Division Responsibilities. The Director of Technology shall serve as the administrator overseeing Division IT systems. The principal or department head shall serve as the site coordinator overseeing IT systems within a school or department/office. The Instructional and Administrative Technology Department or designee shall be responsible for, among other things, authorizing and establishing employee, non‐employee, and student IT accounts, setting quotas for disk usage on IT systems, establishing a retention schedule, establishing a Division virus protection process, providing safe access to Internet resources, and ensuring that processes are in place requiring each employee and non‐employee accessing the Division system to sign the Acceptable Use Policy Agreement (AUP) and for parents/legal guardians to acknowledge the receipt of and compliance with the AUP as included in the Standards for Student Conduct.
Division Responsibilities a. Bilateral Visual Impairment: DBS has the primary responsibility of serving any eligible individual who has bilateral visual impairment which with the best correction, for that individual, constitutes or results in a substantial impediment to employment. Such an individual may have other disabilities which are in addition to the bilateral visual impairment, such as other physical and/or mental disabilities. Such an individual can be considered a dual client and can be served by both divisions; both divisions may simultaneously maintain an open case on such an individual. DBS and DVR are to coordinate the provision of the services under an Individualized Plan for Employment (IPE) for the person. The IPE will identify the services provided and/or coordinated by the most appropriate division and also the division which is responsible for payment of the services.
Division Responsibilities. 5.1 The DIVISION shall make available to the VENDOR the Person-Centered Plan for each DDDS service recipient choosing to be served by the VENDOR.
5.2 The DIVISION will notify VENDOR of changes made to the Delaware Medical Assistance program (DMAP) Provider Specific Policy for DDDS Lifespan Home and Community Based Waiver and State Plan Rehabilitative Services for Persons with Intellectual Developmental Disabilities, DDDS Policies and Procedures, DDDS Provider Standards for Home and Community Based Services or any other documents incorporated by reference.
Division Responsibilities. The Division will provide person centered planning referrals to the Qualified Vendor. The support coordinator will attend the person centered planning session and assist in identifying location and times. The support coordinator will coordinate follow up on action steps identified in the person centered plan.

5.2 DDDS will notify VENDOR of any changes made to the Delaware Medical Assistance program (DMAP) DDDS HCBS Waiver Provider Specific manual or any other documents incorporated by reference.
Division Responsibilities. Division responsibilities and/or requirements in support of this Agreement include: Provide the services described in this SLA and Division product description (see page 8). Meet appropriate response times and associated deadlines. Comply with all statutes and rules as required by the Utah Procurement Code (63G-6) and Utah Administrative Code (R33) Ensure appropriate internal controls are in place for procurements, accounting operations, and contract reviews. Provide services in a cost effective way without compromising quality. Evaluate processes on a regular basis to ensure continuous improvement in service effectiveness, efficiency and quality.
Division Responsibilities. 1. The Division shall:
a. Ensure AdSS provides training to outpatient providers serving Members on conditional release and ensure outpatient providers demonstrate understanding of A.R.S. § 13-3991 and A.R.S. §§ 13-3994 through 13- 4000, duties of outpatient providers; and
b. Establish relationships with the Superior Court and ASH to support streamlined communication and collaboration between the Division, outpatient treatment team, ASH, and the Superior Court.
2. The Division Behavioral Health Complex Care Specialist shall coordinate with the Support Coordinator and care manager, as needed, for Members on Conditional Release from the Arizona State Hospital (ASH) consistent with the CRP issued by the Superior Court to facilitate discharge.
3. The Division shall monitor the status and outcomes of Members subject to a Conditional Release issued by the Superior Court and AHCCCS, including through the Conditional Release Monthly Monitoring Report as directed by the Contractor Chart of Deliverables
4. The Division shall follow all obligations, including those stated in this section, applicable to it as set forth as specified in A.R.S. § 13-3994.
Division Responsibilities. The DIVISION shall:
A. Notify the FACILITY of the number of students and the rotation periods 4 weeks prior to the commencement of the fall and spring semesters and give 90 days notice of assignments.
B. Reserve the right to revoke any assignment, prior to the student entry into the clinical education program of the FACILITY.
C. Send only such students as are, to the DIVISION’S knowledge, in good health at the time for their clinical education.
D. Forward to the FACILITY a summary of the student’s training and experience which shall include at least information on the general and physical therapy education and experience.
E. Appoint an Academic Coordinator of Clinical Education who will be the liaison representative for the DIVISION.
F. Immediately notify the FACILITY in writing of any change or proposed change of the Academic Coordinator of Clinical Education.
G. Afford the representative of the FACILITY a clinical appointment in the Division of Physical Therapy.
H. Provide written communication to the FACILITY regarding its performance in providing clinical education experiences.
I. Withdraw a student from his/her assigned clinical education experience at the FACILITY when, in the DIVISION’S judgment, the clinical experiences do not meet the needs of the student.
J. Not discriminate against any applicant for enrollment in its course of study because of race, creed, color, age, gender, national origin, religion or disability.
K. Provide a statement of philosophy and the objectives of curricular and clinical education.
L. Accept charges for telephone calls regarding the clinical education program.
M. Consider suggestions from the FACILITY regarding curriculum improvement.
N. Provide a written statement to the FACILITY of the student’s completion of OSHA Bloodborne pathogens and HIV/AIDS education as required by the Federal Register 1910.1030 of December 1991.
O. Assign students subject to the availability of the FACILITY’S personnel for teaching and supervision.
P. Maintain the privilege to visit the FACILITY before, after and/or during the clerkship and/or internship period.
Q. Provide the student education with regard to body fluid exposure and body substance isolation as required by OSHA and will provide the FACILITY with proof upon such request.
Division Responsibilities. The DIVISION shall:
A. Provide technical assistance related to the design and construction observation/checkout of the wetland project.
B. Act as the fiscal agent for the PROJECT. The duties of the fiscal agent shall include construction bidding, awarding contract to contractor, managing contract with contractor, and providing payments to contractor. Provide all financing for design and construction, observation/checkout costs, and initial seeding after construction is completed.
C. Provide permitting support for applicable permits required by this project.
Division Responsibilities. The DIVISION shall make available to the VENDOR the plan of care (ELP or other plan of care) for every client that chooses to be served by the VENDOR. The DIVISION shall assign a Case Manager to each waiver client and non-waiver clients receiving residential services to monitor the implementation of the program plan, coordinate the provision of services required by the plan, monitor the quality and quantity of services provided, and evaluate the progress of each client. The DIVISION will assign a Family Support Specialist to coordinate care for non-waiver, non-residential clients. DDDS will notify VENDOR of any changes made to the Delaware Medical Assistance Program (DMAP) DDDS HCBS Waiver Provider Specific manual or any other documents incorporated by reference. In the event that a client seeks to change providers or services, the DIVISION will assist the client in finding a suitable alternative.
