Athletic Training Services. During the athletic events at which Athletic Trainers are in attendance, they shall attend to any injuries sustained by School students as a result of the student's participation in the athletic event. Services provided by Athletic Trainers may include, without limitation: (1) performing physicals for student athletes as necessary, (2) conducting an initial assessment of an athlete's injury or illness and formulating an impression of the injury or illness in order to provide emergency care and referral to a physician for definitive diagnosis and treatment, if necessary; (3) administering first aid and emergency care for acute athletic injuries and illnesses; and (4) providing health care information to and counseling athletes while at the athletic event. Before athletic events, Athletic Trainers may carry out injury prevention measures to minimize the risk of injury during play. In addition, Athletic Trainers will provide on- site injury checks each week during competitive seasons, pre-participation physicals to student athletes at each school (mass participation format only), and will offer ImPACT testing for all high school student athletes in district after district approval (cost paid by student athlete or school). All examinations and treatment of student athletes will be performed in the presence of the parent, coach, another student athlete, or school designee. All services provided by Athletic Trainers will be in accordance with state and federal law, Board of Education policies and administrative procedures, and MSHSAA standards and guidelines with respect to the provision of such services, specifically including the assessment and treatment of concussions.

Athletic Training Services. The Athletic Trainer providing AT Services to the School will have (or will be in as of the Commencement Date diligently pursuing) received certification by the Board of Certification, Inc. and licensure from the Commonwealth of Pennsylvania. In addition, the Athletic Trainer will have completed (or will be in the process of completing) the training requirements of the Child Protective Services Law and a PA Criminal Background Check, PA Child Abuse History and PA FBI Clearance. Copies of all documentation will be forwarded to the School upon request.
Athletic Training Services. During the School Year, the ATs will attend practices and games of the School District’s sports teams (“School Events”), except where the School District has scheduled two or more events at the same time, one or more of which is scheduled to occur outside the School District property. In the event of conflict, the AT and the Athletic Director will determine which events are to be covered. • Upon request, ATs will provide Services for additional events that are not School Events, such as playoff games and league games. School District shall notify LGMG no less than one (1) week in advance of the non-School Event when requesting Trainer Services. • ATs will see out-of-season student athletes as time permits, while granting first priority to in-season student athletes. • ATs will advise the School District as to the supplies and training equipment required for the School District’s athletic training programs. • ATs will assist the School District’s faculty and coaching staff in the design and implementation of a student athletic training aide program. • ATs will provide continuing education programs to the School District’s coaching staff. • ATs will attend preseason coaches’ meetings, and upon request, PTO and school board meetings. • ATs will coordinate and provide injury follow up and evaluations for the School District’s team physician. • ATs will monitor athletic injuries and develop injury prevention training and programs with the advice and consent of School District’s team physician. • School District is responsible for contracting with Emergency Medical Services (“EMS Personnel”), if so desired. Therefore, services under this agreement shall not include emergency medical services, unless such treatment is needed prior to arrival of EMS Personnel or to assist EMS Personnel as needed. • LGMG will work to provide priority access for School District student-athletes to LGMG physical therapy at all Lancaster General Health locations. • LGMG shall provide information and treatment protocols to School District and student-athletes regarding concussions and other common sports injuries, as well as guidelines addressing post-injury return-to-play. • LGMG shall, in coordination with athletic department provide 1 mass sports pre-participation physical over the summer in preparation for the upcoming school year on-site at a designated school district facility. • LGMG shall provide one session of Nutrition Education per season, with additional sessions provided u...

Athletic Training Services. MNPG shall provide up to two (2) licensed athletic trainers (the “Athletic Trainers”) for the period beginning on the first day of fall sports heat acclimation practices and shall continue to provide such Athletic Trainers until the conclusion of spring sports competition. As long as MNPG is exercising reasonable efforts to hire such Athletic Trainers in a timely manner, MNPG shall not be liable for any delays in performance related to its inability to hire such Athletic Trainers. The Athletic Trainers, in cooperation with BEASD’s athletic department staff and coaches, shall provide: 1. Athletic event and practice coverage; 2. Injury assessment; 3. Injury rehabilitation; 4. On-field first aid management; 5. Post injury triaging and care coordination; 6. Pre & post-competition taping and treatment; 7. Strength training, conditioning and nutrition consultation; 8. Athletic training student supervision; and 9. Care consistent with the protocols determined in cooperation with the Physician (as defined below).

Athletic Training Services. Contractor agrees to furnish those athletic training services as set forth on Exhibit A attached hereto (the “Athletic Training Services”) during such times and at those events set forth on Exhibit B attached hereto. In the event Contractor’s attendance would be required at more than one (1) event as set forth on Exhibit B as a result of multiple events being scheduled for the same time, the Athletic Director for the School will give Contractor reasonable prior notice as to whether Contractor shall divide its time between events or whether the Contractor shall only attend one (1) of the events. Contractor represents that its Athletic Training Services shall be provided in a first-class high quality and professional manner and that it has the background and expertise and personnel necessary to provide the Athletic Training Services for the School.

  • Dining Services The Contract is for a space in a University Housing & Dining Services (“UHDS”) facility and not for a particular room or type of housing. By signing this Contract, you agree to accept your residence assignment, and understand this assignment may change. Once you receive a key to your assigned residence or move personal belongings into your room (whichever comes first) you are considered to have taken occupancy (“Occupancy”) and will incur charges. You also agree to familiarize yourself and comply with all University policies governing occupancy, including those set forth in this Contract and in the UHDS publication Student Policy and Information Guide, to be considerate of other residents and to respect the rights of others at all times.   • Hosting Services 13.1 If Supplier or its subcontractor, affiliate or any other person or entity providing products or services under the Contract Hosts Customer Data in connection with an Acquisition, the provisions of Appendix 1, attached hereto and incorporated herein, apply to such Acquisition. 13.2 If the Hosting of Customer Data by Supplier or its subcontractor, affiliate or any other person or entity providing products or services under the Contract contributes to or directly causes a Data Breach, Supplier shall be responsible for the obligations set forth in Appendix 1 related to breach reporting requirements and associated costs. Likewise if such Hosting contributes to or directly causes a Security Incident, Supplier shall be responsible for the obligations set forth in Appendix 1, as applicable. 14 Change Management

Melbourne Archdiocese Catholic Schools Ltd (MACS) governs the operation of MACS schools and owns, governs and operates the School. 1.2 Parents and guardians, as the first educators of their children, enter into a partnership with the Catholic school to promote and support their child’s education. Parents and guardians must assume a responsibility for maintaining this partnership by supporting the school in the provision of education to their children within the scope of School's registration and furthering the spiritual and academic life of their children.

  • Counseling Services SUD therapy/counseling is a relationship between people that works in part because of clearly defined rights and responsibilities held by each person. As a client in the SUD counseling process, you have certain rights and responsibilities that are important for you to understand. There are also legal limitations to those rights that you should be aware of. I, as your SUD counselor, have corresponding responsibilities to you. These rights and responsibilities are described in the following sections. SUD therapy/counseling has both benefits and risks. Risks may include experiencing uncomfortable feelings, such as sadness, guilt, anxiety, anger, frustration, loneliness and helplessness, because the process of therapy often requires discussing the unpleasant aspects of your life. However, therapy has been shown to have benefits for individuals who undertake it. Therapy often leads to a significant reduction in feelings of distress, increased satisfaction in interpersonal relationships, greater personal awareness and insight, increased skills for managing stress and resolutions to specific problems. But, there are no guarantees about what will happen. Therapy requires a very active effort on your part. In order to be most successful, you will want to put into practice things we discuss outside of sessions. The first 2-3 sessions will involve a comprehensive evaluation of your needs. By the end of the evaluation, I will be able to offer you some initial impressions of what our work might include. At that point, we will discuss your treatment goals and create an initial treatment plan. You should evaluate this information and make your own assessment about whether you feel comfortable working with me. If you have questions about my procedures, we should discuss them whenever they arise. If your doubts persist, I will be happy to help you set up a meeting with another SUD professional for a second opinion. PROFESSIONAL FEES The standard fee for a 50-minute individual session is $125.00 and the standard fee for a 90 minute couples session is $175.00 You are responsible for paying the full amount at the time of your session, unless prior arrangements have been made. Payment must be made by cash, or credit card. If you refuse to pay your debt, I reserve the right to use an attorney or collection agency to secure payment. APPOINTMENTS Appointments will ordinarily be 45-60 minutes in duration, once per week at a time we agree on, although some sessions may be more, or less frequent as needed. The time scheduled for your appointment is assigned to you and you alone. If you need to cancel or reschedule a session, I ask that you provide me with 24-hours notice. If you miss a session without canceling, or cancel with less than 24-hour notice, my policy is to collect the full session fee of $125.00/individual or $175.00/couples. It is important to note that insurance companies do not provide reimbursement for cancelled sessions; thus, you will be responsible for 100% of the full rate. If it is possible, I will try to find another time to reschedule the appointment. In addition, you are responsible for coming to your session on time; if you are late, your appointment will still need to end at the scheduled time. Finally, I have the right to terminate treatment after 2 missed appointments or habitual tardiness. Thank you for understanding. INSURANCE I am not a participating provider for any insurance plan. If you would like, I will supply you with a receipt of payment for services, which you can submit to your insurance company for reimbursement. Please note that not all insurance companies reimburse for out-of-network providers. PROFESSIONAL RECORDS I am required to keep appropriate records of the SUD therapy/counseling services that I provide. Your records are maintained in a secured, encrypted, HIPAA compliant web-based system. I keep brief records noting that you were here, your reasons for seeking SUD therapy/counseling, the goals and progress we set for treatment, your diagnosis, topics we discussed, your medical, social, and treatment history, records I receive from other providers, copies of records I send to others, and your billing records. Except in unusual circumstances that involve danger to yourself, you have the right to a copy of your file. Because these are professional records, they may be misinterpreted and / or upsetting to untrained readers. For this reason, I recommend that you initially review them with me, or have them forwarded to another mental health professional to discuss the contents. You also have the right to request that a copy of your file be made available to any other health care provider at your written request.

  • Surgery Services This plan covers surgery services to treat a disease or injury when: • the operation is not experimental or investigational, or cosmetic in nature; • the operation is being performed at the appropriate place of service; and • the physician is licensed to perform the surgery. This plan covers reconstructive surgery and procedures when the services are performed to relieve pain, or to correct or improve bodily function that is impaired as a result of: • a birth defect; • an accidental injury; • a disease; or • a previous covered surgical procedure. Functional indications for surgical correction do not include psychological, psychiatric or emotional reasons. This plan covers the procedures listed below to treat functional impairments. • abdominal wall surgery including panniculectomy (other than an abdominoplasty); • blepharoplasty and ptosis repair; • gastric bypass or gastric banding; • nasal reconstruction and septorhinoplasty; • orthognathic surgery including mandibular and maxillary osteotomy; • reduction mammoplasty; • removal of breast implants; • removal or treatment of proliferative vascular lesions and hemangiomas; • treatment of varicose veins; or • gynecomastia.

  • Educational Services Any service or supply for education, training or retraining services or testing including: special education, remedial education; cognitive remediation; wilderness/outdoor treatment, therapy or adventure programs (whether or not the program is part of a Residential Treatment facility or otherwise licensed institution); job training or job hardening programs; educational services and schooling or any such related or similar program including therapeutic programs within a school setting.

  • Hospice Services Services are available for a Member whose Attending Physician has determined the Member's illness will result in a remaining life span of six months or less.

  • Other Directory Services 73.10.1 Both parties acknowledge that CenturyLink’s directory publisher is not a party to this Agreement and that the provisions contained in this Agreement are not binding upon CenturyLink’s directory publisher. 73.10.2 CenturyLink agrees to include critical contact information pertaining to CLEC in the “Information Pages” of those of its White Pages directories containing information pages, if CLEC meets criteria established by its directory publisher. Critical contact information includes CLEC’s business office number, repair number, billing information number, and any other information required to comply with applicable regulations, but not advertising or purely promotional material. CLEC will not be charged for inclusion of its critical contact information. The format, content and appearance of CLEC’s critical contact information must conform to applicable directory publisher’s guidelines and will be consistent with the format, content and appearance of critical contact information pertaining to all CLECs in a directory. 73.10.3 The directory publisher shall maintain full authority as publisher over its publishing policies, standards and practices, including decisions regarding directory coverage area, directory issue period, compilation, headings, covers, design, content or format of directories, and directory advertising sales.

  • Third Party Services Any services required for or contemplated by the performance of the above-referenced services by the Administrator to be provided by unaffiliated third parties (including independent auditors’ fees and counsel fees) may, if provided for or otherwise contemplated by the Financing Order and if the Issuer deems it necessary or desirable, be arranged by the Issuer or by the Administrator at the direction (which may be general or specific) of the Issuer. Costs and expenses associated with the contracting for such third-party professional services may be paid directly by the Issuer or paid by the Administrator and reimbursed by the Issuer in accordance with Section 2, or otherwise as the Administrator and the Issuer may mutually arrange.
