Short Term Disability, Long Term Disability a) The sickness occurs during the period of deferral and continues until the moment the leave is scheduled to occur: In this case, the employee may choose one of the following: (i) The employee may continue their participation in the deferred salary leave contract and postpone the leave until they are no longer sick. The employee then receives salary continuance, in accordance with article 35 (Salary Continuance) of the collective agreement on the basis of the employee's net remuneration. (ii) The employee may terminate the contract and thus receive the amount of deferred remuneration. In accordance with article 35 (Salary Continuance), the salary continuance payment is based on the employee's actual remuneration. (iii) In the event that the employee becomes eligible for long-term disability benefits, the contract is terminated and the employee thus receives the amount of deferred remuneration. Long-term disability benefits are based on the employee’s actual remuneration. b) The sickness occurs during the period of leave: For the purposes of application of article 35 (Salary Continuance), the sickness is deemed not to have occurred during the period of leave. However, article 35 will apply retroactively to the date of onset of disability if, at the end of the period of leave, the employee continues to be sick. The employee is entitled, during the period of leave, to the amount of deferred remuneration according to the modalities provided for in clause 26.14. At the end of the period of leave, if the employee continues to be sick, they will then receive salary continuance payments based on their actual remuneration.

Short Term Disability, Long Term Disability. Coverage details for Short Term Disability and Long Term Disability are contained in the jointly agreed to brochure entitled “Short Term Disability and Long Term Disability Plan”. For periods of sick leave longer than 15 days Short Term Disability benefits will be provided at 66 ⅔ of the base income. The premium is payable by the employee to a maximum of .5% of base income. Short Term Disability benefits are non-taxable. Long Term Disability benefits will be 66 2/3% of base income and will be taxable. The Long Term Disability premium is payable by the employer. In the event of denial of the LTD benefits the employee will continue to receive STD benefits until completion of an LTD appeal process. Extended health benefit coverage, life insurance coverage and employer/employee pension contributions (employee contributions to be maintained at the same level they were at commencement of the medical absence) will be provided by the Company for the period of medical absence and service credit/seniority continues to accrue. All major medical absence forms will be completed for any absence of 5 continuous days or more or when requested by management. The Company will compensate the employee for the cost associated with completing these forms. Employees will be required to submit all forms required by management through their personal physician. Any discipline related to sick leave that is imposed and grieved by the union will be referred directly to an Arbitrator for resolution after the Facility Manager has a reasonable opportunity to resolve the issue. Employees who are on sick leave for 30 days or more may be eligible to participate in a vocational rehabilitation program in accordance with the Company’s policy.
Short Term Disability, Long Term Disability. Short Term Disability and Long Term Disability coverages cease as of the Retirement Date. 2004 Annual Bonus Plan. You will be paid any amounts payable under the Company's Senior Executive annual bonus plan for 2004 on the normal schedule for calculations and payments. Short Term Disability, Long Term Disability. 31.1 Short Term Disability Pay a. If an employee becomes ill or disabled and unable to work, the employee will be eligible for Short-term Disability (STD). New employees may utilize PTO, to the extent permitted by Article 14 or Article 15 (as applicable to the employee), upon becoming ill/disabled. If circumstances warrant, the Company may require the employee on STD to provide medical certification of the employee's inability to work due to illness or disability. The Company may also require medical certification that the employee may return to work following an absence due to illness or disability. Medical updates may be required periodically during such a leave, and medical certification will be required, including an independent medical examination, paid for by the Company, at the Company’s sole discretion. Eligibility for STD begins after six (6) months of continuous full-time employment. b. Coverage under the STD program begins on the sixth (6th) consecutive workday of an employee’s absence due to illness or disability. The first five (5) consecutive working days of absence due to illness will be deducted from an employee’s PTO allocation. If an individual does not have any PTO time available these first five (5) days are unpaid. An employee who needs to take an individual day(s) for illness is to use PTO. c. Beginning on the sixth (6th) consecutive working day, the employee will be covered by the Company’s STD plan and procedures. The plan provides one hundred percent (100%) pay from the sixth (6th) consecutive (work) day the employee has been absent due to illness/disability through the thirtieth (30th) consecutive calendar day of that illness. The thirtieth (31st) through the ninetieth (90th) day are paid at seventy-five percent (75%) of regular base pay, and the ninety-first (91st) through the one hundred eightieth (180th) day are paid at sixty- seven percent (67%) of regular base pay.
Short Term Disability, Long Term Disability. Effective July 2017, the short and long term disability periods will be changing as follows: o Short Term Disability coverage will begin upon application after seven (7) consecutive days. o Long Term Disability coverage will begin upon application after ninety (90) consecutive days. o Employees will be able to elect to utilize sick leave at 100% for personal illness provided they have a sufficient balance to cover the anticipated duration of the leave. o Employees will retain job protection during their FMLA period or the approved Short Term Disability period, whichever is longer. o Employees unable to return to work at the conclusion of the Short Term Disability period may have access to additional job protection leave on a case-by-case basis.
