Licensure/Certification. The contract between the district and the teacher shall be null and void if the teacher fails to have on file with the board continuously during employment with the district a valid Kansas Teaching License (Teaching Certificate) for the level at which he/she is employed and/or for the subjects for which he/she is employed to teach.
Licensure/Certification. At all times during the term of this Contract, Superintendent shall hold a Colorado administrator’s license appropriate for superintendent of schools. If at any time Superintendent fails to meet this requirement, this Agreement, without further action by either of the parties, shall thereupon be automatically terminated.
Licensure/Certification. It shall be the responsibility of any employee to notify the Human Resources Department of any changes or updates in their areas of licensure/certification.
Licensure/Certification. Each employee who is required by the Employer to be licensed or certified will advise the Employer of any revocation, suspension or other action affecting their license or certification.
Licensure/Certification. To the extent that a professional licensure and/or certification is required to provide the services described herein, Seller and its agents/employees shall be so licensed and/or certified at all times during the term of this Agreement; verification of such licensure or certification to be supplied by Seller, on an annual basis, or at any time upon the proper request of the appropriate Buyer's agent. The Seller shall provide the Buyer with a file on each of its employees assigned to the Buyer. The file shall include a current statement of qualification from a physician, of the reliability of the employee to physically perform the tasks assigned, a document validating annual tuberculosis screening test and immunization for Rubella and Rubeola. All medical screens are subject to the review and approval of the hospital Employee Health Nurse. The file shall also contain a current copy of licensure issued by the State of California and any other information that may be pertinent to the qualifications of the employees and their fitness to perform the assigned functions. The file must be complete, and in the hands of the Buyer prior to services being rendered by the Seller’s employee. In addition, Seller agrees to abide by the Palomar Health “Non-Employee Personnel at Palomar Health” procedure attached hereto and incorporated by reference as Exhibit 4.
Licensure/Certification. The Contractor certifies that it is licensed in Rhode Island as an HMO under the provisions of Chapter 27-41, “the HMO Act” or that it will become licensed as a Health Maintenance Organization (HMO) or Health/dental plan (HP) in the State of Rhode Island by the Rhode Island Department of Business Regulation prior to signing an Agreement with EOHHS. If Contractor is not a licensed HMO in Rhode Island, the Contractor certifies that it is either a nonprofit hospital service corporation that is licensed by the Rhode Island Department of Business Regulation (DBR) under Chapter 27-19 of the Rhode Island General Laws, a nonprofit medical service corporation that is licensed by DBR under Chapter 27-20 of the Rhode Island General Laws, or another health insurance entity licensed by DBR, and that it meets the following requirements: • Meets that requirements under R.I. Gen. Laws section 27-18.9-8: Benefit Determination and Utilization Review Act • Is certified as a utilization review entity by a nationally known health utilization management organization. The Contractor agrees to forward to EOHHS any complaints received from the Rhode Island Department of Business Regulation (DBR) or OHIC. The Contractor also agrees to forward to EOHHS a copy of any correspondence sent by the Contractor to DBR or OHIC which pertains to the Contractor’s contract status with any institution or provider group as it relates to it managed Medicaid network. The Contractor shall notify EOHHS of any person or corporation that has five percent (5%) or more ownership or controlling interest in the Contractor. At least annually, and within 30 days of any change in accreditation, the Contractor will inform EOHHS as to whether it has been accredited by a private independent accrediting entity. If the contractor has received accreditation by a private independent accrediting entity, in accordance with 42 CFR § 438.332(b)(1), 42 CFR § 438.332(b)(2), and 42 CFR§ 438.332(b)(3), the Contractor will authorize the private independent accrediting entity to provide EOHHS a copy of its most recent accreditation review, including its accreditation status, survey type, and level (as applicable); recommended actions or improvements, corrective action plans, and summaries of findings; and the expiration date of the accreditation.
Licensure/Certification. At all times during the term of this Contract, Superintendent shall hold a Colorado administrator's or principal’s license or shall complete Colorado Department of Education approved training sufficient to allow Superintendent to conduct licensed personnel evaluations under Colorado law. Failure to meet this requirement may be considered a material breach of this Contract.
Licensure/Certification. Classified employees who have earned a certificate or a degree, diploma, certification and/or licensure related to their job position may qualify for a stipend of $100/$300/$500 per year to a maximum of $2,500* per employee per year. (The maximum of $2,500 per employee is subject to review at the initial committee meeting.) In order to qualify for this additional stipend, the following criteria must be met: ■ Certificate / degree, diploma, certification and/or license must be specific to current job. ■ Receipt of certificate / degree, diploma, certification and/or license must be from a recognized state or national level trade or professional institution. ■ Knowledge/skill outcomes from the certificate / degree, diploma, certification and/or license must be designed to improve knowledge/skills that are beyond current job description requirements. ■ Requirements for the certificate / degree, diploma, certification and/or license must be completed primarily on the employee’s own time. Any full time classified employee who meets the established criteria may request the stipend pay by completing and submitting the application (APPENDIX B). Applications will be reviewed by the certification team, consisting of the Superintendent, Treasurer, and Classified Supervisor, who will make a determination as to eligibility for a stipend. A certificate from an Adult/Continuing Education program, which requires 50 or more seat hours of instruction, will be considered for a $100 stipend. All classes must be on the same topic and apply to the employee’s job field. The employee must complete the entire course schedule for the program, subject to committee review. Licensure/Certification. The ESC shall ensure that all individuals providing services under this Agreement obtain and maintain all necessary licensure and/or certification. A copy of all such credentials/licenses shall be maintained by the ESC for inspection, upon request, by the Board.
