Sick Leave Documentation. All employees may be required to submit medical documentation after three (3) consecutive work days of illness. All employees will be required to submit medical documentation after five (5) consecutive work days of illness.
Sick Leave Documentation. Before any payment is made under the foregoing regulations, the teacher shall provide:
(i) A teacher who is absent for necessary medical or dental treatment or because of accident, sickness or disability for more than three consecutive school days shall present a medical certificate to the District. A teacher who is absent for the aforementioned reasons for up to three consecutive school days may be required to present a medical certificate to the District provided that the teacher is informed of such requirement on the day of absence or as soon thereafter as the teacher can be contacted by the District and Local No 55 of the Association shall be so informed in writing.
(ii) When the sickness extends for a period of over one month, the employee may, at the discretion of the District, be called upon to furnish a further medical certificate at the end of each month during the duration of the sickness.
(iii) Prior to returning to work after an absence due to illness of over 20 calendar days, an employee shall supply a satisfactory certificate of good health, signed by a qualified medical practitioner.
(iv) The District may direct a teacher to be examined by the District's appointed physician at the expense of the District, provided that both the teacher and Local No 55 of the Association are so informed in writing.

10.4.2 When the sickness extends for a period of over twenty (20) consecutive teaching days, the teacher may, at the discretion of the School Division, be called upon to furnish a further School Division Medical Form signed by a qualified medical practitioner, provided that both the teacher and the Executive Assistant of the Association Local #55 are so informed in writing.
10.4.3 Prior to returning to work after an absence due to illness of over twenty
Sick Leave Documentation. Sick leave shall be allowed only in the case of necessity and actual sickness or disability of the employee, or to meet dental or doctor appointments or other sickness prevention measures for the employee. Sick leave, at the discretion of the Chief of Police, may be granted for an instance of illness of a member of the employee's immediate family. To receive compensation while absent on sick leave, the employees shall notify the supervisor on duty no less than 1 hour prior to the time set for the beginning of his/her shift. A doctor's certificate may be required for absence due to illness in excess of three days or at the discretion of the Chief of Police. If the Chief of Police has a reasonable basis to believe or suspect an employee has abused sick leave privileges, then he may require a doctor's certificate for an illness of less than three days. A doctor's certificate may be required at any time by the Select Board, the Chief of Police or his designee. Abuse of sick leave privilege may be cause for dismissal. Sick leave shall be recorded regularly in the personnel records. The Chief of Police or his designee shall review all sick leave records periodically and shall investigate any causes, which indicate abusive privilege.
Sick Leave Documentation. A signed affirmation for sick leave shall be required for each use of sick leave. Reasonable medical evidence of incapacity may be required for sick leave use of forty-eight (48) hours or less duration, and shall be required for sick leave use for more than forty-eight (48) hours duration.
Sick Leave Documentation. In order for sick leave to be approved the employee must submit a statement as to the nature of the illness sufficient to justify the leave. Falsification of either a written, signed statement or a physician's certificate shall be grounds for disciplinary action including dismissal.

In order to be eligible for paid sick leave, the permanent employee shall produce a medical certificate signed by a licensed medical practitioner after one (1) day's absence if requested by the Employer. Casual employees shall be required to produce a medical certificate signed by a licensed medical practitioner before any sick leave will be granted. The medical certificate must describe the nature, and if known, the anticipated duration of the illness.
