HEALTH, DENTAL AND VISION INSURANCE BENEFITS. Effective 07/1/07, the Public Authority will fund up to 900 healthcare slots at the contribution rate of $185.31 per month. Effective 07/01/08, the Public Authority will fund up to 950 healthcare slots at the contribution rate of $185.31 per month. This benefit is restricted to Providers who work at least 35 hours per month and is limited to employee only (no dependent coverage). Independent providers who are enrolled in the Health Plan of San Mateo shall pay $10.00 per month toward the monthly premium. In the event of an increase in premium, the parties will meet and confer to discuss potential reallocation of funds available for benefits within the County share. Effective 07/01/07, the Public Authority shall fund up to 2,150 dental and vision slots. Effective 07/01/08, the Public Authority will fund up to 2,200 dental and vision slots. Dental and vision insurance is for Independent Providers who work at least 35 hours per month. Dental and Vision coverage shall be provided at the premium rate of $21.28. Dental coverage shall be provided under the Blue Cross 2500 Dental plan. The Public Authority shall continue to pay for those Independent Providers who are currently receiving dependent and family coverage and who were paid for 35 or more hours in June 2003, to continue at their current level of coverage as long as they are continuously employed as an Independent Provider at 35 or more hours per month with no break in service. The number of filled slots for Health will be reviewed on a quarterly basis on September 30, December 31, March 31, and June 30. If the number of filled slots is within 10 of the maximum allowable slots as stated above, the Union and the Public Authority will meet no later than 30 days of the dates stated above to review allocation of slots. Independent Providers not currently receiving dependent and family coverage may purchase dental coverage for their dependents under the condition that the Independent Provider shall be responsible for all costs for such coverage. SEIU Local 521 shall collect and make all payments for such coverage and the Public Authority shall have no responsibility for such dependent coverage. All Independent Providers currently enrolled in the Dental PPO Plan will be transferred to the Blue Cross 2500 HMO Plan. The number of hours worked per month is determined by the “Provider Hours Paid for Month Report” from EDS, or a similar report agreed to by the Public Authority and SEIU Local 521. The employee shall pay any increase over this amount. If plan costs should increase by more than 20% year to year, the Town and Membership agree to meet and confer as to the disposition of those cost increases. Town retirees will have access to the CalPERS Health Care Plan in accordance with plan specifications under CalPERS' "unequal method" of Town funding. Employees who are able to provide proof of health coverage provided by their spouse or domestic partner's employer, the U.S. Military, or another employer group coverage other than the Town, will be eligible to opt out of the Town's health care coverage. In compensation for opting out the employee will receive one half (1/2) of the monthly amount they would receive as a contribution at the time of opt out from the Town toward their health care. Changes in the employee's health care status will result in changes in the opt out compensation (e.g.: an employee's child "ages out" of the plan shall no longer be eligible for the family opt out level). This amount will be distributed evenly over 24 of the Town's payroll periods for the coverage year. Employees hired by the Town after February 1, 2011 are only eligible for Employee-only compensation regardless of their dependent status. The Town will also maintain an Employee Wellness Program.

The “fixed amount” contribution for dental and vision rates shall be increased by the lesser of 10.0% or the actual average increase in the dental and vision rates, respectively. The employee shall pay any increase over this amount. If plan costs should increase by more than 20% year to year, the Town and Membership agree to meet and confer as to the disposition of those cost increases. For the purposes of calculating health care premium increases, rates shall include the base premium, administrative fee percentages, and GASB 45 costs. Town retirees will have access to the CalPERS Health Care Plan in accordance with plan specifications under CalPERS' "unequal method" of Town funding. No later than June 30, 2025 or the signing of this agreement, whichever comes later, the Town will bring a proposal to the Town Council for the establishment of a TGEMA Healthcare Designation. The proposal shall contain the following provisions:
HEALTH, DENTAL AND VISION INSURANCE BENEFITS. During the Term of this Agreement, the District agrees to contribute to basic medical insurance premiums, excluding dental and vision, in the following manner: (A) for the Employee only plan, the District will pay one hundred percent (100%) of the premium for the lowest plan available; (B) for the Employee plus one dependent plan or the Employee plus two or more dependents, the District shall pay the premium at the previous year maximum contribution rate and shall adjust up to ninety-three percent (93%) of the costs of the current year premium for the lowest plan available, as shown in Exhibit B, which is attached hereto. Notwithstanding the above, the District shall not pay for a premium increase that exceeds twelve percent (12%) of the previous year's premium. For dental and vision insurance, the District shall pay the premium for the Employee only, but coverage may be extended to Employee's dependents at the option and expense of Employee.

Out-of-state employers must provide Oregon workers' compensation coverage for their workers who work at a single location within Oregon for more than 30 days in a calendar year. Contractors who perform the operations without the assistance or labor of any employee need not obtain such coverage.
