Online Survey. SmithGroup will prepare an online survey to gather additional input from city residents and parklands uses to expand outreach beyond the public meeting attendees. The survey will focus on gathering input on existing and potential uses, needs and goals. It will utilize an interface that mimics the public input experience, utilizing imagery and graphics with an opportunity for open-ended input. We anticipate the survey being live for three weeks following the public input meeting.
Online Survey. An online survey will be conducted to inform and gather feedback on the plan priorities. The survey would gather general input on transportation needs and priorities. Survey results would be analyzed and summarized to identify key outcomes.

Online Survey. The teachers at School A, School B, School C, and School D were contacted to participate in the study. The survey, Teacher Survey – Mathematics and PLC, was sent to the teachers via email and respondents were limited to one submission. An online survey was utilized as the instrument for data collection because it is the most effective method for “both response rate and reaching a high number of participants” (XxXxxxxx & Xxxxxxxxxx, 2010, p. 241). All responses were recorded on a Google Form that was exported into an excel spreadsheet. Each section of the instrument was developed to address the study’s research questions. A goal of the survey was to identify participants’ level of understanding of their comfort and background in mathematics, instructional practices and beliefs about mathematics, and Professional Learning Communities at their school site. For the survey, 25 questions were adapted from the Trends in International Mathematics and Science Study (TIMSS) Teacher Questionnaire facilitated by the United States Department of Education as part of the instrument. The purpose of TIMSS is to “study the home, community, school, and student factors associated with student achievement in mathematics” (United States – TIMSS 2015 Encyclopedia, 2016). Data collected is to understand each country’s context for learning through questionnaires that were completed by students, as well as their parents, teachers, and principals. All questionnaires were reviewed by the National Research Coordinators and by each country (United States – TIMSS 2015 Encyclopedia, 2016). The first section of the teacher survey for this research included six questions regarding the participant’s demographics and Likert scaled questions. The Likert scaled questions were adapted by the TIMSS and asked the participant’s opinion on student learning, teaching responsibilities beyond the classroom, and job satisfaction. Likert-scaled questions ranged from never (1), neutral (3), and always (5) and the job satisfaction question consisted of the following responses: never or almost never (1), rarely (2), sometimes (3), often (4), always or almost always (5). The information gathered in this section will be used for research question one. Section two consisted of twelve questions regarding teaching mathematics. Online Survey. Each participant was provided a unique, 4-digit ID number. This code was used by participants to access the Qualtrics-formatted online survey. Participants were queried about their sex, race and ethnicity. In addition, women were asked “Are you taking any form of hormonal contraceptive/birth control?” and if they answered “yes” they were asked to specify the method, (“What kind of hormonal contraception are you taking?”), and provided with options that included oral contraceptives (i.e. pill), hormonal patch or injection, intrauterine device (IUD), or implant. The survey included six different inventories of person-factors that could plausibly be involved in hormonal and/or psychological responses to academic stress (see below). The entire survey took approximately 15 – 20 minutes to complete. Participants had two weeks from the time they completed the first sample to before the first exam to complete the survey.
Online Survey. The Xxxxxx team will develop a simple but effective online survey (e.g., Survey Monkey) that will include a range of questions that will build upon the public outreach in Task 1. The survey is expected to include specific questions regarding the existing plan, potential goals, questions about new policies related to sustainability, housing, etc., or any number of questions that can help modify or further refine the city’s vision as a part of Task 3. We typically incorporate images as part of the survey to ask the public about their ideas on community character and urban design. Surveys can be done with multiple questions in one survey or several surveys that have one to two questions each depending on how well staff believes the public will react to surveys.

Online Survey. Transpo will develop an online survey for City review. Transpo work on this task will involve developing an online survey that can be linked to a City landing page and/or online open house. The online survey can also be developed as a paper handout that is mailable back to the City, if needed. The online survey can also be used to recruit for a targeted focus group.
