Health Care Cost Containment. The Employer at its option may implement any of all of the following health care cost containment programs:
(a) Pre-admission certification of the necessity of hospitalization (BC-BSM predetermination program or equivalent).
(b) Excluded from reimbursement under the prescription drug program are cosmetic drugs and non-prescription smoking cessation aids.
(c) Excluded from benefits coverage are maternity benefits for persons acting as surrogate mothers.
(d) When more than one family member is employed by the Employer, there shall be no duplicate coverage by Employer health plans.
(e) In the event of any payment under the Employer’s health insurance plan on behalf of any person covered by such insurance plan, the Employer shall be subrogated to the extent of said payment to the covered person’s right of recovery therefore against any persons or organization in a tort action. It is further understood between the parties that subrogation applies to direct medical expenses paid and not to subjective damages such as “pain and suffering.”
(f) In a joint continuing effort to control the cost of insurance, the Employer and the Union agree to a strict coordination of benefits program which is designed to prevent people from making a profit on health insurance by collecting more than the actual cost of covered services. Under this program, the benefits payable under Employer health insurance and any other group health insurance policy which an employee or any covered dependent may have will not exceed the total amount of medical expenses.
Health Care Cost Containment a. Any premium contribution will be paid by the applicable employee through payroll deductions. All payroll deductions will be made pursuant to a qualified pre-tax payroll deduction plan.
b. Employees hired by the district after July 1, 2012 who are then or thereafter subject to this collective bargaining agreement will contribute toward health insurance, vision insurance and dental insurance. These contributions will be by payroll deduction beginning the first pay period for the school year for the applicable number of pay periods. Such deductions will be pursuant to a qualified premium-only, pre-tax payroll deduction plan. The employee contribution for dental and vision insurance will be ten percent (10%) of the applicable insurance’s cost or illustrative rate. The health insurance contribution is as indicated in paragraphs c and d below.
c. Administrators receiving the health care benefits shall pay the following monthly premium contributions for 2014-2015 based on their date of hire in the district: Hired: On or before 7-1-2012 After 7-1-2012 i. Single: $ 78.74 $ 88.74 ii. Two Person: $169.73 $212.97 iii. Full Family $229.77 $266.20
d. For fiscal years beginning after June 30, 2015, the monthly contribution for 1 person, 2 person, and full family coverage will be 15.0% of the illustrative rate that becomes effective July 1, of that fiscal year for the selected health care plan.
Health Care Cost Containment. For each year this Contract is in effect, the Board agrees to pay one hundred ten percent (110%) over the prior year’s Board-paid portion of the monthly premium cost for applicable coverage outlined in this Article. Any premium amounts in excess of one hundred ten percent (110%) of the prior year’s Board-paid rates will be shared equally by the employee and employer on a cumulative dollar for dollar basis through payroll deduction, in addition to any employee contribution specified above.
Health Care Cost Containment. A. The City and Union recognize the mutual benefit to be achieved from health care cost containment.
B. It is mutually agreed that discussions concerning health care cost containment and issues involving employee health will continue outside of the contract negotiation setting.

Health Care Cost Containment. The Union and the Board of Education agree to form a Healthcare Cost Containment Committee made up of an equal number of members from the Union and the Board of Education or designees, which will review and agree to future cost containment programs to cover both active employees and future retirees during the term of this Agreement. Said cost containment programs shall not diminish the level of benefits provided in the basic plans. The parties are committed to investigate programs, which will reduce costs. Programs to be considered would include alternative health care providers, additional cost containment programs and alternative traditional plans. Any program(s) agreed to by the parties will be implemented during the term of the agreement subject to ratification of the membership.
Health Care Cost Containment. Use of Recreation Center
1. Employees must purchase a three-month, unlimited, usage card from the Recreation Division.
2. Cost of the Card is $45
3. It is understood that employees who purchase said card shall not be eligible for reimbursement under the City Wellness Program, and that the Card may be used only by the employee.
4. Said card may be purchased from the Recreation Division upon presentation of valid City employee identification.
5. Employees may utilize the Recreation Center during times when the Center is open to the general public. In addition, employees of the unit may utilize the gymnasium and weight room facilities between the hours of 6 a.m. and 8 a.m. provided:
a. A Recreation employee is on duty during those hours to provide access at the facility;
b. Use of the facility during said hours does not interfere with the activities of maintenance employee(s); and
c. Employees must leave the facility in an orderly and clean condition. Should employees not comply with the above, the Recreation Manager may restrict employee use of the facility during the hours of 6 a.m. and 8 a.m.
Health Care Cost Containment. The Union and the Board of Education agree to form a Healthcare Cost Containment Committee made up of an equal number of members from the Union and the Board, which will review and agree to future cost containment programs to cover both active employees and future retirees during the term of this Agreement. Programs to be considered would include alternative Health care providers, additional cost containment programs and alternative traditional plans.
Health Care Cost Containment. The Union and the Employer recognize that rapidly escalating health care costs, including the costs of medically unnecessary services and inappropriate treatment, have a detrimental impact on the health benefit program. The Union and the Employer agree that a solution to this mutual problem requires the cooperation of both parties, at all levels, to control costs and to work with the health care community to provide quality health care at reasonable costs. The Union and the Employers are, therefore, committed to fully support appropriate programs designed to accomplish this objective. This statement of purpose in no way implies a reduction of benefits or additional costs for covered services provided under the Plan.
Health Care Cost Containment. The Health Care Cost Containment Committee, one-third of which is Service employee representatives, will continue to study cost containment measures. The Health Care Cost Containment Committee will continue to meet on an ongoing basis to monitor the effectiveness of recommended cost containment measures and to develop related promotional wellness programs. The Committee shall meet regularly as determined by the needs of the Committee. The Human Resources Division shall serve as staff to the Committee. Regular Committee members shall receive release time to attend Committee meetings. In advance of attending meetings, members shall obtain permission through supervisory channels.
Health Care Cost Containment. The City and the Union shall participate in the discussion of establishment of health care cost containment measures in addition to those which are presently part of its medical and dental benefit plans. Such measures may include, but shall not be limited to:
1. Prior authorization for non-emergency or elective hospitalization, surgical procedure or extended hospital stay;
2. Notification requirements for emergency treatment;
3. Pre- and post-admission or treatment utilization review;
4. Limitations on diagnostic testing;
5. Limitations on treatment for nervous and mental disorders, and for substance abuse, which may include but shall not be limited to required use of preferred providers;
6. A managed prescription drug program which combines a retail network with a mail order program;
