Verification Requirements. Company shall verify each End User’s status as a Qualifying Individual in accordance with all of the following requirements: (a) At the point an End User initially attempts to register for the Student Offer (including where an individual who is not an End User is required to become one as part of such registration process), Company shall verify status as a Qualifying Individual utilizing an Approved Verification Partner ****. To the extent Company permits an End User who cannot be verified as a Qualifying Individual to benefit from the Student Offer, no wholesale discount shall apply to otherwise applicable Wholesale Fees under the Agreement. Verification Requirements. 23.8.1 It is the responsibility of the employee to provide verification for earned credits. Verification of coursework shall consist of a transcript, grade card, or certificate from a training institution. If a course does not provide unit credit, the employee must provide verification of the number of hours attended, and that the course was satisfactorily completed. 23.8.2 This verification must be submitted to the Associate Superintendent of Human Resources to be recorded for processing appropriate payroll documents.
Verification Requirements. 11.13.1 After any absence due to illness or injury, the unit member shall verify the absence by submitting a completed and signed District absence form to his/her immediate supervisor. 11.13.2 The District Superintendent or designee shall require verification from the unit member’s physician whenever (1) a unit member has been absent for five or more consecutive school days or (2) the unit member’s absence record shows chronic absenteeism or a pattern of absences immediately before or after weekends and/or holidays. 11.13.3 The District Superintendent or designee may require a unit member to visit a physician selected by the District, at District expense, when the unit member’s conduct or performance at work is inconsistent with the medical statement provided by the unit member’s physician. 11.13.3.1 In requiring a unit member to visit a physician selected by the District, the District is entitled only to information concerning whether the unit member is able to perform the essential duties of his or her assignment and whether the unit member requires any accommodation(s) in order to perform the essential duties or has restrictions in the performance of the essential duties. If the unit member will require an accommodation or has restrictions, the unit member’s physician shall indicate the accommodation(s) the unit member will require, the restriction(s) the unit member has and the length of time he or she will require the accommodations or have the restrictions. Verification Requirements. The Approved Sponsor must: (a) ensure that skills assessments undertaken by an Overseas Worker are videotaped and stored on appropriate media, and understands the Department may request production of these videos.  Videos are to include proof of identity of the Overseas Worker. At a minimum this will include production of legible identity documents, such as national identity cards or passports, during taping.  Where video-taping is not practicable due to exceptional circumstances, the Approved Sponsor (not the MINTRAC-registered assessor) must notify the Department of this within seven (7) days.  Video-taping may be undertaking remotely, such as via the internet, by the MINTRAC-registered assessor, but in all cases must be done in real-time. (b) maintain records of how it has assessed and been satisfied of the work experience requirements outlined at Schedule 4 item 1(b), (noting this is distinct and separate from the skills assessment), and understands the Department may request evidence of these individual assessments and/or related policies and procedures. (c) verify within two (2) days of a Primary Sponsored Person’s arrival that the Primary Visa Holder is the same person for which a visa application was lodged and/or the same person for whom the skills assessment was undertaken, and a digital photograph was provided; (d) notify the Commonwealth immediately if there is any inconsistency identified in terms of the identity of the Primary Visa Holder; (e) notify the Commonwealth within one (1) month if the Primary Sponsored Person does not have the skills required to work in the nominated position and propose remedial action to address any identified issues with the skills assessment process; (f) comply with any Commonwealth request by the Department for the Approved Sponsor to obtain at the Approved Sponsor’s expense an independent audit, by an independent authority specified by the Commonwealth, such as use the services of any assessor associated with a skills assessment failure; (g) re-assess any Primary Sponsored Person assessed by an assessor associated with a skills assessment failure; and (h) ensure that Primary Sponsored Persons and any Secondary Sponsored Persons have adequate arrangements for health insurance in place from their date of arrival in Australia.

Verification Requirements. If an Employee is seeking to use or has used sick leave for any of the above health-related authorized uses for more than three (3) consecutive workdays, he or she may be required to provide verification that establishes or confirms that the use of sick leave is for an authorized purpose. When an Employee or the Employee's immediate family is sick for more than three (3) consecutive days for which he or she would otherwise be required to work, acceptable verification will typically be in the form of a doctor's note or a signed statement by a health care provider, indicating that the use of sick leave is necessary to take care of the Employee or the Employee's immediate family. (In exceptional circumstances where the Employee can demonstrate that the doctor's note requirement will impose an undue burden or expense on the Employee, a written or oral statement from the Employee indicating that the use of sick leave is necessary to take care of themselves or immediate family will be deemed acceptable verification.) In all cases when verification is required, sick leave will be paid no later than the regular payday for the pay period during which verification is provided by the Employee to the Health District. 19.9 Any Employee covered by this Agreement on the date of signing in 1995 upon separation of employment for any reason shall be compensated for all unused sick leave subject to the following limitations: 19.9.1 The rate of payoff shall be twenty-five (25%) of all the Employee's accumulated sick leave. Payment of accumulated sick leave is paid by multiplying the Employee's base hourly rate times twenty-five percent (25%)of the Employee's accumulated sick leave hours. All payments of accumulated sick leave are based on the Employee's salary at the time of separation. 19.9.2 For this purpose, retirement shall not include Employees who have terminated prior to retirement, but left their vested Washington Public Employees Retirement Funds on deposit with the Department of Retirement Systems (DRS). 19.10 Any new Employees hired after the date of signing of this Agreement in 1995, upon separation of employment (either voluntarily or is terminated), shall forfeit any unused sick leave and any unused sick leave shall not be paid as separation pay except in the case of death, retirement, or layoff. Verification Requirements. It is the responsibility of the employee to provide verification for points completed. This verification must be submitted to the Executive Director, Personnel, to be recorded on the permanent record of the employee. After a stage of the program is completed, the Executive Director, Personnel, shall authorize payment for the stage completed as stated in the preceding paragraphs. Payment of the stipend will be made as soon as possible after completion of the stage is verified.
