Employee Physical Sample Clauses

The Employee Physical clause requires employees to undergo a physical examination as a condition of employment or continued employment. Typically, this clause outlines when and how the physicals are to be conducted, such as before starting work or periodically during employment, and may specify who bears the cost or which medical professionals are authorized. Employee Physical. 1. The Board shall schedule and provide annual physicals to those employees whose job descriptions require it, at no employee cost, by June 30, at a provider with more than one physician. 2. An employee who elects to receive his annual physical from his personal physician shall be responsible for any cost incurred. The personal physician selected must be approved by the Lake County Educational Service Center.

Employee Physical. The College will pay for a biennial physical for employees who are covered under the JCC medical plan, are forty-five (45) years or older, and the physical exam is administered by a PPO network provider. Physicals administered by a non-PPO network provider are not covered.
