For seasonal employees, effective February the reimbursement level under the Dental Care program will be applicable to all Part A services (preventative, endodontics, periodontics and denture repairs). Part B services remain at (dentures, crowns and bridges) and Part C services remain at (orthodontics). The Corporation agrees to pay one hundred (100) percent of the cost of Ontario Health Insurance, Group Medical and Life Insurance and Long Term Disability Insurance. The Group Life Insurance will provide coverage of two hundred (200) percent of an employee's current annual salary to the nearest one thousand dollars ($1,000.00). The Corporation will provide coverage under the current insurance contract of seventy (70) percent of normal monthly earnings to a maximum monthly benefit of three thousand five hundred dollars ($3,500.00). The Corporation will provide Vision Care coverage under the benefit contract of: three hundred dollars ($325.00) effective January 1,2006; three hundred fifty dollars ($350.00) effective January I,2007; and four hundred dollars ($400.00) effective January I, The Corporation will provide an eye examination by an Optometrist limited to one examination in a month period for the member and each insured dependent.
