Wellness Bonus. Employees who have perfect attendance for a period of two (2) consecutive calendar months will be given the opportunity of using one (1) day of any accumulated sick hours for a Wellness Day. This is a day off which is requested three (3) weeks in advance with the supervisor and the supervisor shall respond in one (1) week from the date the request was submitted. Such day must be taken within the following three (3) calendar months and must not be taken on a holiday. Your scheduled Wellness Day is not considered as an absent day.

Wellness Bonus. Each employee (even those who do not receive their health benefits through the City and have accepted a cashout under 14.5) will be eligible for up to $400.00 per year as a wellness bonus. Wellness bonuses are available if an employee provides proof of participation in the following wellness activities in the amounts stated: i) Annual physical examination with primary care physician or physician’s assistant--$100. ii) Annual or semi-annual dental examination (cleaning) with dentist or dental assistant--$100. iii) Completion of annual online risk assessment by the stated deadline--$100.00. Deadline will be identified each year no less than three months after start of fiscal year. iv) Participation in a list of approved wellness activities designated by Human Resources--$50.00 each for up to total of $100.00. For purpose of example only, these activities may include: proof of health club or gym membership, proof of enrollment in a weight management program, proof of completion of a smoking cessation program, proof of completion of any of the preventative care screenings included in the City’s health plan, such as mammogram, annual gynecological exam, colonoscopy, prostate exam, or proof of participation in any of the offerings included in the City’s EAP program.
Wellness Bonus. ‌ 1. In the event that a Member works a twelve (12) month period, concurrent with the calendar year, and takes zero (0) hours of sick leave during said calendar year, the Member shall receive a "wellness" bonus of $300.00. In the event that a Member works said calendar year and takes one shift or less (i.e. 8 hours or less or 10 hours or less, depending on scheduling) of sick leave during said calendar year, the Member shall receive a "wellness" bonus of $150.00. 2. In addition, during a twelve (12) month period, concurrent with the calendar year, a member may voluntarily elect to participate in incentivized wellness opportunities offered by the City. These opportunities are meant to support the overall physical and mental wellness of the members. To encourage participation, incentives are offered as follows: a) Annual physical exam: Members who elect to undergo a physical examination by their personal physician, as is included in the City’s group health insurance plan coverage, will receive$100.00. The exam shall be scheduled by the member during off-work hours and be paid for through the members health insurance coverage or other means available to the member. A member shall only be eligible to earn the incentive once in a calendar year. b) Annual mental health exam: Members who elect to schedule and attend a meeting with a mental health professional of their choosing will receive $100.00. The exam shall be scheduled by the member during off-work hours and be paid for through the members health insurance coverage or other means available to the member, including the City’s Employee Assistance Program (EAP) which is available at no cost to the employee. A member shall only be eligible to earn the incentive once in a calendar year.

Wellness Bonus. ‌ The wellness bonus is provided to active members of the CBU for the purpose of maintaining their physical well-being and remaining physically fit as defined by the Contract. Employees who resign in good standing during the bonus period and meet all other qualifications at the time of resignation will receive a prorated payment for the time worked during the bonus period. Employees who are terminated or resign in lieu of termination during the bonus period are not eligible for the wellness bonus. Part-time members of the CBU will receive a prorated portion of the wellness bonus beginning with their first eligible quarter. The wellness bonus is paid at $1000.00 for a period year and is paid on a quarterly basis at the end of each quarter. The bonus amounts represent gross dollars and are subject to taxes. Payments will be made during the next full pay period after the quarter ends. Qualification -must be verified prior to the quarterly payments. The criteria for the wellness bonus will be posted and remain, in Armories, Break rooms, and on Union Bulletin Boards. The Company understands that the awarding of the bonus pursuant to this section may require the Company to increase the regular rate of pay for the calculation of overtime and/or double time earned and paid during the bonus performance period. As of the first bonus period calculated after December 15, 2019, the Company will determine if the awarding of a bonus causes the need to adjust overtime and/or double time premium earnings, and the increased amount will be paid to the affected Employees in the pay period following the bonus payment.
Wellness Bonus. Unit members who take zero (0) days of Personal Necessity Leave (Article 10.8) or sick leave (Article 10.9) in a contract year shall be entitled to a bonus of two-hundred fifty dollars ($250) payable on a paycheck by the end of the fiscal year. Unit members who take one (1) day of Personal Necessity Leave (Article 10.8) or sick leave (Article 10.9) in a contract year shall be entitled to a bonus of one-hundred twenty-five dollars ($125) payable on a paycheck by the end of the fiscal year.

  • Salary, Bonus and Benefits For services rendered by the Employee on behalf of the Company during the Employment Term, the following salary, bonus and benefits shall be provided to the Employee by the Company: (a) The Company shall pay to the Employee, in equal installments, according to the Company’s then current practice for paying its executive officers in effect from time to time during the Employment Term, the Annual Base Salary. (b) The Employee shall participate in the Sealy Corporation Annual Bonus Plan (the “Bonus Plan”) in accordance with the provisions of that Plan as in effect as of the date of this Agreement based on the Target Annual Bonus Percentage. (c) The Employee shall be eligible for participation in such other benefit plans, including, but not limited to, the Company’s Profit Sharing Plan and Trust, Executive Severance Benefit Plan, Benefit Equalization Plan, Short-Term and Long Term Disability Plans, Group Term Life Insurance Plan, Medical Plan or PPO, Dental Plan, the 401(k) feature of the Profit Sharing Plan and the 1998 Stock Option Plan, as the Board may adopt from time to time and in which the Company’s executive officers are eligible to participate. Such participation shall be subject to the terms and conditions set forth in the applicable plan documents. As is more fully set forth in Section 6 hereof, the Employee shall not be entitled to duplicative payments under this Agreement and the Executive Severance Benefit Plan. (d) Without limiting the generality of Subsection 3(c) above, for so long as such coverage shall be available to the executive officers of the Company, the Employee shall be eligible to participate in the Company’s Group Term Life Insurance Plan with a death benefit to be provided at the level of one and one half (1 ½) times annual base salary at Company expense, plus extended coverage with a death benefit to be provided of at least the level in effect on the date of this Agreement for the Employee under such Plan at the Employee’s discretion and expense. (e) The Employee shall be entitled to take, during each calendar year period during the Employment Term, vacation time equal to four (4) weeks per year. (f) In addition, the Parties do hereby further confirm that any shares of Class A Common Stock of the Company (“Class A Shares”), and any options to purchase additional Class A Shares previously granted to Employee are in addition to, and not in lieu of, any shares or options which may be granted under any other plan or arrangement of the Company after the date of this Agreement, and (b) the various stock agreements and stock option agreements, and any related Stockholder Agreement (the “Stockholder Agreement”) between the Parties (such agreements being hereinafter referred to collectively as the “Pre-existing Agreements”), all remain in full force and effect except as otherwise provided herein. Notwithstanding the foregoing, to the extent that any provision contained herein is inconsistent with the terms of any of the Pre-existing Agreements, the terms of this Agreement shall be controlling.

  • Longevity Bonus After twenty (20) years of continuous service, an employee will receive a longevity bonus of seven hundred dollars ($700) per year, payable in one lump sum by the second pay period following the employee's anniversary date.

  • Cash Bonus Executive shall be entitled to a fraction of any Cash Bonus for the fiscal year of the Company within which Executive’s termination of employment occurs which, based upon the criteria established for such Cash Bonus, would have been payable to Executive had he remained employed through the date of payment, the numerator of which is the number of days of such fiscal year prior to his termination of employment and the denominator of which is three hundred and sixty-five (365); and

  • Sick Leave Bonus ‌ For every six (6) months of perfect sick leave attendance after July 1, 1987, the employee will receive eight (8) hours of bonus time. This bonus time will be prorated for part-time employees. Such bonus time can be used for any leave purpose covered by this Agreement. Such bonus time shall be counted as vacation leave credits for purposes of determining eligibility for carry- over and cash payments.

  • Short-Term Incentive Compensation In addition to the foregoing Base Salary, the Executive shall be eligible during the Term to receive cash short-term incentive compensation, determined and payable in the discretion of the Compensation Committee of the Board. At least annually, the Compensation Committee shall consider awarding short-term incentive compensation to the Executive.

  • Long-Term Incentive Compensation Subject to the Executive’s continued employment hereunder, the Executive shall be eligible to participate in any equity incentive plan for executives of the Firm as may be in effect from time to time, in accordance with the terms of any such plan.

  • Annual Cash Bonus During the Term, Executive may be eligible to receive an annual cash bonus, on terms and conditions as determined by the Committee in its sole discretion taking into account Company and individual performance objectives.

  • Vacation Bonus Employees shall receive one day's base pay (or adjusted earnings) for each year of service beyond twenty-five (25) years, to a maximum of ten (10) days’ pay.

  • Bonus The Executive shall be eligible for Bonuses determined by the Board.

  • Base Salary and Bonus As compensation for the Executive's services under this Agreement, the Executive shall receive and the Company shall pay a weekly base salary set forth on Exhibit A. Such base salary may be increased but not decreased during the Term or Renewal Period in the Company's discretion based upon the Executive's performance and any other factors the Company deems relevant. Such base salary shall be payable in accordance with the policy then prevailing for the Company's executives. In addition to such base salary, the Executive shall be entitled during the Term or Renewal Period to a performance bonus set forth on Exhibit A and to participate in and receive payments from, at the Company's election, other bonus and other incentive compensation plans, if any, as may be adopted by the Company.
