Catastrophic Leave Bank Committee Sample Clauses

Catastrophic Leave Bank Committee. The Catastrophic Leave Bank Committee will be composed of the Vice President of Human Resources and President of the Association. Days contributed to the bank by unit members cannot be withdrawn for other than catastrophic leave. This committee will mutually develop a standard form to be used as an application when unit members are first applying for catastrophic leave from the bank. The Catastrophic Leave Bank Committee will accept or reject use of days from the bank. Unit members applying for catastrophic leave days/hours must submit to the Office of Human Resources the Faculty Catastrophic Leave Donation and Request Form together with a letter of request and a signed and dated doctor’s statement verifying the incapacitating nature and probable duration of illness or injury.
Catastrophic Leave Bank Committee. 19.17.2.1 A Catastrophic Leave Committee shall be comprised of two (2) ITA members and one (1) District representative and shall administrate the Bank. 19.17.2.2 Decisions of the Committee shall be final and are not subject to the grievance procedure contained in Article 10 of this Agreement.
Catastrophic Leave Bank Committee a. A Catastrophic Leave Bank Committee shall be established that includes three (3) unit members appointed by the Association and two (2) members appointed by the District. The Committee shall be responsible for administering the Catastrophic Leave Bank in accordance with this Agreement and applicable state law. The Committee’s duties are: (1) Receiving leave requests (2) Verifying the validity of requests (3) Approving or denying requests (4) Communicating the Committee’s decisions to affected unit members and the District (5) Soliciting donations of sick leave from eligible unit members as needed (6) Reviewing special circumstances of a member that prohibit him or her from donating additional days in order to continue as a member of the Bank and (7) Accepting donations of days of sick leave for the Bank from unit members who wish to donate in the name of a colleague. This constitutes immediate membership in the Bank. The donated days will not increase the maximum number of days the individual may receive. b. The Committee shall designate one of its members as Chairperson. c. The Committee will establish appropriate record-keeping procedures, including the total number of accumulated days in the Bank and the names of participating members. d. The Committee shall keep all records confidential and shall not disclose the nature of any illness except as is necessary to process the request for leave and appeals of denials. e. Catastrophic Leave requests shall only be approved by a majority vote of the Committee.
Catastrophic Leave Bank Committee the Catastrophic Leave Bank Committee Will be composed of two (2) members appointed by the District and two (2) members appointed by the Association.
Catastrophic Leave Bank Committee. There shall be a Catastrophic Leave Bank Committee to administer the Catastrophic Leave Bank. 29-1 1: The Catastrophic Leave Bank Committee will consist of three (3) teachers and three (3) ESP representatives appointed by the UVEA, one Administrator/Supervisor appointed by the Superintendent or his/her designee. Each appointee from UVEA will serve for a three-year rotating term; with two new appointments each year. A member may be reappointed if approved by XXXX.
Catastrophic Leave Bank Committee. A. The Association shall establish a Committee to review and approve or deny requests from the Bank. B. The Committee shall consist of six (6) members. Five voting members shall be appointed by the Association and one ex-officio member shall be appointed by the District. C. The District’s non-voting member shall serve as a communication liaison between the Committee and the District’s Business Office. D. The Committee will treat all applications and attendant information as confidential information. E. The District, upon request, shall provide the Committee information necessary for administration of the Bank.
Catastrophic Leave Bank Committee. 24.5.1. The Catastrophic Sick Leave Bank shall be administered by a three (3) member Catastrophic Sick Leave Bank Committee appointed by UTSC. The terms of the committee shall be staggered three (3) year terms. Initial appointments shall be for one (1) year, two (2) years, and three (3) years. 24.5.2. The Committee shall designate one of its members as Chairperson. 24.5.3. The Committee shall be responsible for administering the Catastrophic Sick Leave Bank in accordance with this Agreement and applicable state law. The Committee’s duties are as follows: 24.5.3.1. Develop an application procedure; 24.5.3.2. Receive leave requests; 24.5.3.3. Verify the validity of requests; 24.5.3.4. Approve or deny requests within ten (10) duty days of receipt of the application; 24.5.3.5. Communicate the Committee’s decision to affected unit members, to the Human Resources Department and the District; 24.5.3.6. Solicit donations of additional day or days of sick leave, depending upon need, when the bank of days drops to or below seventy-five (75) days; 24.5.3.7. Approve and designate appropriate forms for donating and requesting catastrophic sick leave credits; 24.5.3.8. Require a medical review by a qualified physician if a participant has drawn thirty (30) days from the Bank and requests an extension in order to grant additional days. 24.5.3.9. Request, if appropriate, that an applicant for Catastrophic Sick Leave apply for STRS disability allowance in lieu of using the leave provided in this section; 24.5.3.10. Maintain appropriate record-keeping procedures, including the total number of accumulated days in the Bank, the names of donors and participating members, and the number of days used by each recipient; 24.5.3.11. Keep all records confidential and not disclose the nature of any illness except as is necessary to process the request for leave and appeals of denials; 24.5.3.12. Approve, only by a majority vote of the Committee, requests to the Bank, 24.5.3.13. Determine the method of prorating the number of sick leave days; and 24.5.3.14. Provide, upon request by Human Resources, the documentation identified in 24.5.3.11. 24.5.4. The decision of the Committee to approve or deny a request shall be final and not subject to Article 11, Grievance, or appeal.
Catastrophic Leave Bank Committee. There shall be a Catastrophic Leave Bank Committee to administer the Catastrophic Leave Bank.

The District, upon request, shall provide the Committee information necessary for administration of the Bank.
Catastrophic Leave Bank Committee. 11 The Association President shall convene the Catastrophic Leave Bank Committee (CLBC) as soon as 13 responding to employee requests. 14 a. The CLBC shall consist of the Association President, two (2) other unit members 15 appointed by the President of the Association with approval of the Executive Board, the Assistant 16 Superintendent for Personnel and two (2) employees appointed by the District. The Association President will 17 serve as the CLBC’s chairperson and primary spokesperson, convene and preside over its meetings. 18 b. The CLBC shall be responsible for approving or denying a request in total or in part and 19 communicating its decision, in writing, to the requester(s) and the Business Office within ten (10) days following 20 receipt of the request. 21 c. The CLBC, in reviewing requests, shall consider the number of days requested, the 22 number of individuals requesting withdrawals, the status of credits in the Bank and necessary records and 23 information. 24 d. All records and information submitted to or obtained by CLBC related to an employee’s 25 health, finance, or employment status shall be deemed confidential and may not be discussed by a Committee 26 member outside of formal meetings. 1 e. All CLBC decisions are final and shall not be subject to appeal or to the 2 grievance/arbitration provisions of the agreement. A tie vote constitutes a denial. 3 f. If the Catastrophic Leave Bank is terminated for any reason, the hours remaining in the 4 Bank, if any, shall be returned to that fiscal year’s donors proportionately, but not to exceed the donor’s actual 5 contribution. Any hours still remaining will be returned proportionately to donors from previous years, but not 6 to exceed the donor’s individual contribution. Donors who are no longer employed by the District will not be 7 eligible for such distribution. If, following such distribution, there are undistributed hours remaining, such hours 8 will be deemed to have been used and no longer available for use.
