Xxxx St., Channahon IL 60410 xxxxxxx@xxx00.xxx 000-000-0000 Processing and Reviewing a Report or Complaint Upon receipt of a report, the Title IX Coordinator and/or designee will promptly contact the Complainant to: (1) discuss the availability of supportive measures, (2) consider the Complainant’s wishes with respect to supportive measures, (3) inform the Complainant of the availability of supportive measures with or without the filing of a Formal Title IX Sexual Harassment Complaint, and (4) explain to the Complainant the process for filing a Formal Title IX Sexual Harassment Complaint. Further, the Title IX Coordinator will analyze the report to identify and determine whether there is another or an additional appropriate method(s) for processing and reviewing it. For any report received, the Title IX Coordinator shall review Board policies 2:260, Uniform Grievance Procedure; 5:20, Workplace Harassment Prohibited; 5:90, Abused and Neglected Child Reporting; 5:120, Employee Ethics; Conduct; and Conflict of Interest; 7:20, Harassment of Students Prohibited; 7:180, Prevention of and Response to Bullying, Intimidation, and Harassment; 7:185, Teen Dating Violence Prohibited; and 7:190, Student Behavior, to determine if the allegations in the report require further action. Reports of alleged sexual harassment will be confidential to the greatest extent practicable, subject to the District’s duty to investigate and maintain an educational program or activity that is productive, respectful, and free of sexual harassment.

Xxxxxxx Xxxxxx 0000 Xxxx Xxxxxx, Franklin Park, IL 60131 xxxxxxx@xxxxxx000.xxx 000-000-0000 Any person may report sex discrimination under Title IX, including Title IX Sexual Harassment (whether or not the person reporting is the person alleged to be the victim of conduct that could constitute sex discrimination under Title IX or Title IX Sexual Harassment), in person, by mail, by telephone, or by electronic mail, using the contact information listed for the Title IX Coordinator, or by any other means that results in the Title IX Coordinator receiving the person's verbal or written report. Such a report may be made at any time (including during non-business hours) by using the telephone number or electronic mail address or by mail to the office address, listed for the Title IX Coordinator.
Title IX Coordinator. The University will publish its notice of the Title IX Coordinator’s name or title, office address, email address, and telephone number consistent with the requirements of Title IX at 28 C.F.R. § 54.135(a) and 34 C.F.R. § 106.8(a), within fourteen (14) calendar days of the United States’ approval of the notice. If the University chooses to designate one or more persons to assist the Title IX Coordinator, the publication will make clear the scope of each person’s responsibilities (e.g., who will handle complaints of sex discrimination and who will handle complaints by students, employees, student employees, and faculty), and will designate the University’s Title IX Coordinator to have ultimate oversight responsibility with regard to Title IX matters. Additionally, the University will publish a notice of nondiscrimination with the Title IX Coordinator’s information consistent with the requirements of Title IX at 28 C.F.R. § 54.140 and 34 C.F.R. § 106.9. By August 22, 2013, the University will disseminate this notice through the University’s website, student handbook, and any other means of notification the University deems effective to ensure that the information is widely disseminated.

Notice of Nondiscrimination o By July 15, 2013, the University will provide the United States with a draft of the notice to be published regarding the Title IX Coordinator and notice of nondiscrimination pursuant to Section IV above. o Within 30 calendar days of the United States’ approval of the draft publication pursuant to Section IV, the University will provide the United States with documentation that it has implemented Section IV, including copies of any printed publications, and web links to any electronic publications containing the notice.
Title IX Coordinator. The District will designate at least one individual (the Title IX Coordinator) to coordinate its efforts to comply with and carry out its responsibilities under Title IX, including the equitable resolution of complaints of sex discrimination, including sexual and gender-based harassment, filed by students, staff and faculty. In collaboration with the consultant, the District will develop a training program for the Title IX Coordinator that covers recognizing and appropriately responding to Title IX complaints filed by students, staff, and faculty. The training will address how to conduct and document adequate, prompt, reliable, and impartial investigations, including conducting interviews of victims of sexual assault and communicating in a fair, non-biased, and objective manner. The training will also focus on the responsibility of the Title IX Coordinator to regularly develop and participate in activities designed to raise awareness within the District regarding sexual and gender-based harassment and the District’s Policy and Procedures regarding sexual and gender-based harassment. REPORTING REQUIREMENT: By February 7, 2014, the District will inform OCR of the identity and qualifications of the Title IX Coordinator, and outline the training program developed for the Title IX Coordinator pursuant to this Item II.
Title IX Coordinator. Monitoring Program
Title IX Coordinator. The College will review and revise the current responsibilities of its Title IX Coordinator and Deputy Coordinators (collectively, “Title IX Coordinators”) to ensure that their responsibilities are consistent with Title IX. Based on the Title IX Coordinators’ responsibilities, the College will develop a description of corresponding mandatory training requirements for its Title IX Coordinators. The responsibilities and training requirements will include the following:
Title IX Coordinator. 5. By December 15, 2015, the District will develop a job description for its Title IX Coordinator. The job description will set forth the responsibilities of the Title IX Coordinator to coordinate the District’s compliance with Title IX, including the District’s revised Procedure 4170, and to oversee the District’s responses to all complaints involving possible sex discrimination, including sexual harassment, which includes monitoring outcomes, identifying and addressing any patterns, and assessing effects on the student community. In addition, the Title IX Coordinator is responsible for developing and participating in activities designed to raise awareness within the District regarding sex discrimination, including sexual harassment. If the District opts to designate additional coordinators to assist the Title IX Coordinator, it will make clear the scope of each coordinator’s duties and will ensure that the Title IX Coordinator has ultimate oversight responsibility over the other coordinators, which will be clearly stated in that coordinator’s job description.
