PTO Request Procedure. PTO requests will be granted by date of submission for PTO up to six (6) months (or up to twelve (12) months where the unit agrees), from the date of the request. If conflicting requests are received on the same day, seniority shall control. Notification of approval or denial shall be given to the nurse no later than fourteen (14) days from the date of the request. Prime time summer vacation shall be from June 1 through September 15. During summer prime time only, fourteen (14) days (two (2) weeks) plus the three (3) summer holidays may be taken. The holiday time may be taken consecutively if approved by the unit manager. Additional days may be granted if the time is available. Holiday annual leave requests shall be governed by Section 11.5.

PTO Request Procedure. Requests for PTO or vacation should be inclusive of the entire block of time the nurse is requesting. The nurse will only need to use PTO equivalent to their FTE. A. The number of nurses who may be on pre-scheduled time off at one time is defined at the unit level. B. Should the Medical Center be unable to find adequate coverage for a nurse’s requested PTO or requested unpaid time off, a nurse’s request for PTO hours or unpaid time off may be denied even though a nurse has given the required advance notice of fifteen (15) days. C. A decision to grant or deny a request will be made before the schedule’s posting deadline. The response will be in writing. Failure to respond to the request before the schedule’s posting deadline shall be considered approval. D. If more than one nurse in a unit asks for the same time off, and gives the required advance notice under the scheduling article, but the unit level staffing needs will not allow all such nurses to take this time off, the nurse(s) with the higher seniority will be given preference. Where the time off requested is for New Year’s Day, Thanksgiving, or Christmas, a nurse who received that holiday off in the previous year may not use the preference provided for in the preceding sentence. E. PTO requests submitted outside these two submittal windows shall be granted based on the date of submission, and employees shall be notified of approval or disapproval as soon as possible but not later than thirty (30) days. Within each department, the Employer will post a seniority roster and a vacation calendar. The purpose of this calendar is to provide employees with the opportunity to view vacation requests of other more senior employees. In addition to submitting the written requests for PTO to their supervisor, it is also the responsibility of the employee to place the request(s) they submit to their supervisor on the calendar. For Xxxx Bridge Children’s Hospital and Health Center LPNs, all requests for PTO within a specified quarter will be submitted to the Clinical Director/Manager at the beginning of the previous quarter and approved within one (1) month.

Only two (2) weeks may be approved during June 15 and September 30 (prime time) unless there are no conflicts. PTO requests between October 1 and March 31 must be submitted in Kronos by the employee by July 1. The Employer will respond to the PTO request by August 1. No more than one (1) week may be approved during the Thanksgiving holiday week and December 19 through January 5 (prime time) unless there are no conflicts. Employees may indicate their first and second choice for prime time PTO. Employees who are denied prime time PTO in one year shall have priority over the least senior employee, whose request would have otherwise been granted, for prime time in the next year. The Employer shall post a PTO calendar in each department. PTO requests submitted by an employee outside the designated application periods shall be responded to within thirty (30) days indicating whether the PTO request is approved or denied. In the case of conflicting requests made during the designated PTO application periods, department hire date will be the determining factor, subject to the rotation of prime time vacations.
