Members’ Rights and Responsibilities. The Contractor shall furnish Medicaid MHN Program members with both verbal and written information about the nature and extent of their rights and responsibilities as members of the Contractor's plan. The rights afforded to current members are detailed in the MHN Policy and Procedure Guide, Members’ Xxxx of Rights. The written information shall be prepared at a reading comprehension level no higher than fourth (4th) grade, “or as determined appropriate by SCDHHS.” The minimum information shall include: the member's rights to receive written information about the Contractor's managed care plan including information on the structure and operation of the Plan; the network providers/subcontractors providing the member's health care, including information on any providers who are non-English speaking; information about the amount, duration, and scope of benefits available and how to obtain these benefits; information on the confidentiality of patient information; the right to file grievances or complaints about the Contractor and/or care provided; any restrictions on the member’s freedom of choice among network providers; the extent to which, and how, after-hours and emergency coverage are provided; and any other information that affects the member's enrollment into the Contractor's plan. The Contractor shall notify the Medicaid MHN Program members at least annually following initial enrollment of their right to request and receive this information. The Medicaid MHN Program members’ responsibilities shall include but are not limited to: informing the Contractor of the loss or theft of their Medicaid ID cards; presenting their ID cards when using health care services; being familiar with the plan’s procedures to the best of their abilities; calling or contacting the Contractor to obtain information and have questions clarified; providing participating network providers with accurate and complete medical information; following the prescribed treatment of care recommended by the provider or letting the provider know the reasons the treatment cannot be followed, as soon as possible; making every effort to keep any agreed upon appointments; and, accessing preventive care services.

Members’ Rights and Responsibilities. 5.1 To pay to the Company the Membership Fee payable without any deduction in advance on for the period from the Agreement fee Commencement Date to the end of one month following such date to be made on the date of this Agreement together with such VAT as may be payable on the Membership Fee 5.2 The Member shall be entitled to receive the services subject to these Terms and Conditions. 5.3 The Member must only carry on business in the name specified on the Agreement. 5.4 The Brew brand and logo are registered Trade Marks of the Company. Members may not use The Brew logo, brand or images in any document or publication, including the internet and in any way in connection with their business, unless previously agreed in writing with the Company. 5.5 The Member must only use the Hotdesk Hub for its permitted use, and only for the business stated in the Agreement or subsequently agreed with the Company. 5.6 The Member will not do or permit to be done in the Building anything which is illegal or which may be or become a nuisance, (whether actionable or not) damage, annoyance, inconvenience or disturbance to the Company or to Members or occupiers of the Development or any owner or occupier of neighbouring property; 5.7 The Member will not browse or download material of an illegal or offensive nature. This includes but is not limited to: copyrighted movies, music, or software, hacking/cracking material, or adult material. 5.8 The Member must not carry on a business which involves frequent visits by members of the public or which competes with the Company business. 5.9 The Member must not display any advertisement, signboards, nameplate, inscription, flag, banner, placard, poster, signs or notices at the Hotdesk Hub or elsewhere in the Building; 5.10 The Member may not use the address of the Building as their business address nor the Registered Office of their business, unless previously agreed with the Company via the purchase of a special membership tier or in exceptional cases, where there will be a further charge as per the current price list. 5.11 The Member will not be allowed to use the Hotdesk Hub outside of Business Opening Hours unless specifically authorised in writing in exceptional circumstances. 5.12 The Member's obligations are to pay the stipulated fee and the costs of all other services provided on the due dates and to perform all of the obligations on the part of the Member contained in the Agreement. 5.13 The Member will fully indemnify the Compa...

Members’ Rights and Responsibilities. The Contractor shall furnish Medicaid MCO program members with both verbal and written information about the nature and extent of their rights and responsibilities as a member of the Contractor's plan. The rights afforded to current members are detailed in MCO Policy and Procedure Guide, Members’ Bill of Rights. The written information shall be written at a reading comprehension level no higher than fourth (4th) grade, “or as determined appropriate by SCDHHS.” The minimum information shall include: the member's rights to receive written information about the Contractor's managed care plan including information on the structure and operation of the Plan; the network providers/subcontractors providing the member's health care; information about how to obtain benefits; confidentiallyconfidentiality of patient information; the right to file grievances or complaints about the Contractor and/or care provided; information regarding advance directives as described in 42 CFR 417.436 (2006, as amended) and 42 CFR 489, Subpart I (2006, as amended) and any information that affects the member's enrollment into the Contractor's plan. Information regarding advance directives shall include a description of the applicable State law (Chapter 66, Section 44) and must reflect any changes in State law as soon as possible, but no later than 90 days after the effective date of the change. The Contractor shall provide the member written evidence of coverage. The Medicaid MCO program members responsibilities shall include but are not limited to: informing the Contractor of the loss or theft of their ID card; presenting their ID card when using health care services; to bebeing familiar with the plans procedures to the best of the member's abilities; and/or to callcalling or contactcontacting the Contractor to obtain information and have questions clarified; to provideproviding participating network providers with accurate and complete medical information; followfollowing the prescribed treatment of care recommended by the provider or letletting the provider know the reasons the treatment cannot be followed, as soon as possible; and makemaking every effort to keep any agreed upon appointments, and follow-up appointments; and to accessaccessing preventive care services.”
Members’ Rights and Responsibilities. A. Member Rights and Responsibilities Contractor shall develop, implement and maintain written policies that address the Member's rights and responsibilities and shall communicate these to its Members, providers, and, upon request, potential members. 1) Contractor's written policies regarding Member rights shall include the following: a) To be treated with respect, giving due consideration to the Member’s right to privacy and the need to maintain confidentiality of the Member’s medical information. b) To be provided with information about the organization and its services. c) To be able to choose a Primary Care Provider within the Contractor's network. d) To participate in decision making regarding their own health care, including the right to refuse treatment. e) To voice grievances, either verbally or in writing, about the organization or the care received. f) To receive oral interpretation services for their language. g) To formulate advance directives. h) To have access to family planning services, Federally Qualified Health Centers, Indian Health Programs, sexually transmitted disease services and emergency services outside the Contractor's network pursuant to the Federal law. i) To request a State Medi-Cal fair hearing, including information on the circumstances under which an expedited fair hearing is possible. j) To have access to, and where legally appropriate, receive copies of, amend or correct their Medical Record. k) To disenroll upon request.

Members’ Rights and Responsibilities. 22.1. The Member must only use Connect Hub for its permitted use, and only for the business stated in the Agreement or subsequently agreed with the Provider. 22.2. The Member must not display any advertisement, signboards, nameplate, inscription, flag, banner, placard, poster, signs or notices at the Desk or elsewhere in the Co-working Space; 22.3. The Member will not be allowed to use the Connect Hub outside of Business Opening Hours unless specifically authorised in writing in exceptional circumstances. 22.4. It is the Member's responsibility to arrange insurance for his/her own property, brought into the Co-working Space and for his/her own liability for employees and to third parties. 22.5. When the Member makes use of the Provider's Connect Hub and meeting rooms the Member agrees that: 22.5.1. The Connect Hub and meeting rooms shall be used for general office purposes only. 22.5.2. The Member shall maintain the Connect Hub and meeting rooms in their existing condition and shall notify the Provider immediately of any damage caused by the Member and the Member's employees and visitors. 22.5.3. The Member shall be liable for all damage caused by the Member and the Member's employees and visitors. 22.6. The Common Parts of the Co-working Space will only be used in such a way as to have regard to the rights and interests of other users. 22.7. The Member must take good care of all parts of the Connect Hub, its equipment, fittings and furnishings. The Member must not alter any part of it. 22.8. The Member is liable for any damage caused by the Member or those in the Connect Hub with his/her permission or invitation. 22.9. The Member must not install any furniture or office equipment, cabling, IT or telecoms connections without the Provider consent, which it may withhold at its absolute discretion. 22.10. The Member agrees to comply with the procedures and regulations which the Provider imposes generally on users of the Connect Hub for health and safety and other reasons. It is the Member's responsibility to ensure that everyone in the Connect Hub with his/her permission or invitation also complies with the member’s obligation as set out in this agreement. 22.11. The Member agrees not to do anything that will or might constitute a breach of any Necessary Consents affecting the Connect Hub or which will or might vitiate in whole or in part any insurance in respect of the Co-working Space from time to time. 22.12.
