Cafeteria Benefits Plan. 3.1 Effective January 1, 1999, employee benefits will be offered to eligible employees through a Cafeteria Plan qualified under IRS Codes §105, §125, and §129. The cafeteria plan will contain a core set of benefits. Enrollment in these core benefits is required in order to participate in the cafeteria plan and receive any Employer contributions. Additional optional benefits are offered allowing employees to select benefits that meet their individual needs.
Cafeteria Benefits Plan. In addition to salary, the Employer offers a Cafeteria Benefits Plan to employees. The Employer shall contribute designated amounts as referenced below per month to each regular full-time employee dependent upon classification for health insurance and/or other benefits through the Cafeteria Benefits Plan. This takes effect thirty (30) days after the beginning of employment, or in the case of an employee beginning County service other than the first day of a month, it shall be thirty (30) days after the first of the month following the month in which he/she becomes employed. The employee must complete a Cafeteria Benefits Enrollment form at least fifteen (15) days prior to the effective date of eligibility for the plan. Regular employees working thirty (30) to forty (40) hours per week shall receive pro-rata portion of the benefit per month. Regular employees working less than thirty (30) hours per week are not eligible for participation in the Cafeteria Benefits Plan. Effective January 1, 2020, all employees who choose a single plan and were employed on or before December 31, 2019, shall receive an Employer contribution of one thousand, three hundred and twenty-five dollars ($1,325.00) per month. Effective January 1, 2021, all employees who choose a single plan and were employed on or before December 31, 2019, shall receive an Employer contribution of one thousand, three hundred fifty dollars ($1,350.00) per month. Effective January 1, 2022, all employees who choose a single plan and were employed on or before December 31, 2019, shall receive an Employer contribution of one thousand, three hundred seventy-five dollars ($1,375.00) per month. Effective January 1, 2020, all employees who choose a single plan and were employed after December 31, 2019, shall receive an Employer contribution of seven hundred and sixty-five dollars ($765.00) per month. Effective January 1, 2021, all employees who choose a single plan and were employed after December 31, 2019, shall receive an Employer contribution of seven hundred and ninety dollars ($790.00) per month. Effective January 1, 2022, all employees who choose a single plan and were employed after December 31, 2019, shall receive an Employer contribution of eight hundred and ten dollars ($810.00) per month. Effective January 1, 2020, all employees who choose a single plus children plan or a single plus spouse plan shall receive an Employer contribution of one thousand, three hundred fifty dollars ($1,350.00) per month....
Cafeteria Benefits Plan. The Employer shall provide each benefit-eligible employee with a combination of core benefits and a set dollar amount to apply to a variety of optional benefits based upon the employee’s health insurance election.

All full-time employees covered by this Agreement will have the opportunity to participate in the Village's Cafeteria Benefits Plan which will enable employees to select the type and kind of benefits they desire from among those offered by the Village on the same terms and conditions that are applicable to other Village employees who are participating in said Cafeteria Benefits Plan. If the total cost for the types and kinds of benefits selected by the employee exceeds that amount that the Village contributes, the amount that the employee is responsible for paying shall be done through payroll deduction.

2.1.1 Effective January 1, 2020, each eligible full-time employee with single coverage shall receive $861 per month which may be spent in a District-qualified cafeteria benefits plan. Each eligible full-time employee with family or single+1 coverage shall receive $1,300 per month.
2.1.2 Effective January 1, 2023, each eligible full-time employee with single coverage shall receive $941 per month which may be spent in a District- qualified cafeteria benefits plan. Each eligible full-time employee with family or single+1 coverage shall receive $1,395 per month.
Cafeteria Benefits Plan. “Educated Choices” Group Coverage

All employees covered by this Agreement will have the opportunity to participate in the Village’s Cafeteria Benefits Plan which will enable employees to select the type and kind of benefits they desire from among those offered by the Village on the same terms and conditions that are applicable other Village employees who are participating in said Cafeteria Benefits Plan. If the total cost for the types and kinds of benefits selected by the employee exceeds that amount that the Village contributes, the amount that the employee is responsible for paying shall be done through payroll deduction. Notwithstanding the above, for the 2018-19, 2019-20 and 2020-21 fiscal years the hospitalization insurance benefits provided to all employees covered by this Agreement shall be equal to the hospitalization insurance benefits extended to the non-supervisory police officers represented by the Chapter and such benefits shall be subject to the same terms and conditions and the same effective date(s).

In lieu of the provisions previously governing of hospitalization insurance, dental insurance, optical insurance, and term life insurance, all employees covered by this Agreement will have the opportunity to participate in the Village’s Cafeteria Benefits Plan which will enable employees to select the type and kind of benefits they desire from among those offered by the Village on the same terms and conditions that are applicable other Village employees who are participating in said Cafeteria Benefits Plan. If the total cost for the types and kinds of benefits selected by the employee exceeds that amount that the Village contributes, the amount that the employee is responsible for paying shall be done through payroll deduction.
Cafeteria Benefits Plan. 1. A represented, full-time employee who has health and dental care through another employer group plan, or through a spouse or domestic partner, may choose to waive his or her CRA/LA health and dental plan coverage and receive a taxable $100 monthly cash compensation.
2. A qualified employee may apply for this benefit by submitting a completed Cash-In-Lieu Affidavit to the Human Resources Department during the Cash-In-Lieu Open Enrollment Period. The initial open enrollment period will be for 45 days beginning on the first day of the month following the adoption of this MOU. Re-enrollment in a CRA/LA health plan will be allowed only if you experience a qualifying family status change (i.e. spouse/domestic partner loses health coverage) or during the Annual Employee Benefits Open Enrollment Period. A request for enrollment must be made within 30 days following a qualifying family status change. After the initial open enrollment period, future Cash-In-Lieu open enrollment periods will correspond with the medical benefits open enrollment periods.
