Evaluation Components. 8.3.1 Contract faculty are expected to go through a four year tenure process. At the end of the fourth year, a decision to grant or not grant tenure must be made. In exceptional cases, tenure may be recommended in fewer than four years. 8.3.2 The supervising administrator, under direction of the appropriate vice-president, is responsible for overseeing the evaluation process on behalf of the Board. The supervising administrator will oversee the collection of all evaluation materials and will prepare the annual evaluation report, which will include a recommendation regarding the continued employment of the faculty member. 8.3.3 The evaluation record will include the following components: 1. A Self Evaluation: Strengths and Areas for Improvement 2. Student Evaluation Surveys: Numerical Data and Typed Written Comments 3. Classroom observations by the supervising administrator and two tenured faculty peers (selected by the department chair, or, if necessary, in consultation with the academic senate) 4. Colleague surveys (which must include all members of the department, plus other appropriate faculty, administrators and/or staff recommended by the peer evaluators and selected by the administrator): Numerical Data and Typed Written Comments 5. Administrative Summary Evaluation Report/Administrative Recommendation (by the supervising administrator) 6. A Performance Improvement Plan if necessary (see 8.8.5) 7. Additional evaluations if necessary (see 8.17) 8.4 SELF-EVALUATION - PORTFOLIO 8.4.1 The probationary faculty member will compile a self-evaluation portfolio to be kept in the supervising administrator’s office which will include a written self-evaluation indicating her/his perceived strengths and/or areas for improvement, appropriate class materials (syllabi, sample assignments, etc.), and other pertinent documents (copies of publications, verification of staff development completion, awards, etc.)

Evaluation Components. 5.4.2.1 The evaluation shall consist of a completed final Report of Performance form for supervisors. The evaluation may also include the Supervisor’s Self-Evaluation/Staff Feedback Survey form obtained from appropriate college/district contacts. The primary rater may also ask the supervisor to complete one of the Supervisor’s Self-Evaluation/Staff Feedback Survey forms. 5.4.2.2 The final evaluation will be completed by the manager based on his/her observation and findings, as well as the results of the Supervisor’s Self- Evaluation/Staff Feedback Survey, if included. 5.4.2.3 The audience for the Supervisor’s Self-Evaluation/Staff Feedback Survey will be mutually agreed upon by the manager and supervisor; the survey will be distributed to a sample of persons served by the supervisor and his/her office. Such surveys may be distributed to other supervisors, managers, faculty, and classified staff with whom the supervisor interacts. 5.4.2.4 The immediate manager shall be responsible for conducting the supervisor’s survey, but the manager may designate another person to distribute the survey with the approved cover memo or email. The surveys will be submitted online or returned to the office of the manager where the survey ratings and written comments will be summarized. 5.4.2.5 The ratings may be categorized separately by employee groups (supervisor, classified, faculty, management) if five (5) or more responses are received from each of the groups; otherwise, the ratings will be combined into one (1) category.

Evaluation Components. A. The Grantee shal l improve the immunization coverage rate for children during the grant period to a minimum level of 90% or higher. B. The Grantee sha ll improve the immunization coverage rate for adolescents during the grant period and maintain at 90%. C. The Grantee shal l minimally achieve outreach education for influenza, shingles, and pneumococcal vaccines for 80% of the adult population 65 years of age and older with the ultimate goal of meeting the 90% objectives set by Healthy People 2020 by 2020 or sooner. D. The Grantee sha ll complete investigations for 100% of all VAERS suspected cases reported in accordance with Department approved guidelines. E. The Grantee shal l ensure Immunization Clinics and vaccine safety protocols are in compliance with the Department Vaccine Accountability System guidelines as verified by the Department Quality Assurance staff during annual site visits. F. The Grantee sh all conduct appropriate protocol follow up for 100% of all identified infants born to Hepatitis B Surface Antigen positive females and their contacts. G. The Grantee shal l improve the immunization coverage rates in identified PONs for children and adults each Grant year and maintain at 80% or higher when that level is attained. H. The Grantee sh all promote an active coalition working toward improving immunization coverage levels across the lifespan by providing education and outreach activities that support and enhance the Grantees Immunization Program. I. The Grantee sh all maintain linkage to PA-SIIS and successfully enroll one FQHC, one RHC and two private providers in PA-SIIS as required by stage one meaningful use for public health reporting of immunization data. J. The Grantee sha ll ensure populations are targeted, outreach activities conducted and type and volume of educational materials distributed as provided to the Department in the Immunization Annual Highlights Report (AHR).
