The Formal Evaluation Procedures clause establishes a structured process for assessing whether certain standards, deliverables, or performance criteria have been met within a contract. This response and summary of constituency evaluations will be available to the Library Faculty Member at least two working days before his or her formal evaluation conference.

A. Conference and Copy of Evaluation A copy of each formal evaluation shall be given to the employee and a conference shall be held between the employee and the immediate supervisor. B. Responses If the employee feels that his/her formal evaluation is incomplete, inaccurate, or unjust, he/she shall have fourteen (14) days to put his/her objections in writing and have such attached to the evaluation report to be placed in his/her evaluation file.

4.10.2 The immediate supervisor and the faculty member shall sign the team evaluation form. The faculty member’s signature indicates only that he/she has read the contents of the evaluation.
4.10.3 If the faculty member does not sign the formal evaluation, the evaluating supervisor will forward the original with his/her signature and date with a cover letter indicating the faculty member did not respond. The evaluator will give a copy of the letter and evaluation to the faculty member.
4.10.4 The original copy of the team evaluation form shall be forwarded to the Human Resources Department in a timely manner for inclusion in the faculty member’s personnel file. If the member under review wishes to respond to the evaluation, he/she may submit a response to be attached to the evaluation in the personnel file. Evaluations will be held for ten (10) days before filing to allow for response prior to filing. Responses may also be submitted at any time for addition to the file. Peer evaluation forms may be added to the personnel file upon the faculty member’s request, but otherwise become the property of the evaluatee.
Formal Evaluation Procedures. THE ODE approved OTES evaluation rubric shall be used in calculating the Teacher Performance fifty- percent (50%). For non-OTES unit member’s formal written evaluations will be on forms provided by the local Superintendent. Each teacher evaluation conducted under this policy shall be conducted by a building administrator who holds a credential established by the Ohio Department of Education for being an evaluator and is employed by the Board or assigned to work in the district on a regular/full-time basis.

c. Review of the evaluation instrument and clarification of specific elements to be included in the evaluation.
d. Agreement as to the general time of the evaluation visit.
e. Discussion of other matters that might be pertinent to the process.
