technological skills and equipment to develop and deliver the presentation. • Participate in monthly check in calls with and provide reports to the DVHA Communications and Outreach team as requested. • Notify the State immediately if any named grant participant terminates employment with grantee organization. • Ensure that presenter(s) are competent, professional and have the appropriate skills to adequately deliver information, engage participants, and disseminate information. • Track the number of people reached through presentations. The Subrecipient shall ensure its information used is: • Up to date with the most current policies. • Provided to Vermonters in a fair, accurate and impartial manner. • Clarifies the enhanced subsidies for qualified health plans as a result of the American Rescue Plan Act. • Provide information in a manner that is culturally and linguistically appropriate to the needs of the population being served by the Subrecipient, including individuals with limited English proficiency, and ensures accessibility and usability of presentation and informational tools and functions for individuals with disabilities in accordance with the Americans with Disabilities Act and section 504 of the Rehabilitation Act.
