Procedural Guidelines. 1. Since it is important that grievances be processed as rapidly as possible, the number of days indicated at each level is to be considered the maximum, and every effort should be made to expedite the process before the deadlines are reached. 2. Time limits provided in this Agreement may be extended by mutual agreement when signed by the parties. 3. All grievances shall be brought to the attention of the appropriate principal or supervisor within fifteen (15) workdays of the alleged violation, or they will not be considered. 4. All claims for back wages shall be limited to the amount of wages that the employee would have otherwise earned, less any unemployment compensation that he may have received during the period of the back pay. 5. Any grievance which arose prior to the effective date of this Agreement shall be processed through the procedure in effect at the time of the grievance. 6. Failure at any step of this procedure to communicate the decision on a grievance within the specified time limit shall permit the employee to lodge an appeal at the next step of this procedure, but any grievance not advanced from one step to the next within the time limits of that step shall be deemed resolved by the Administrator or the Board’s answer to the previous step. 7. All grievances must be processed through the grievance procedure, and after the grievance is filed, it shall be amendable only by mutual consent of the Board and the Association and may be withdrawn by the grievant. 8. When the presence of an employee or a key witness at a grievance hearing is requested by either party, illness or other incapacity of the employee or the key witness shall be grounds for any necessary extension of grievance procedure time limits.

Procedural Guidelines. Employees seeking access to sick leave credits for the purposes of regular local specialist, medical or dental appointments will apply to their principal/supervisor (Caretaking and Maintenance Staff apply to their respective Area Supervisor). The Principal/Supervisor/Area Supervisor may approve the utilization of sick leave in such cases (usually part day requests).
Procedural Guidelines. The purpose of the Sick Leave Bank is to enable employers to provide protection in cases of long-term catastrophic illness and to encourage employees to “save” sick leave for a valuable future economic benefit (terminal leave.) Membership in the Sick Leave Bank must precede the diagnosis date for the current illness for which days are being applied.

The employee requesting shared sick leave shall complete an application for shared sick leave and submit the application to the Superintendent of Schools. 2. The employee requesting shared sick leave shall submit, with application, a medical certificate from a licensed physician or health care practitioner verifying the severe or extraordinary nature and expected duration of the condition.

When the request meets the following guidelines, it may be approved by the Supervisor: For out of town services -that the service is not readily available locally For accompanying family members -that no other family member is available or that the employee is the logical choice to accompany the family member That the length of time requested is reasonable (i.e. not more than a day or two allowing travel time) For out of town services that there is supporting documentation of the appointment That this is not something that should be approved under compassionate leave that this is a scheduled appointment For out of town services the appointment is for the employee who is entitled to sick leave, the employee’s significant other or children If the above criteria are not met, the Supervisor should contact Human Resources for consultation as to whether or not the request should be approved. The employee shall be granted leave without pay to attend such appointments in the event the utilization of sick leave credits is denied.

Procedural Guidelines. A. In the event of an offender escape, the CCCS Transportation Officers shall: 1. Perform those actions that can result in the safe and timely apprehension of the escapee(s). 2. Call 9-1-1 immediately and provide responding law enforcement authorities with the physical description of the escapee, the P&P wanted sheet of the escapee(s), the direction the escapee(s) were heading at the time of the escape, the facility to which the escapee was being transported, etc. 3. Immediately contact the CCCS Transportation Supervisor. 4. Comply with all law enforcement requests. a. Complete a formal interview process if required. b. Provide a written statement. c. Provide law enforcement with relevant contact information. d. Notify MSP Command Post 5. Remain at the scene of the escape until such time as local law enforcement has assumed full control of the search and releases the Transportation Officers from any further formal involvement in any attempts made to locate the escapee(s). B. The CCCS Transportation Supervisor shall contact the following authorities immediately after escape has occurred: 1. The appropriate DOC Staff (Command Post). 2. The Command Post at the respective prison. 3. The Director of Community Correctional Programs/Transportation for CCCS, Inc. 4. The Chief Executive Officer for CCCS, Inc. 5. Discuss and gain approval from the Director of Community Correctional Programs/Transportation for CCCS, the Chief Executive Officer of CCCS and appropriate officials from the MDOC in coordinating any news releases made to the media C. After the escape has occurred, the CCCS Transportation Supervisor shall also: 1. Complete and forward a written Escape Form (see attached 1.9.A) and send it to: a. The appropriate DOC Staff (Command Post). b. The Director of Community Correctional Programs/Transportation for CCCS, Inc. c. The Chief Executive Officer of CCCS, Inc. 2. Ensure that those Transportation Officers on-duty at the time of the escape, complete an Accident/Incident Report Form (see attached 1.9.B) prior to the conclusion of their shift on the day of the escape. 3. Review all written reports submitted as part of the ongoing escape investigation with the following authorities: a. The appropriate DOC Staff (Command Post). b. The Director of Community Correctional Programs/Transportation for CCCS, Inc. c. The Chief Executive Officer for CCCS, Inc. d. Assigned investigators from the Montana Department of Corrections (MDOC). 4. Upon completion of the review of...

A requestor must submit a written request for incentives provided by this Policy to the Office of Economic Development, Xxxxxx Town Xxxx, 0000 Xxxx Xxxxx Xxxxxxx, Xxxxxx, Xxxxx. 2. Upon receipt of said written request, the requestor shall be required to complete the Economic Development Department’s Application for a Chapter 380 Grant. 3. The City shall have the right, if it deems necessary, to request and examine the owner’s/developer’s business plan and financial information that may be applicable to the requestor’s project. 4.
