Such leave shall be granted only for health of self or family member, provided the following requirements are met: 1. Employees seeking such leave must make application on the form provided by the Office of Leave/Retirement/Unemployment Compensation. 2. Such application must be supported and accompanied by the following: a. identity of person in ill health; b. statement from a M-DCPS-certified physician explaining why such leave is necessary; and c. effective dates of requested leave (beginning and ending). 3. Such leave shall not exceed one year in duration. 4. Employees on such leave, upon expiration of leave, may apply for an extension. The employer may grant such extension, as warranted; however, the maximum time an employee may be absent on such leave shall be three years. 5. Such leave granted to annual contract employees shall expire no later than the end of the fiscal year in which the leave was granted. Such leave may be automatically extended whenever the employee is reappointed and his/her contract extended. 6. While an employee is on extended sick leave without pay, the employer agrees to continue to pay its regular contributions to the employee's insurance benefits. 7. An employee who has been granted extended sick leave without pay for illness of self may return earlier than indicated on the original request for leave form, upon receipt of an unconditional medical release form from his/her physician.

Identity of person in ill health; 2. Statement from physician explaining why such leave is necessary; and 3. Effective dates of requested leave (beginning and ending). C. Such leave shall not exceed one year in duration. D. Employees on such leave may, upon expiration of leave, apply for an extension. The employer may grant such extension as warranted; however, the maximum time an employee may be absent on leave for illness of self, illness of relative, or any combination thereof shall be three years. E. While an employee is on extended sick leave without pay, the employer agrees to continue to pay its regular contribution to the employee's insurance benefits.
Extended Sick Leave Without Pay. At the termination of his accumulated sick leave, an employee who cannot perform scheduled duties because of extended illness or injury shall, upon written request, be placed on an extended sick leave of absence without pay up to the end of the current school year in which the request is made. The employee must give three (3) weeks’ notice to the District, in writing, prior to returning. An employee may return to work earlier than three (3) weeks if mutually acceptable between the employee and the District. A second extended sick leave of absence without pay shall, upon written request, be granted up to the end of the current school year in which the request is made. Upon granting the request, an employee may return to work before the end of the year if mutually acceptable between the employee and the District. If not mutually acceptable, and if the employee desires to return to work prior to the end of the school year, the District will make every reasonable effort to employ that employee as a substitute with substitute benefits. A third extended sick leave request in any three (3) year period shall be at the District's discretion. An employee on extended leave as a result of a state recognized on-the-job injury, shall be allowed to extend such leave to cover a total leave of at least three (3) years. An employee on any extended sick leave will hold his/her place on the salary schedule, and will be eligible for the scheduled increment the following year provided he/she has completed one- half (1/2) or more of the contractual year in which he/she was placed on extended sick leave of absence. Please refer to the Staff Handbook for information on the Family Medical Leave Act (FMLA) and the Oregon Family Leave Act (OFLA).
Extended Sick Leave Without Pay. Employees shall be granted extended sick leave of absence without pay of up to six (6) months with up to one (1) year of service, and twelve (12) months if over one (1) year of service, beyond the paid sick leave entitlement provided in Article 11.01, during periods of lengthy illness or disability as certified by a medical doctor. During that period of leave beyond the paid sick leave entitlement, seniority will be retained.

Upon returning to duty after sick leave with full pay or extended sick leave without pay, a member of the bargaining unit shall be returned to the same position held before the beginning of sick leave. If the position no longer exists, the teacher shall be assigned to a substantially 3-3.3. All rights, privileges, and benefits to which a teacher was entitled at the time his/her absence commenced will accrue to him/her upon his/her return to his/her position.

Extended Sick Leave Without Pay. If any employee is ill for a prolonged period of time and because of such long period of absence he/she has exhausted all of his/her paid leave credits, such employee, upon request, may be granted an extended sick leave of absence without pay, not to exceed one (1) year, but not more than once during each four (4) years of continuous service.

Extended Sick Leave Without Pay. (a) An employee shall be granted up to three (3) months extended sick leave without pay in addition to the fifteen (15) week paid sick leave referred to in Article 10.02 of this Article, provided that the employee has a minimum of one (1) year’s continuous service with the Employer, the employee does not qualify for Long Term Disability Plan benefits and the Employer has received a letter from a certified medical doctor stating the period of sick leave expected. (b) Prior to commencement of extended sick leave, the employee shall prepay fifty percent (50%) of all Welfare Plan premium costs for the period of extended sick leave without pay. (c) An employee shall continue to accrue seniority during the fifteen (15) week period of paid sick leave and the three (3) month period of extended sick leave without pay. (d) An employee who is unable to return to work at the expiration of extended sick leave without pay, shall be placed on the recall list for a further period of twelve (12) months.
