a. A copy of the receipt and a grade report for each course attached to the reimbursement form
b. Receipts must be printed with name, date of course or indication of school term, and detail of tuition paid.

d. Reminder: fees (i.e. technology, parking, graduation, etc.) will not be considered for reimbursement.
2. Reimbursement applications are due by October 20 of the current year for courses completed in the previous school year and April 1 for courses completed in the current school year.
3. All clock hours are eligible for reimbursement. This may include clock hours earned for completion of National Board components, professional growth plans, or other professional development hours that are accessible through PD Enroller.
4. All course information must be listed on the second page of this application. Please include additional pages as necessary.
5. Please remember that for reimbursement, all credits and clock hours will be converted to quarter credits.
