Doctor’s Note. If a Pack Member misses a day of work due to illness or injury, a doctor's note excusing the Pack Member for that particular date will excuse the absence and the Pack Member will not receive attendance points for the date in question. The note from the treating physician must be presented on the first shift back to work and have the dates which the Pack Member was under his/her care, a valid address and phone number for the doctor, and the doctor's name/signature. The note must contain any restrictions in detail the treating physician has prescribed or a notation of a full release to return to work. The Pack Member may be sent home if they do not provide a release to return to work. Doctor’s Note. In the case of absences for illness in excess of three consecutive days, a certificate from a physician, or if the treatment is by prayer or spiritual means, from the advisor or practitioner of such employee's faith, shall be required as a basis of pay during such leave. In any instance when a supervisor has a reasonable suspicion that an employee is abusing sick leave, he or she may demand a physician certificate after an absence of one day.
Doctor’s Note. It is agreed that the Employer will not automatically require doctor’s notes when employees call in sick.
Doctor’s Note. A written note from a duly licensed and practicing physician, dentist, chiropractor or a certified physician’s assistant (physician’s assistant is limited to when the period of illness or injury is seven (7) calendar days or less and when the employee is not hospitalized) containing his/her opinion of the cause, nature, extent and duration of the illness or injury shall be required to return to work if an employee is out sick for more than two (2) consecutive days.
Doctor’s Note. An employee may be required to produce a certificate from a medical practitioner for any health-related absence in excess of three (3) working days, certifying that he/she was unable to carry out his/her duties due to illness or injury.

E 2.04 The Company will ensure that all required paper work needed for medical leave or coverage will be filled out and processed as quickly as possible.
Doctor’s Note. In all instances, employees claiming entitlement to sick leave may be required to submit a doctor’s note. The note must indicate the medical problem and explicitly excuse the employee from work on each day absent. Notes that merely indicate that the employee had a doctor’s visit are not acceptable. Doctor’s notes may be required regardless of the number of days absent. This is true even if the employee is attending to a seriously ill immediate family member. In that situation, the employee may be required to supply a note for the immediate family member’s doctor indicating the degree of illness and the need for the employee’s absence from work each day the employee is absent. Failure to submit required doctor’s notes to the start of next scheduled workday may result in denial of paid sick leave and may also result in discipline up to and including discharge.
Doctor’s Note. The Company’s requirement of a doctor’s note in order to receive the 80 hour guarantee in the event of an absence due to an illness or injury is now restricted to members with a poor attendance history.
Doctor’s Note. The Employer may request a doctor’s note or other documentation from the employee upon the employee’s return to work after three (3) consecutive days off sick, or upon returning to work after being off sick on the last scheduled day before or after a holiday, or on a holiday that the employee was scheduled to work.
