Reports and Reviews. The Recipient shall: (a) maintain policies and procedures adequate to enable it to monitor and evaluate on an ongoing basis, in accordance with indicators satisfactory to the Association, the carrying out of the Project and the achievement of the objectives thereof; (b) prepare, under terms of reference satisfactory to the Association, and furnish to the Association, on or about eighteen (18) months after the effectiveness date, a report, to constitute the basis upon which the mid-term review shall take place, integrating the results of the monitoring and evaluation activities performed pursuant to paragraph (a) of this Section, on the progress achieved in the carrying out of the Project during the period preceding the date of said report and setting out the measures recommended to ensure the efficient carrying out of the Project and the achievement of the objectives thereof during the period following such date; and (c) review with the Association, no later than three (3) months after submission, or such later date as the Association shall request, the reports referred to in paragraph (b) of this Section, and, thereafter, take all measures required to ensure the efficient completion of the Project and the achievement of the objectives thereof, based on the conclusions and recommendations of the said report and the Association’s views on the matter. Reports and Reviews. 4.2.1 PTLGateway will use industry standard tools to monitor and automatically report on the levels of service that are being delivered to Customer. The reports will include statistics on Network, application and support Calls. 4.2.2 Monthly and/or weekly Service Level Agreement reviews will be scheduled with Customer where performance and service Levels will be discussed. These reviews may take the format of a telephone call to Customer's Representative by XXXXxxxxxx’s Representative, or a more formal meeting between PTLGateway and Customer at a mutually convenient location.

Implementation requirements Can produce most recent annual report, financial statements and/or latest financial audit report; demonstrated capacity of proper records keeping (financial and other); willing to sign performance-based contract with Ministry of Health that would trigger disbursement of fund advances; clear field supervision, monitoring and evaluation strategy for the scaling up of treatment; has Forms and formats relevant to patient monitoring; can produce implementation action plans with associated cost estimates; and preferably has the capacity to receive drugs from a central location and distribute to the treatment sites. Reports and Reviews. The Contractor will promptly notify the “Academic Contact” and “Administrative Contact” of any student who becomes “inactive”. The University will make reasonable efforts to contact the “inactive” student to determine the reason for their inactive status. The University will inform the Contractor of a student’s change in status. Upon request, the Contractor will promptly and fully inform the University of all Deliverables that have been completed. The Contractor will promptly and fully inform the University if the Deliverables are at risk of non-completion. If the University determines a remedy for such potential non-completion, the Contractor will comply with the University’s remedy.
Reports and Reviews. The COUNTY may, at any time, require the PROFESSIONAL to report in writing in a format approved by the COUNTY on the status and overall progress of the work with a projection as to whether deliverables will be on time and within budget. In addition, the COUNTY may require the PROFESSIONAL to provide an estimate of the overall percentage of completion. The COUNTY will monitor the PROFESSIONAL'S work product for compliance with project standards and criteria and may make comments and recommendations to the PROFESSIONAL regarding the work. However, any such monitoring or comments and recommendations by the COUNTY, or absence thereof, shall not relieve the PROFESSIONAL of any of its obligations under this Agreement. The PROFESSIONAL retains sole authority and responsibility for the adequacy and accuracy of its PROFESSIONAL SERVICES in accordance with generally accepted practices.

Reports and Reviews. 18.1 The Recipient will provide to KCC: 18.1.1 An monthly update report detailing the current position in relation to the delivery of individual projects as identified in the Pathfinder project plan as set out in Schedule 3; 18.1.2 A detailed year end financial statement, setting out and accounting for the use of the Capital Grant. 18.2 The Recipient shall provide KCC with information relating to the completion of individual projects including details of any opening ceremonies, press coverage or possible delays to any project delivery dates; 18.3 The Recipient shall meet with the KCC’s Authorised Representative if required by the KCC to review the progress of the development of the Play Space. Where KCC identifies shortfalls against progress and/or delivery, KCC will specify the timescale for remedy and the Recipient will prepare an action plan specifying how such shortfalls will be remedied within that timescale. 18.4 If the Organisation experiences any difficulties in achieving the targets or objectives set, KCC’s Authorised Representative must be informed immediately in order that possible solutions can be discussed and agreed. 18.5 The operation of the Agreement shall be formally reviewed by the parties on a date to be agreed.
