Life Insurance, Accidental Death & Dismemberment i) Life Insurance coverage shall be equal to two (2) times annual salary to the nearest $1,000.00 benefit;
ii) Accidental Death and Dismemberment coverage shall be equal to the life insurance;
iii) All coverage ceases at termination of employment or retirement, but in any event at age seventy (70).

Life Insurance, Accidental Death & Dismemberment. 1. The provisions of the benefit plans described below are fully set forth in the formal Plan Document(s) (including any insurance policy documents). In the event of any difference between the Plan Document(s) and anything stated in this Agreement or in any benefits summary provided to employees, the Plan Document(s) will control, and no provision of this Agreement or of any such benefits summary can amend, supplement or otherwise change the Plan Document(s). PROVIDED, however, that the current level of benefits as of the date of ratification of this Agreement will not be reduced during the term of this Agreement.
2. Employees working at least 30 hours per week may enroll in CPI’s accidental death and dismemberment coverage and life insurance benefit programs. Life and accidental death and dismemberment insurance is double an employee’s salary up to $400,000. Internal Revenue Service regulations currently require employees who receive more than $50,000 in life insurance to pay taxes on this benefit. The amount for an employee’s coverage is added to the employee’s gross pay each pay period.

If a member of this group insurance plan becomes totally and permanently disabled from a sickness or accident before the age of years, after a waiting period of months and as long as they remain totally and permanently disabled they will be eligible to receive the amount of this Life Insurance in monthly instalments spread over a year period.

If a member of this group insurance plan becomes totally and permanently disabled from a sickness or accident before the age of years, a waiting period of six (6) months and as long as they remain totally and permanently disabled, which prevents them from performing work of any kind, they will be eligible to receive the amount of this Life Insurance in monthly installments spread over a month period. This benefit is availableto all members and is a condition of on Company trawlers. The coverage is effective on completion of the first trip to sea. The benefits are payments of per week. Weekly sickness benefits start on the fourth day, non-industrial accident benefits start on the first day; both continue through to weeks of disability. This means that the first weeks of disability are covered by this plan. The next weeks are covered by (Industrial accidents are, of course, covered by Workers’ Compensation and not by this policy). This benefit continues to age if still on Company vessels. This benefit is available to all members and is a condition of on Company trawlers. The coverage is effective in the month following the first premium paid. Specifics of the plan will be found in the Group Benefit Plan Booklet which will be provided to each employee.
