Insurance Waiver. Any of the terms or conditions of this Article 10 may be waived by the City’s Risk Manager in writing, signed by the Risk Manager, and attached to this Agreement as Appendix F. Such waiver is fully incorporated herein. The waiver shall waive only the requirements that are expressly identified and waived, and under such terms and conditions as stated in the waiver.

Insurance Waiver. A. Any certificated/licensed staff member who waives the right to health insurance for a benefit year (January 1 through December 31), and provides proof of other insurance coverage that is not through the Health Insurance Marketplace, will receive a lump sum payment on the second pay period following the completion of that benefit year that shall be equal to, for those on single coverage, $1,500.00, and for those on family coverage, $3,000.00. If the employee resigns his/her employment with the District, the employee will not receive the waiver amount, unless the resignation was for retirement purposes. In the case of a retirement, the waiver amount will be prorated based on the date of the retirement. B. In the event of a change in the need for coverage due to a major life event causing the cessation of the teacher's alternate source of coverage during the waiver year (death of spouse, divorce, dissolution, loss of spouse's employment, etc.), the teacher may reinstate coverage effective the first day of the following month without having to meet any pre-existing condition requirement provided the proper application card has been completed and submitted to the Treasurer. Such entry into the insurance program will preclude the bargaining unit member from receiving the healthcare insurance waiver payment in lieu of coverage as indicated during the waiver year. C. Should a certificated/licensed staff member elect to participate in the program, the certificated staff member must complete a written notification of waiver and verification of alternative coverage to the Treasurer during open enrollment or at the time of hiring. Such waiver shall remain in effect from year to year unless changed by the member.
Insurance Waiver. Sublessee shall procure and maintain all insurance policies required by the tenant under the Master Lease with respect to the Premises. All such liability policies shall name Sublessor and Master Lessor as additional insureds. Notwithstanding anything to the contrary herein, Sublessor and Sublessee hereby release each other, and their respective agents, employees, subtenants, assignees and contractors, from all liability for damage to any property that is caused by or results from a risk which is actually insured against or which would normally be covered byall riskproperty insurance, without regard to the negligence or willful misconduct of the entity so released.
Insurance Waiver. Employees who choose to waive their health insurance on or after January 1, 2012 will receive a cash payout equal to 20% of the cost of the premium for single coverage, provided that such payout is permissible under the City’s health insurance contracts and applicable laws. The employee must prove proof of acceptable alternative health coverage and apply for payment in accordance with the written procedures provided by the Personnel Manager. Payments for waivers of less than twelve (12) months will be prorated. Payments will be made no less than once a year.

An employee eligible for family health insurance coverage may decline Board Insurance coverage and receive an annual payment of $2,000 as long the employee takes insurance coverage other than the benefit package offered by the Board. 2. An employee eligible for family health insurance coverage may elect to take single insurance coverage and shall receive an annual payment of $1,000. 3. Employees who participate in the above insurance waiver must notify the Board Treasurer in writing no later than the last day of the open enrollment period of any year that they wish to decline coverage. 4. Any employee who has elected to participate in the insurance waiver and during the year loses insurance coverage through divorce, death, job loss, layoff, etc. shall be provided Board insurance coverage upon notification of the Treasurer. The employee will return a prorated portion of the annual payment. 5. An employee electing to take the insurance option shall receive payment no later than 90 days after days after the close of the open enrollment period. 6. The insurance waiver option shall be annual.
Insurance Waiver. Eligible bargaining unit members who elect to forego all insurance coverage provided by the Board (healthcare and prescription drug only) shall be eligible for a one-time annual payment of $2,000. An eligible bargaining unit member’s insurance coverage waiver shall apply to one-full 12-month year. The $2,000 annual payment shall be paid in 50% installments twice during the relevant 12-month period (May and November) after the bargaining unit member completes their employment for which the insurance coverage would have been in effect. Eligible bargaining unit members who complete a partial employment year will receive a prorated payment commensurate with the partial employment year. This provision applies only to those bargaining unit members determined to be eligible for insurance coverage as defined in this Article.
Insurance Waiver. Members of the bargaining unit who are eligible for insurance coverage under this section may voluntarily elect to waive such coverage, in whole or in part, provided that such waiver does not conflict with the rules regulations or requirements of the appropriate insurance carrier(s). Teachers electing such a waiver shall receive cash payments from the New London Board of Education in an amount equal to 25 percent (25%) of the insurance cost which would have been charged to the Board on their behalf had the insurance coverage not been waived. In order to exercise this waiver option, teachers must apply in writing to the Superintendent of Schools not later than June 20 in any year covered by this Agreement with such waiver to be effective the following September 1. All waiver applications must be completely voluntary on the part of the staff member and must be accompanied by a signed waiver of coverage document acceptable to the Board of Education. If the waiver application is acceptable to the applicable insurance carrier(s), it shall be effective as of September 1 and shall remain in effect until revoked as provided below. Teachers whose waiver applications are acceptable to the applicable insurance carrier(s) shall receive quarterly cash payments from the New London Board of Education in September, November, February and June. There shall be an annual open enrollment window of May 20 - June 20 of each year. At this time an employee may choose to reinstate or waive his/her insurance for the fiscal year commencing September 1. There shall be no reinstatements other than the May 20 - June 20 period, however, there shall be no lapse of coverage if an employee rescinds his/her waiver due to: 1) Coverage was waived by the employee because another group health 2) Coverage is lost under that plan due to employment termination, death of a spouse, or divorce; and 3) The employee applies for coverage under this contract within thirty (30) days after the loss of coverage under the other plan.

When both spouses are employed by the District and are enrolled in the District’s health insurance plan, neither spouse is entitled to receive the insurance waiver payment set forth in this Section. 2. Insurance Waiver. 1. Notwithstanding the above, effective with the execution of this agreement, the Employee may voluntarily elect to waive in writing all health insurance coverages outlined above and, in lieu thereof, shall receive an annual payment of three thousand dollars ($3,000) for family or member plus one election or fifteen hundred ($1,500) for individual coverage. Payment to the Employee waiving such coverage shall be made at the conclusion of the fiscal year during which insurance was waived and is subject to applicable tax laws. 2. Where a change in the Employee’s status prompts the Employee to resume Board-provided insurance coverage, the waiver may, on written notice to the Board, be revoked. Upon receipt of revocation of the waiver, insurance coverage shall be reinstated as soon as possible; subject, however, to any regulations or restrictions, including waiting periods, which may then be prescribed by the appropriate insurance carriers. Depending upon the effective date of such reinstated coverage appropriate financial adjustments shall be made between the Employee and the Board to ensure that the Employee has been compensated, but not overcompensated, for any waiver elected in this section. 3. Notice of intention to waive insurance coverage must be sent to the Superintendent no later than April 1, to be effective in the following contract year. 4.
