Annual Physicals. Effective October 1, 2019, employees are eligible for reimbursement of up to $250.00 dollars of the coinsurance cost of an annual physical. The following criteria must be met in order to be eligible for reimbursement: •Only one physical will be covered by reimbursement within a fiscal year. •Each physical must be six months or greater from the prior year’s physical. •At a minimum, the physical must meet the same standards as a pre-employment physical required by the City of Porterville Fire Department. •The physical must be conducted by an in-network physician. •Any and all additional expenses related to the annual physical will not be covered by the employer (i.e. travel, lodging, etc.). Annual Physicals. Bus drivers and bus assistants shall submit to and satisfactorily pass a physical once every year. The cost of the physical shall be borne by the Board and shall be conducted by Board-approved physicians.
Annual Physicals. The Employer and the Union agree that each employee shall be required to undergo a complete annual physical examination by the County Physician. It shall be the responsibility of each individual employee to schedule and complete his or her own physical examination during the year in accordance with the following schedule: January - March Employee's Last Name Beginning A-F April - June Employee's Last Name Beginning X-X July - September Employee's Last Name Beginning M-R October - December Employee's Last Name Beginning S-Z Should the employee desire to undergo a physical examination by their own physician at their own expense, if not covered by insurance, in lieu of the County Physician, they should obtain the county physical examination forms, and after completion by their physician, the forms should be forwarded to the Bay County Physician for his review. The results of all physical examinations shall be held in confidence by the Personnel Director and the Sheriff, and the information contained therein released on a "need to know only" basis. Any employee who fails to set up and complete their physical examination either by his/her own physician or through the County Physician, shall be subject to disciplinary action. If the employee fails to obtain a physical he/she shall be given a thirty (30) day notice to attain a physical before any disciplinary action is taken. The Sheriff may take appropriate action based upon the physical exam results.
Annual Physicals. (A) Research shows that there are higher instances of cancer amongst firefighters. Annual physicals shall be consistent with current NRS statutes. Annual Physicals. A. Physical exams will be provided annually by the City at a maximum cost to the City of $300.00 per bargaining unit member, for all employees covered under this Agreement. A Labor Management Committee will select the physical exam vendor for the term of this contract B. All bargaining unit employees will be eligible to undergo the physical exam annually. Bargaining unit employees will have access to the City’s Health and Wellness Clinic for other medical issues throughout each year. C. The components of the physical exam will be determined by the Chief of Police in consultation with the City’s Risk Manager and may be revised from time to time upon recommendation of the City’s Risk Manager. The timing and logistics of the annual physical examinations will be determined by the City and the agreed-upon provider. D. An employee may elect additional exams or services offered by the agreed-upon provider but will be responsible to pay out-of-pocket (or if covered by the employee’s health insurance) for any additional fees incurred. E. Should a bargaining unit member be identified as "not fit for duty" by the agreed- upon provider or other relevant examination, the employee will be permitted to return to full duty if he or she can provide a written clearance from the employee’s personal physician specializing in the condition identified by the agreed-upon provider indicating fitness for full duty under the provisions outlined in the current job description for the employee’s job classification without regard to any additional standards. The written clearance from the employee's personal physician must specifically address the condition that resulted in a "not fit for duty" recommendation. The employee must provide the Form developed by Risk Management to be signed by the attending physician (see attached). Employees providing such "fitness for duty" certification from their personal physician shall not be required to retake or repeat any portion of the examination successfully completed, including the portion of the test failed; however, the employee must complete all other portions of the examination. Annual Physicals. Forestry‌ Employees in job classifications covered under NRS 617 are required to participate in an annual physical examination. Annual physicals will be scheduled during working hours and will be considered work time. Office of the Military‌ Employees in job classifications assigned to the Office of the Military are required to participate in an annual physical examination pursuant to NRS 617 and the National Fire protection Association (NFPA) Standard 1582, and any amendments thereto. Annual physicals will be scheduled during working hours and will be considered work time. The Employer and the Union agree to meet and discuss any future revisions to NFPA 1582.
Annual Physicals. (A) To xxxxxx employeeshealth and well-being, each member is encouraged to undergo an annual physical exam (APE). Employees may visit a health care provider of their own choosing. Upon the City obtaining proof of the completion of the APE, the City shall pay the employee $350.00 at the same time the longevity payment is made. (B) The City shall provide a medical evaluation by a doctor paid for by the City. The standards for the medical evaluations shall be specified in departmental policies.
