Weekly Indemnity Insurance. (a) An insured employee who is totally disabled and unable to work due to sickness or an accident not covered by Workers’ Compensation, will be paid a weekly benefit commencing on the first day of absence due to an accident or hospitalization and the fourth scheduled day of absence due to a sickness. The amount of the benefit and the maximum period for which the benefit is payable shall be as set out in the following schedule: Years of Continuous Full-time Employment with the Company At Least But Less Than Amount of Weekly Benefit Maximum Period First of the month next following date of hire 3 years 75% of basic weekly wage 26 weeks after 3 years 85% of basic weekly wage 26 weeks The weekly benefit shall be determined based on basic weekly wage and service as of the date of commencement of disability.
(b) It is not required that the insured employee be confined to home, but the employee must be under the regular care and attendance of a licensed physician during the period for which benefits are being claimed.
(c) Certification of illness for loss of time benefits, up to a maximum of ten (10) weeks for each period of disability, will be accepted if completed by a licensed Chiropractor, Podiatrist or Oral Surgeon.
(d) A disability resulting from the same cause as a previous disability will be treated as a continuation of the previous disability unless the employee has returned to work on a continuous full-time basis for at least fourteen (14) days.
(e) Weekly benefits are not payable for the following:
(i) disability for which an employee receives benefits under any Workers’ Compensation or similar law;
(ii) disability resulting from willfully, self-inflicted injury or from any attempt at self-destruction;
(iii) disability of an employee who is not under treatment by a physician or surgeon duly licensed to practice medicine, or during the first four (4) weeks of disability of an employee who is not under treatment by a licensed Chiropractor;
(iv) pregnancy where a female employee is not disabled;
(v) any period during which a female employee is on a pregnancy leave of absence in accordance with this Agreement.
Weekly Indemnity Insurance. The Plan shall provide sickness and accident benefits, to a maximum of 75% of regular earnings x base hourly rate) for 26 weeks which shall be paid from the first day of accident, and from the fourth working day of absence due to certified illness. The Plan shall be effective the first day of the month following commencement of employment. In the event that an employee has not received the first weekly indemnity payment on an approved claim, within 10 calendar days of the date such claim was approved by the carrier, the Employer shall advance the net amount of such payment to the employee, provided the employee agrees to repay such advance by turning over to the Employer the first payment he receives from the carrier.
Weekly Indemnity Insurance the Employer agrees to pay one hundred percent (100%) of the billed premium for the plan or at its option self- insure the plan which provides seventy-five percent (75%) of weekly earnings up to a maximum benefit per week. Benefits begin the first day of injury, the first day of hospitalized illness or the fourth day of non-hospitalized illness.

Weekly Indemnity Insurance. Article 20.01 (g) will be amended for employees assigned to the twelve hour shift schedule with the intent that employees will not have a benefit reduction as a result of working twelve hour shifts.
Weekly Indemnity Insurance. The weekly indemnity insurance plan will provide for payment of lost wages on the third day of sickness. The coverage will be to 26 weeks. The Company commits that no employee shall be negatively affected by working the weekend shift. Any waiting period shall be equivalent to eight (8) hours = one (1) day. Therefore twelve (12) hours = one and one half (1 ½) days. If delays in payment of claims occur, the company will continue, as in the past, to advance payment prior to final adjudication of a claim.
Weekly Indemnity Insurance. The weekly indemnity insurance plan to cover the loss of income when Employees are disabled and unable to work due to sickness or non-occupational accident will be as follows: Payment - to be 66.7% of the Employee's regular straight time hourly pay for time lost due to above causes, to the maximum of $660.00 per week (yr 2 - $670.00, yr 3 - $680.00). Employees must be under the care of a physician for the period to be covered (medical certificate presented). In cases of psychological illnesses, a medical certificate from an appropriate specialist will be required. Coverage - first working day coverage for non-occupational accident or hospitalization. Fourth working day coverage for illness. Coverage will cease if an Employee voluntarily quits, is terminated, is on lay-off, extended leave of absence, or retires. Life, health and dental premiums will continue to be covered during the period of weekly indemnity so that Employees may make claims during this period. Premiums - Company to pay 100% of premium costs. Any E.I. rebate will belong in full to the Company. Eligibility - 1st of the month following completion of the probationary period.

Weekly Indemnity Insurance. 66 2/3% of earnings - Maximum weekly benefit: E.I. maximum - Qualifying period: 3 days of disability or, if shorter, the period before the 1st day the patient was admitted to a hospital as an in-patient and hospitalized overnight, or none if disability is due directly to an injury caused solely by accidental, violent and external means and disability began within 30 days of the initial injury. - Benefit period: 26 weeks - Termination of insurance: 65th birthday
Weekly Indemnity Insurance. All full-time union employees - 70% of weekly salary to a maximum benefit. Benefits are based on a normal work week (Monday through Friday) and payments commence on the fourth working day of disability if due to an accident and the fourth working day of disability if due to sickness and are payable for a maximum of 26 weeks.
