COLLEGE RESPONSIBILITIES a. The College will place in the Fieldwork only those Interns who are registered in The Sage Colleges, Program of Dietetic Internship , and who have successfully completed any appropriate prerequisite courses and/or other clinical educational experience. b. The College will select and assign the Interns, who shall not be subject to any employment recruitment process or screening by the Facility. c. The College will inform the Facility of the level of training each student has received and will provide current information regarding the student’s clinical educational goals. d. The College shall determine the expectations and requirements for Fieldwork participation, and award academic credit for the Intern’s successful completion of the Fieldwork. e. The College shall provide the Facility Representative with copies of applicable curricula. f. The College shall notify the Facility of any changes in the academic curriculum. g. The College shall provide time and opportunity for orientation of Facility personnel to the Dietetic Internship Program of the College. h. The College shall notify the Facility when there is a change in the staffing of Academic Fieldwork Coordinator. i. The College’s assignment of Interns to the Facility for practical experiences shall be for a pre-determined period of time, which shall not be extended. j. The College shall contact the supervising clinical instructor during the affiliation to discuss student performance. k. The College shall have the right to remove an Intern from the Fieldwork at any time, in accordance with the College’s policies. l. The College shall ensure that all Interns obtain and maintain health insurance covering accidents and sickness with such companies and with such terms and limits as the College shall reasonably require. m. The College shall ensure that all Interns obtain and maintain personal professional liability insurance in the amount of $1,000,000/ $3,000,000 with such companies and with such terms and limits as the College shall reasonably require. n. The College shall ensure that all Interns are aware of and have complied with the Facility’s policies regarding the need for any health screenings, vaccinations, criminal background checks and/or other Facility requirements prior to the commencement of the Fieldwork. o. COLLEGE RESPONSIBILITIES. Each semester, College shall advise its Students of the nature of the Services that the Agency has available according to information that the Agency provides. The College may have access to the Agency to observe and evaluate the Students' delivery of the Services.
COLLEGE RESPONSIBILITIES. The College shall have the sole right to determine which property shall be classified as surplus property and transferable to the Entity in accordance with this Agreement.
COLLEGE RESPONSIBILITIES a. The College shall be solely responsible for determining and administering the educational program offered by it pursuant to the Agreement, and for offering and/or canceling classes. All persons employed by the College shall be selected and hired solely by the College, shall be its employees exclusively and shall be subject solely to its direction, control, compensation, evaluation, and discharge. The College shall bear all liabilities and expenses imposed by law or contract incident tosuch employment, including, but not limited to, workers' compensation insurance, unemployment insurance, Social Security contributions, if applicable, and tax withholdings. b. The College will name and provide a program coordinator/director that will serve asthe contact to the District and work with the identified school administrator to ensure the program runs smoothly and all details are covered. c. The College will hire all instructors providing they meet the College minimum qualifications. All instructors will be paid by the College. d. The College will hire all substitutes providing they meet the College minimum qualifications for the subject they will substitute. All substitutes will be paid by theCollege. e. The College will provide a mandatory orientation meeting prior to the first day of instruction. Date and time to be arranged by the College in consultation with the District. f. The College shall provide the District with a complete accounting of the enrollmentof persons attending classes. Classes must maintain a minimum enrollment of 25 students throughout the term. The College reserves the right to cancel a class or program in case of insufficient enrollment, or at the discretion of the College. Changes to the minimum enrollment for specific classes shall be by mutual writtenagreement and made a part of this MOU. g. The College will provide the District with all required documentation and forms;including, but not limited to, course outlines of record, applications, registrationcards, attendance rosters, timecards and instructional sheets.
COLLEGE RESPONSIBILITIES. College shall have the following major responsibilities: 1. Provide the coursework and program of which the Internship is a part. 2. Coordinate with on-site Agency staff concerning observations, evaluations, and other Internship activities. 3. Adhere to applicable rules and regulations of the Agency.
COLLEGE RESPONSIBILITIES. The COLLEGE shall: A. Give FACILITY at least thirty (30) days written notice of a STUDENT assignment unless this notice is specifically waived by the FACILITY by agreeing to this in less than 30 days. B. Reserve the right to revoke any assignment prior to the STUDENT’S entry into the clinical practicum and/or internship/externship program of the FACILITY. C. Contact or visit the FACILITY for the purpose of monitoring student progress and performance and facilitating information exchange between the COLLEGE, FACILITY, and STUDENTS. D. Provide supervision for STUDENTS as required by the program-related appropriate licensing board and/or accrediting body. E. Reserve the right to withdraw any STUDENT from assigned clinical practicum and/or internship/externship rotation at the FACILITY when, in the COLLEGE’S judgment, the clinical practicum and/or internship/externship experiences do not meet the needs of the STUDENT or if the STUDENT’S performance does not meet the COLLEGE’S specifications. The COLLEGE will notify FACILITY in the event the STUDENT can no longer participate in the clinical practicum and/or internship/externship experience. F. Provide general liability protection in the amount of one ($1,000,000) million per occurrence and three ($3,000,000) million aggregate limit, covering STUDENTS acting within the specific authorization of the COLLEGE’S scope of the clinical practicum and/or internship/externship. Upon request by the FACILITY, the COLLEGE shall provide verification of liability coverage. G. Provide FACILITY with a statement of expectations and objectives of curricular and clinical education, upon request. H. Inform STUDENTS of their responsibilities under this Agreement as outlined in Appendix A.
COLLEGE RESPONSIBILITIES. The College shall assign a Faculty Supervisor to a student participating in an internship/co-op. The Faculty Supervisor shall consult in establishing learning objectives, working with the student and site supervisor(s) to ensure a productive learning experience. The Faculty Supervisor shall establish consistent communication with the student. The Faculty Supervisor shall establish a method of grading the student. The Faculty Supervisor shall consult with the site supervisor(s) regarding the student’s performance at least twice during the off-campus learning experience. The Faculty Supervisor shall determine whether a student has satisfactorily completed an internship/co-op and award a grade based on the student’s performance, accomplishment of the learning objectives and required experience. The Faculty Supervisor and/ or the Career Development Counselor shall assist the student or employer with addressing/resolving issues or disputes that may arise at the internship/co-op site.
COLLEGE RESPONSIBILITIES. The COLLEGE will provide the SCHOOL instructor with necessary information to prepare for courses (such as Orientation, COR, Canvas support, etc.).

To assume full responsibility for the planning and the execution of the curriculum for its students including the administration, curriculum content, and Faculty appointments. b. To provide an assignment schedule of dates for the affiliation periods throughout the academic year. c. That students assigned for internship experience will receive no compensation. d. To require that students conform to the rules, regulations, and policies of Agency. These rules, regulations and policies will be available and reviewed with the students and Faculty by the Agency. e. To require student’s statement of health screening to include: 1. physical exam 2. proof of negative reaction to two (2) consecutive Mantoux tests given within three (3) weeks, or if positive reaction, negative chest x-ray result on file 3. rubella, rubeola, varicella and mumps immunity as proven by blood test or written physician’s confirmation 4. hepatitis B vaccine or signed waiver, if student refuses to be immunized 5. current CPR certification f. To provide medical documentation of any special physical needs of students and to provide for any special allergic needs of the students. g. Student curriculum, attendance, and scheduling shall be under the direction of the College as long as they do not conflict with the Agency’s policies, rules, and regulations. In the event of such conflict, the parties shall meet in an attempt to resolve same.
