Group Benefits Plan. 20.1 The Group Benefits Plan (hereinafter referred to as the Plan) refers to life insurance, accidental death and dismemberment insurance, supplementary health benefits (hospitalization, major medical, vision care, prescription drugs), dental benefits and long term disability insurance as outlined in the applicable group insurance policies, and the Alberta Health & Wellness plan. The Board shall provide a pay direct card for employees participating in the Group Benefits Plan.
20.2.1 Participation in the Plan shall be a condition of employment. Employees must complete an enrolment card within thirty-one (31) days of the first day of employment. There is no waiting period for eligibility.
20.2.2 An employee who is on layoff pending recall or who is absent for more than four (4) weeks on a Professional Improvement Fellowship, secondment, leave of absence without pay or a deferred salary leave, must pay the full cost of such benefits in advance of the absence if the employee chooses to maintain their coverage. When an employee wishes to continue participation in the benefit plans during maternity leave or adoption leave, the premiums shall continue to be shared between the Board and the employee pursuant to this Agreement. Notice of the employee’s intention to continue participation in the benefit plans must be provided to the Board at the same time the maternity leave or adoption leave is requested. When the employee works one (1) day in any calendar month, benefits will be provided for that calendar month and the premiums shall be shared between the Board and the employee pursuant to this Agreement.
20.3.1 The cost sharing of the Plan between the Board and the employees under this Collective Agreement shall be in the following proportions: Board Employee Life and Accidental Death and Dismemberment 100% 0% Supplementary Health Benefits 100% 0% Dental Insurance Plan 100% 0% Long Term Disability 0% 100%
20.3.2 The parties acknowledge that prior to January 1, 2009, the Board contributed 100% of the cost of the Alberta Health Care Insurance Plan (AHCIP), referred to in previous collective agreements as “Alberta Health & Wellness”. In the event that premiums for ACHIP or a substantially similar plan are reintroduced, the Board and Association will negotiate the Board’s contribution to the premiums prior to the required implementation date for premium collection.

Group Benefits Plan. 50.01 The Northern Employee Benefits Services (NEBS) Group Benefit Plan:
(a) Basic Group Life Insurance (3 x annual salary);
(b) Accidental Death, Disease & Dismemberment (max. 3 x annual salary);
(c) Dependants Insurance;
(d) Long Term Disability (60% non-taxable);
(e) and Short Term Disability (Weekly Indemnity 60% non-taxable) plan are terms and conditions of employment for all eligible employees.
50.02 The Northern Employee Benefits Services (NEBS) Extended Health Care and Dental Insurance plans are optional plans available to each individual eligible employee.
Group Benefits Plan.
24.02.1 The Employer shall pay the premiums for all employees, their spouses and eligible dependants, for the Group Benefits plan available to the Employer through the Canadian Union of Public Employees or, with mutual agreement of the parties, an equivalent plan. Employees, their spouses, and eligible dependants must comply with all requirements set forth by the insurer in order to receive benefits.
24.02.2 The Union acknowledges that the Employer has no control over the terms of the Group Benefits Plan available to the Employer from the Canadian Union of Public Employees or such other plan as is in effect as per Article 24.
02.1. Should any benefit available under the plan be reduced during the term of this Agreement, the Employer shall endeavour to minimise the effect of such a reduction in consultation with the Labour-Management Committee.
Group Benefits Plan. 19.01 (a) For all relevant Group Insurance Benefits Plan provisions refer to the appropriate benefit booklet(s) from the Company’s benefit provider. Decisions regarding benefits are made solely by the third party benefits provider, and the Company's obligation hereunder is limited to paying premiums for such coverage.
Group Benefits Plan.
15.1 The Company agrees to provide a group insurance plan for all employees as of their date of hire. The plan includes basic group life insurance, group disability income insurance (STD & LTD), basic accidental death, dismemberment and loss of use of insurance, extended health and dental plan.
15.2 The coverage will be shared at a 70% employer/30% employee ratio.
15.3 A summary of benefits is included in the brochure available in the Human Resources Department.
15.4 A Group Benefit Committee will be established no later than September 1st, 2013 and will be comprised of a minimum of two (2) unionized member and two (2) Management representatives. The objective of the committee is to ensure the members’ needs are well represented during the negotiation of the plan’s renewal.
Group Benefits Plan. The Northern Employee Benefits Services (NEBS) Group Benefit Plan {i.e. Basic Group Life Insurance (3 x annual salary); Accidental Death, Disease Dismemberment (3 x annual salary); Dependants Insurance; and Long Term Disability taxable)} and Short Term Disability (Weekly Indemnity non- taxable) plan are terms and conditions of employment for all eligible employees. The Northern Employee Benefits Services (NEBS) Extended Health Care and Dental Insurance plans are optional plans available to each individual eligible employee. The Employer shall advise the plan administrators of any adjustments to subject to these plans, terminations of employees covered by these plans, new eligible employees under these plans, and other required data as determined by these plans without delay. The Employer shall remit all required premiums for the plans under this Article within a reasonable period, and shall forward all claims under these plans in a timely manner. The Employer shall distribute to all employees eligible for coverage under the plans in this Article all literature, statements and materials produced by NEBS and the insurers, which are intended for distribution to the employees. New eligible employees shall be provided with plan booklets upon hire and shall be enrolled in a timely manner. All issues concerning the insurance plans, including issues of premiums (approximately a split depending on tax effectiveness), and eligibility for benefits shall be determined by the benefit plans providers.
Group Benefits Plan. The Buyer hereby assumes all liabilities under the Quanex Corporation Group Benefits Plan for all participants who are employees or former employees of the Company, as set forth on Schedule 4.1(b) of the Disclosure Schedule, and their dependents, as of the Closing Date (the "LaSalle Group Benefit Plan Participants") subject to the terms and conditions of such plan. Effective as of the Closing, the Seller shall (i) cause the Company to terminate its participation as an adopting employer with respect to the Quanex Corporation Group Benefits Plan and (ii) cause that portion of the Quanex Corporation Group Benefit Plan covering the LaSalle Group Benefits Plan Participants to be spun-off into a new welfare benefit plan entitled the "LaSalle Steel Company Group Benefits Plan" which shall be sponsored by the Company and which, as of the Closing, shall otherwise be substantially identical to the Quanex Corporation Group Benefits Plan.

The Employer shall pay the premiums for all full-time Employees, their spouses and dependents, for the Group Benefits plan available to the Employer through the Canadian Union of Public Employees. All full-time Employees must comply with all requirements set forth by the insurer in order to receive benefits. It is understood that this Article includes retroactive coverage of Limited Term Disability premiums to January 1, 1995, for the Staff Representative employed on that date.
Group Benefits Plan. Great-West Life group benefit plan Group policy No. ("the Plan") shall be provided to all employees effective date of ratification. The following outlines the main features of the Great-West Life group benefit plan coverage and is not meant to be exhaustive: current employees are eligible to participate in the Plan on date of ratification of this Agreement. Thereafter, newly hired employees are eligible to participate after three (3) months of continuous employment.
