Medical Certificate Requirement. Employees who are absent from duty as a result of a work related accident may be required to produce any medical certificate necessary within the first ten (10) working days of absence. It may be necessary to renew such certificates every twenty (20) working days thereafter unless the Employer is satisfied with the initial certificate indicating the total period of absence and probable date of return to work.

Medical Certificate Requirement. (a) The College shall require a certificate from a duly qualified medical practitioner certifying that an Employee’s absence was necessitated by illness if any such absence exceeds five (5) working days. (b) The cost, if any, for this certificate, shall be borne by, or reimbursed to the Employee by, Human Resources.

(c) An employee who is absent due to illness or injury more than four (4) times in any one (1) calendar year involving absences of five (5) working days or less may be required by the Employer to undergo a medical examination by a medical practitioner mutually acceptable to the parties in order to establish that the employee is medically fit to perform his or her normal duties. The Parties will develop a list of up to ten (10) doctors who are mutually acceptable to perform medicals under this Section. The employee shall provide a copy of the medical practitioner’s report (M-186 form) to the Employer, attesting that the employee is medically fit to perform his or her normal duties, prior the employee returning to work. The Employer shall give reasonable notice to any employee required to provide a medical practitioner’s report under this Article. Notwithstanding this, when the Employer believes that an employee’s absenteeism is excessive, it may require the employee to obtain the above medical certificate in the case of any absence as a result of illness or injury. The Employer will notify the Union and the employee before it invokes this right and will discuss the matter with the Union at its request.

Medical Certificate Requirement. (All Employees) At the request of the Company, a medical certificate must be presented by an employee upon return to work when that employee is away from work for reasons of illness or injury, after being absent in excess of two (2) consecutive days. Failure of the employee to provide the required certificate will result in the absent hours not being paid to that employee. Any charge by the doctor for producing a medical certificate, no paid by the employee’s medical plan, will be paid by the Company.
