Project Experience. 150 points available:
1. Experience with K-12 Schools and/or Higher Education facilities in California.
2. Experience with projects for Public Agencies in California.
3. Experience working on a campus while school is in session.
4. Experience relevant to the scope of services described in this RFQ Applicants may list projects that may not match the exact criteria listed, however, projects listed are to be representative of the Applicant’s ability to perform Hazardous Materials Assessment services. A majority of the projects Applicant lists in the Applicant and Sub- Consultant History & Project Experience Form (RFQ, Attachment 3) shall be Applicant’s history and project experience, not Applicant’s Sub-Consultant’s history and project experience. i.e. the Applicant must submit at least six (6) or seven (7) and the Sub- Consultants at least three (3) or four (4) (that is a total for all Sub-Consultants combined), for a total of ten (10) projects. If there are no Sub-Consultants, the Applicant must submit ten (10). Do NOT submit more than ten (10) projects. Projects listed that do not match exact criteria requested, may receive fewer points by evaluators. Each of the ten (10) projects submitted will be evaluated and scored by the Evaluation Committee based on the factors listed above and reflected in Attachment 3 – Applicant Team Project History and Performance and awarded from 0 to 15 points for each of the ten (10) projects submitted. If less than ten (10) projects are submitted, it reduces the number of available points to an Applicant. (Example: with ten (10) projects submitted, there are up to 150 points available (i.e., 10 projects x 15 points = 150). However, if only nine (9) projects are submitted, there would be only 135 points available (i.e., 9 projects x 15 points = 135 available points), etc. Do NOT submit any more than ten (10) projects total.
Project Experience. The Xxxxxx XX team has provided on-call consulting services to various public agencies since 2001. Our personnel work very closely with your team in order to effectively function as an extension of your staff. Xxxxxx XX will prepare and enforce all contract specifications to confirm that the City of Avondale receives the expected level of quality and value from contractors. Xxxxxx XX provided full-time inspection services for the Avondale Public Works Department as part of the City’s Historic Avondale Enhancement Project. The scope of work included new sewer lines replacing the previously undersized system. A new upsized waterline was also being installed to provide better service to area residents and businesses as well as increased fire protection. These improvements were within Central and Western Avenues, making traffic control and monitoring of public safety a high priority. Xxxxxx XX provided construction management and inspections for a complete replacement of the booster pump station at the Northside Arsenic Treatment Facility. The new station was designed to accommodate phased expansion and possible pressure zone modification. Improvements included four new booster pumps, new yard piping, new hydropneumatic tank, and complete electrical and control systems. Schedule control was critical on this project to ensure that it could be constructed during the winter off- peak demand period. Avondale 127th Avenue Waterline Improvements Xxxxxx XX is providing construction management and inspection on the installation of 3930 feet of 16 inch, 84 feet of 12 inch, and 1242 feet of 8 inch water main, service lines, fire hydrants, the construction of a pressure reducing valve station, and pavement replacement. The 127th Avenue Waterline Improvements project is located on 000xx Xxxxxx at Vermeerch Road, County Line Road, and Pioneer Road. on Xxxxxx XX provided constructi n management and inspections for Xxxxxx Road improvements between Western Avenue and Van Buren Street. g nd Improvements included new sidewalk, driveway entrances, and pedestrian/parkway lightin on both sides of the street a d as landscaping. Project Experience. The City of Placentia has experience administering similar projects through the State of California Department of Parks and Recreation 2000 and 2002 Parks Bond Act Per Capita, State of California Safe Routes to Schools and Orange County Transportation Agency Measure M Funding. The City of Placentia completed a $2 million dollar construction renovation project to improve the 9,000 square foot Xxxxxxx Community Center facility that provides social services, health services, afterschool programs, recreational classes, and classes to assist low-income adults and families in the La Jolla Neighborhood. The project to renovate this Neighborhood Facility included use of the State of California Department of Recreation 2000 and 2002 Bond Act funds along with leveraging of other gran sources and City General Funds. The project was completed in 2003 and involved relocating the Human Services Office to the facility, upgrading restrooms and walkways in the entire facility to full ADA compliance, creating a courtyard area outside of the main multipurpose room, and creating additional tutoring and multipurpose rooms in the facility. The State of California Department of Parks and Recreation 2000 Parks Bond Act Per Capita consisting of $455,000 grant funds. The funds were utilized to renovate both City owned pools to bring both facilities up to date with current safety standards and chemical storage standards; to renovate three city park playgrounds; upgraded the park lighting of our largest community park which consisted of 18 lamps from 9500L to 22000L and the installation of two new lamps and made improvements to another public park/facility which included removal of wood building structure and concrete pad, installation of new turf and new picnic tables. State of California Routes to Schools Grant Funding. The purpose of Safe Route to School Funding is to improve pedestrian safety and reduce congestion at local intersections. Improvements consist of traffic signal installation street widening construction of curb, gutter and sidewalk, handicap access, and bike lanes. The Van Buren Project was allocated 7/23/03, completed 1/31/07 - $231,000 and Xxxxxxx/Patrician Project was allocated 7/11/06, completed 12/14/2008 - $177,000. Applicants may list projects that may not match the exact criteria listed, however, projects listed are to be representative of the Applicant’s ability to perform Commissioning services. A majority of the projects Applicant lists in the Applicant and Sub-Consultant History & Project Experience Form (RFQ, Attachment 3A) shall be Applicant’s history and project experience, not Applicant’s Sub-Consultant’s history and project experience.
Project Experience. Provide a brief description of facility analysis (Task 1 of Scope) and facility design experience (Task 2 of Scope). Provide five examples of each Task that are ongoing or were completed in the last 5 years. List contact information for each facility cited in your experience.
Project Experience. The City of Los Alamitos has participated in the CDBG program for many years. In 2010- 11, the City received a CDBG grant of $200,000 for an alleyway project which was completed successfully. In 2011-12 the City received a CDBG grant of $88,058 for handicap access ramps which was completed successfully. We were also awarded a $146,325 grant for 2012-13 for ADA ramps and curbs which has been completed. In 2014-15 the City was awarded $126,000 for the Apartment Row Alley Improvement project. In 2015-16 and in 2016-17, the City was awarded $135,000 for the continuation of the Apartment Row Alley project and $90,000 in FY2017-18 for Cerritos Street Improvements. The City has a good track record in working with the County of Orange.
