Nursing Home Transfers. The Employer agrees that an employee of a different facility listed in Schedule below who is a member of a bargaining unit for which the CAW is the recognized bargaining agent may apply for a job posting at Carlingview Manor, Ottawa, at her own expense, subject to the following conditions: Said employee shall provide the management Carlingview Manor, Ottawa a written request for a job application. Such written request shall constitute an application for a job posting at Carlingview Manor, Ottawa and shall exist for a period of one calendar year from the date it is actually received by Carlingview Manor, Ottawa. The application shall include the position at the facility she is working at the time of the application and her qualifications and the position for which she is applying, agree to recognize the employee’s seniority which the employee has at the facility at which she is working at the time the decision is made by the Employer about the job posting. In the event the employee who made the written request is the successful applicant for the job posting, then: The Employer will recognize the seniority and years of service of the employee at the facility at which she is working at the time of the hiring at Carlingview Manor, Ottawa, excluding Manor (Thunder Bay) and Xxxxxxxxx The employee will otherwise be subject to the terms of the collective agreement between CAW and Carlingview Manor, Ottawa; The employee will be deemed to have quit at the facility she is leaving to accept the job posting at Carlingview Manor, Ottawa. Notwithstanding the foregoing, an employee will not be entitled to receive more vacation than she would otherwise have if she had not accepted the position at Carlingview Manor, Ottawa. In the event the employee who applied for the job posting according to the above is terminated with cause after being offered the posted position but before commencing to work at Carlingview Manor, Ottawa, then the offer of will be deemed to be a nullity and the job will be to the employee who next would have been offered the job, if any. Any grievance about such a termination shall be made according to the terms of the collective agreement in effect at the facility in which the employee was working at the time of the discharge. Nursing Home Transfers. An employee leaving the employ of the Employer at the Albion Road Nursing Home (as a result of a change in residence) who applies for a similar position within forty-five (45) days thereafter and is accepted for employment at the Employer’s Nursing home in Thunder Bay, Ontario, shall retain his seniority for the purpose of vacation entitlement, and wage level based on seniority in accordance with overall Company seniority. The employee shall advise the Employer when making such application that he has previously been employed by the Employer at the Albion Road Home.

[b] If an applicant is permitted to transfer from one Extendicare Nursing Home to another as a result of this transfer procedure, she will retain any service that she had previously accrued. The applicable wage rate shall be paid according to the position to which the employee transferred. However, an employee so transferring will exercise bargaining unit seniority accrued at the new facility for purposes of transfers, promotions, lay-offs and reductions in hours.

  • Federally Qualified Health Centers (A) The Contractor shall not restrict an Enrollee’s right to obtain FQHC services outside the PMHP through the Fee For Service Medicaid program.

  • Cemetery The Local Church agrees and insures that, after the Disaffiliation Date, its cemetery and/or columbarium, will continue to be maintained in substantially the same manner as presently maintained. In addition, Local Church will honor any and all contracts, deeds, and agreements for burial and/or internment in its cemetery or columbarium, as well as insuring and continued access for families and loved ones of United Methodists buried there and for burials in unfilled xxxxxx and columbarium slots (including granting an access easement to the Conference and members of the United Methodist Church for visitations, historical research, and related purposes).

  • Orthodontics We Cover orthodontics used to help restore oral structures to health and function and to treat serious medical conditions such as: cleft palate and cleft lip; maxillary/mandibular micrognathia (underdeveloped upper or lower jaw); extreme mandibular prognathism; severe asymmetry (craniofacial anomalies); ankylosis of the temporomandibular joint; and other significant skeletal dysplasias. Procedures include but are not limited to: • Rapid Palatal Expansion (RPE); • Placement of component parts (e.g. brackets, bands); • Interceptive orthodontic treatment; • Comprehensive orthodontic treatment (during which orthodontic appliances are placed for active treatment and periodically adjusted); • Removable appliance therapy; and • Orthodontic retention (removal of appliances, construction and placement of retainers).

  • Outpatient Dental Anesthesia Services This plan covers anesthesia services received in connection with a dental service when provided in a hospital or freestanding ambulatory surgical center and: • the use of this is medically necessary; and • the setting in which the service is received is determined to be appropriate. This plan also covers facility fees associated with these services.

  • Health Services At the time of employment and subject to (b) above, full credit for registered professional nursing experience in a school program shall be given. Full credit for registered professional nursing experience may be given, subject to approval by the Human Resources Division. Non-degree nurses shall be placed on the BA Track of the Teachers Salary Schedule and shall be ineligible for movement to any other track.

  • Prosthodontics We Cover prosthodontic services as follows: • Removable complete or partial dentures, for Members 15 years of age and above, including six (6) months follow-up care; • Additional services including insertion of identification slips, repairs, relines and rebases and treatment of cleft palate; and • Interim prosthesis for Members five (5) to 15 years of age. We do not Cover implants or implant related services. Fixed bridges are not Covered unless they are required: • For replacement of a single upper anterior (central/lateral incisor or cuspid) in a patient with an otherwise full complement of natural, functional and/or restored teeth; • For cleft palate stabilization; or • Due to the presence of any neurologic or physiologic condition that would preclude the placement of a removable prosthesis, as demonstrated by medical documentation.
