ELIGIBILTY. (1) The following products are eligible for coverage: A. Fabric Furniture B. Area Rugs C. Leather, Vinyl & Nubuck Furniture, Suede D. Mattresses such as Innerspring mattresses that use a core support system made of metal springs or coils and box spring purchased at the same time as Covered mattress; Memory foam (polyurethane foam); and Hybrid mattresses combine an innerspring coil support system with memory foam.
ELIGIBILTY. (1) The following products are eligible for coverage:
ELIGIBILTY a. Only those teachers who contribute days to the Bank are eligible to borrow days from the Bank. Teachers who do not contribute an additional day to replenish the Bank are no longer eligible for Sick Leave Bank day loans. b. A teacher may make a request to borrow days from the Bank before all of his/her accumulated sick leave days are exhausted. c. A teacher must use all of his/her accumulated sick leave, personal leave, buy-out days, and take one (1) day without pay before borrowing days from the bank. d. A teacher cannot continue to borrow days from the Bank and also receive compensation from the Corporation’s long-term disability insurance policy or Worker’s Compensation. If a teacher is eligible to receive benefits from either of these sources, he/she must apply for those benefits before borrowing days from the Bank. e. Teachers are eligible to borrow days only during the period of time for which they are under contract to the Xxxxxxx-Xxxxxx-Xxxxxx Community School District and not during the summer or school vacations. The exception shall be teachers who have extended contracts. f. The reason for a request to borrow Bank days must be because of illness, injury, or incapacitation to the teacher and not a spouse, child, or relative. If the teacher applies to the Bank for such above-named reason, then it is assumed that the teacher is not well enough to attend school or extra-curricular functions. g. Until a Sick Leave Bank loan is paid back, a teacher who borrows sick leave bank days will pay back these days by having two (2) days of each subsequent school year’s sick leave days allotment withheld as credit against the Sick Leave Bank loan. If a teacher xxxxxx employment with Xxxxxxx-Xxxxxx-Xxxxxx Schools, then any unused sick day leave shall be forfeited to the Bank, to be applied against the remaining balance of the Sick Leave Bank loan. If a teacher has not repaid all of the borrowed days at the time employment is severed, then the remaining days owed to the Sick Bank will be charged at the current sub rate. That amount will be deducted from the teacher’s final pay. The days thus repaid will be re-instated to the Sick Bank. h. The above notwithstanding, no teacher may receive Sick Leave Bank day loans which would cause his/her total outstanding loan to exceed fifty (50) days. i. A “pay back” day or a borrowed day may be credited or paid, as applicable, in full or half day units.

(2) Products must have at least a ninety (90) day(s) valid manufacturer warranty in effect when plan is purchased.
ELIGIBILTY. This applies only to employees as of June 30, 2005. An employee must be no less than fifty-five years of age by July 1 of the year the employee applies for the ERIP. Furthermore, the employee must have nineteen years of teaching experience of which 12 years must be in the South Xxxxxxxxxx Community School Corporation. This applies only to employees as of June 30, 2005.
ELIGIBILTY. MANAGER who is eligible for group health and long-term disability insurance may, after initial employment and following a thirty (30) day waiting period from the first day of the month following CITY employment, enroll in the CITY's group health insurance plan, which includes dental, vision, life insurance, and long-term disability insurance plan, provided such MANAGER is not excluded from enrollment by conditions of the insurance contract.

ELIGIBILTY a. The Apartment is designated as non-First Year, single student housing, unless otherwise approved by UCR-HS and Owner. Married occupants and occupants in a domestic partnership may not occupy rooms, be they shared or unshared with other assigned occupants, in the assigned community. b. Children/minor dependents may not reside in the premises and students with children/minor dependents are not eligible for residency in the Apartment. c. Resident must be regularly enrolled and remain a full-time registered UCR student at all times as defined by the Registrar’s Office. d. Resident is required to give Owner a thirty (30) day prior written notice in the event Resident will cease to be a currently-enrolled student in the next quarter by reason of graduation, termination of student status for more than one quarter, or transfer to another educational institution. The failure to give such notice shall result in the assessment of 30 days pro rata rent or $75.00, whichever is greater, as a liquidated damages charge to cover the Owner’s administrative costs. e. A one-quarter Leave of Absence may be approved by Owner upon request if Resident conduct and financial record are in good standing. A Leave of Absence may be requested for less than full-time enrollment, filing fee status, or withdrawal from the University. A Leave of Absence may not be requested for Resident’s initial quarter of occupancy or extend occupancy beyond the graduation date on file with the University. Only one housing-related Leave of Absence shall be granted while Resident is a UCR student. Residents with an approved Leave of Absence are required to present evidence of registration for the next regular session quarter or submit a formal Contract Cancellation Request at least 30 days prior to the end of the quarter in which the Leave of Absence has been approved. During a Leave of Absence, payments must continue to be paid in accordance with the Payment Schedule. f. The Owner and UCR-HS reserve the right to permanently exclude from campus housing a Resident who has been legally evicted from any campus housing unit or whose Contract has been otherwise terminated by Owner. Excluded Residents are not permitted to enter/return to the buildings or grounds of any campus housing facility for any reason.
