Survivor Income Benefits. (a) Transition Survivor Income Benefit If an employee dies while insured for Survivor Income Benefits, leaving one or more Survivors, as defined below, the Insurer shall begin payment of not more than 24 Monthly Survivor Income Benefits ("Transition Survivor Income Benefits"), provided at least one of such Survivors is living on the first day of the month following the employee's death and then qualifies as his/her Survivor, and provided that no waiver of benefits is in force. The amount of the monthly Transition Survivor Income Benefit payable to the eligible Class A, Class B, or Class C survivors of employees shall be $875.00 per month, except that for any month in which an eligible Class A survivor has a dependent child as defined in subsection (a)(2) herein and for any month in which an eligible Class B survivor has no parent surviving, the amount of the transition survivor income benefit shall be $950.00 per month. For the months in which two or more eligible Class B or Class C survivors share a Benefit, each survivor's share is computed as a fraction of the Benefit that would be paid to him/her as a sole survivor, according to his/her own eligibility for statutory benefits. The first such Benefit is payable on the first day of the month following the employee's death. Thereafter, a Monthly Survivor Income Benefit is payable on the first day in each of the next 23 months, but if on the first day of any month after the employee's death no person then living qualifies as his/her Survivor, no such Benefit is payable for that month or any subsequent month. Survivors are classified and defined as follows:
Survivor Income Benefits. 4.01 On the death of an insured employee, a monthly benefit will be payable to his/her spouse and/or children under age 18. This benefit will be payable until the later of: - (a) the date of death or, remarriage of the spouse, or (b) the date on which the youngest eligible child attains age 18. 4.02 The amount of the monthly benefit, subject to paragraph 4.04 of this Appendix, will be one-twelfth (1/12) of the greater of: - (a) $600.00, or (b) 20% of the employee's average annual basic earnings during the three (3) years immediately prior to the employee's death, or (c) 66-2/3% of the total annual Company Retirement Pension benefit which the employee has accrued to the date of the employee's death. 4.03 In the event of the remarriage of a surviving spouse with no eligible children under the age of 18, the benefit shall cease. In the event of the remarriage of a surviving spouse with eligible children under the age of 18, the benefit shall be reduced to 50% of the previous amount and be payable until the youngest surviving child attains age 18. 4.04 The maximum monthly benefit payable will be $700.00 per month. 4.05 This coverage shall terminate at the earliest of the following dates: - (a) the fifteenth (15th) day following the date of termination of employment with the Company; (b) the date of the employee's retirement or early retirement pension under the Company's Pension Plan; (c) the date on which the employee attains normal retirement age; (d) the date on which the employee ceases to have any eligible dependents; (e) the date of remarriage of a surviving spouse. 4.06 The insurance for an insured employee, who becomes totally disabled before normal retirement age, will be continued during the continuance of total disability until the employee recovers or attains normal retirement age. The insurer shall reserve the right to require the employee to submit to physical examination by physicians designated by it. Total disability means continuous disability which, during the first two (2) years of total disablement prevents an employee from performing any and every duty pertaining to the employee's own occupation and thereafter from engaging in any occupation for which the employee is fitted through education, training or experience. 4.07 The Company will continue to provide the following Welfare Benefits for survivors: - (a) Semi-private Hospital and Ambulance Expense Insurance; (b) Prescription Drug Expense Insurance;
Survivor Income Benefits. The Company will fund basic life and accidental dismemberment and death insurance benefits. Additionally, employees will be offered the opportunity to purchase voluntary supplemental life insurance and accidental death and dismemberment coverage. These programs will be provided under the same terms and conditions as it is provided to non-represented employees.

Survivor Income Benefits. (a) Transition Survivor Income Benefit (1) The Company agrees to provide Transition Survivor Income Benefits Insurance for a period of up to a maximum of Twenty-Four (24) months in the amount of: - $600.00 per month payable to the eligible survivor of an employee who dies on or after January 1, 2005; except that $625.00 per month effective January 1, 2005 will be payable in any month in which; (i) an eligible Class A survivor has a dependent child as defined in sub- section (c) of this section, or (ii) an eligible Class B survivor survives both parents. (2) Such insurance shall also be provided for an employee who is retired under any Pension Plan of the Company due to total and permanent disability, but only until they have attained age 65. No other retired employee shall be insured hereunder.

The Insurer shall reserve the right to require the employee to submit to physical examination by physicians designated by it. An employee shall be deemed to be totally and permanently disabled only if he/she is not engaged in regular employment or occupation for remuneration or profit and on the basis of medical evidence satisfactory to the Insurer the employee is found to be wholly and permanently prevented from engaging in regular employment or occupation with the Company at the plant or plants where he/she has seniority for remuneration or profit as a result of bodily injury or disease, either occupational or non occupational in cause.
Survivor Income Benefits. 1. Transition benefit - A benefit of $625 per month effective June 1, 1999; $650 per month effective June 1, 2000; $675 per month effective June 1, 2001 is payable for twenty-four (24) months for all Class A and B and D survivors. No survivor shall be able to receive a benefit under more than one Classification. The health care coverages will be provided by the Company for 36 months. 2. Bridge benefit - A benefit of $625 per month effective June 1, 1999 - $650 per month effective June 1, 2000; $675 per month effective June 1, 2001 is payable, commencing on the first day of the calendar month following the month for which the 24th transition benefit is paid and monthly thereafter to surviving spouses who are 45 years or over at the time of the employee’s death or to surviving spouses whose age at the time of the employee’s death plus the deceased employee’s credited service under the pension plan equals 55. The survivor may continue to be covered by the Health Care Benefits, beyond the 36 months cov- erage during the Transition Benefit, providing the survi- vor pays the full premium cost and is eligible for the Bridge benefit. Bridge benefits cease on the earlier of: (1) The date the survivor remarries, (2) The date of the survivor’s death, (3) age 65. 3. Waiver - a Class A or Class B eligible survivor may waive any right to receive survivor income benefits with re- spect to any period by completing a waiver form fur- nished by the Company for that purpose regardless of the date the deceased employee last worked. If the Waiver is received by the Company on or before the date the survivor’s application for survivor income benefits is re- ceived by the Company, such Waiver shall become ef- fective the first day of the first month for which such benefits are payable, or, if later, the first day of the month designated by the survivor; otherwise such Waiver shall become effective the first day of ...
