Dental Insurance Benefits. The City agrees to pay fifty percent (50%) of the cost of the dental insurance program.
Dental Insurance Benefits. Dental Expense Insurance for teachers and dependents of teachers as described in the Summary Plan Description Booklet issued by the Southwest Ohio EPC Dental Plan as this plan.
1. The foregoing Medical Benefits Program and Dental insurance shall be available for all teachers on the active working payroll who have made application for such insurance on or before the effective date of this agreement, who are regular teachers employed by the Board, who either: (1) work at least half-time, and work at least thirty-six (36) weeks per year; or
Dental Insurance Benefits. The County agrees to pay Fifty Percent (50%) of the cost of the dental insurance program. Probationary employees must indicate whether or not they desire to be covered by the County's dental insurance program within the first thirty (30) days of employment, with coverage to be effective upon the first of the month following date of hire. No employee shall make any claim against the County for additional compensation in lieu of or in addition to the County's contribution because the employee does not qualify for the family plan. Upon receipt of any notices concerning any rate increase or decrease, the County shall provide the Secretary of the Association with a copy of such notice within ten (10) days. Employees will be offered the Delta Dental PPO plan. In the event that the current dental insurance program/plan provider discontinues our program/plan or will not continue to offer the same level of benefits to our group, the parties agree to reopen the dental insurance benefits to negotiations if permitted by law.
Dental Insurance Benefits. 11.1 The Board agrees to provide each teacher with a single dental insurance plan. The Board will provide the cost of a single dental insurance plan toward the cost of a family dental insurance plan for any teacher choosing to participate in a family dental plan.
11.2 The coverage for the plan is as follows:
A. $25 deductible per year family maximum of 2 deductibles for $50).
B. 80% or usual and customary charges for all covered dental services. $1,000 maximum payment per policy year for the member and each dependent covered at the teacher’s expense. Orthodontia: 85% of usual and customary charges for dependent children less than 19 years of age covered at the teacher’s expense to a lifetime maximum of $2,500 for each child. A teacher hired subsequent to January 1, 1986, who teaches less than a full day shall receive a pro-rated share of the Board’s contribution toward the dental insurance benefits based upon the pro-rated number of hours worked.

Part-time employees in the position of Bus Operator I are limited to employee-only dental insurance eligibility beginning January 1, 2021. No employee shall make any claim against the City for additional compensation in lieu of or in addition to the City's contribution because he/she does not qualify for the family plan. The City may change coverage, benefit levels, benefit providers, carriers and/or self-fund its dental program. Upon receipt of any notices concerning any rate increase or decrease, the City shall provide the Secretary of the Union with a copy of such notice within ten (10) days. The City's contribution towards dental insurance shall be limited to fifty percent (50%) of the premium rate for the plan.
Dental Insurance Benefits. A. The Liberty Center Board of Education will pay an annual amount not to exceed $450.00 for dental insurance through NBEC or its equivalent. The balance of the premium will be deducted from the participating employee's salary checks as due. It is the choice of the employee to participate or not to participate in this dental insurance plan.
B. Except for all bus drivers, employees who are employed for less than 20 hours weekly shall not be eligible for full Board payment of dental benefits as described above. All employees hired prior to July 1, 1987 shall remain eligible if employed 15 hours weekly.
Dental Insurance Benefits. The Employer agrees to provide a dental plan for employees and their dependents at its expense, which plan shall be either Delta Dental Plan C (50% Class I and II) or a substantially equivalent plan with the carrier to be determined by the Employer.
Dental Insurance Benefits. Unisys shall permit Transitioned Employees, and their eligible dependents, to participate in the Unisys Dental Plan no later than the first of the month on or after the Employment Transition Date, provided the Transitioned Employee enrolls for such coverage.
Dental Insurance Benefits. LIMITATIONS
Dental Insurance Benefits. 2.1 During the length of this contract, the City shall pay the total sum of eighty-seven dollars ($87.00) per month for dental insurance provided by the City for each Full-Time Unit member and his/her eligible dependents.
2.2 Two dental plans are available to Unit members:
a. Under the Indemnity Plan: A Unit member may select any dentist; there is an annual deductible; claim forms must be submitted to the insurance company for payment; preventive maintenance is covered in full; eighty percent (80%) of basic maintenance is covered; and fifty percent (50%) of major maintenance is covered.
b. Under the Maintenance Plan: Unit members must select a Maintenance Plan dental provider; there are no claim forms; there are no deductibles; however, there are co- payments for some services provided.
2.3 The City shall continue to self-fund the dental plan and shall:
a. Set up an account for this program within the General Fund.
b. Post all dental insurance benefits monies for AFSCME Unit members on a monthly basis to designated account.
2.4 The City shall provide AFSCME with quarterly reports of actual costs for AFSCME represented Unit members enrolled in the City’s self-insured dental plan.
2.5 Each year AFSCME will be provided with the City’s master dental policy and plan which includes rates paid.
2.6 Without cost to the City, the City shall provide to retirees the opportunity to purchase the same dental insurance current Unit members receive at the same cost to the retiree as the City pays for Unit members. This section applies only to retirees who retire after the adoption of Resolution No. 94-043.
2.7 Usual and customary fees shall be based upon dentist fees paid within a twenty-five (25) mile radius of City Hall. The maximum annual per person coverage shall be fifteen hundred dollars ($1,500.00). First and/or secondary fees shall be paid when two dental plans are available to Unit member.
2.8 Unit members may elect to discontinue or not enroll in the dental insurance program provided that they submit written proof of equivalent coverage. Unit members electing to discontinue or not enroll in the dental insurance program shall receive seventy-five percent (75%) or sixty-five dollars and twenty-five cents ($65.25) to be placed into a City-sponsored deferred compensation plan credited to the Unit member.
