Notice of Sick Leave. Absence a. Electronic absence procedures will be followed. b. In cases of ¼, ½, ¾, and 1 whole day(s), or short-term absences, the employee will notify the building principal or his/her designee. All appropriate absence forms should be completed and submitted to the building principal. c. In other cases (such as, but not limited to, a scheduled surgery or pregnancy), when the employee knows of the need for extended sick leave in advance of the date of the absence, the employee must notify the Treasurer’s office in writing. The notice to the Treasurer’s office must include the date on which the leave is expected to begin and its anticipated duration as soon as the employee becomes aware of the fact that illness or disability will require sick leave. All treasurer notification documents should be copied to the building principal. d. Should there be any changes in the expected starting date or duration of the leave, the employee must promptly advise the Treasurer. While on extended sick leave of more than one (1) month, the employee must advise the Treasurer or Superintendent, if requested, at least monthly of his/her status and anticipated return date. In addition, this paragraph will apply to unpaid leaves of absence approved because of illness or disability.

Notice of Sick Leave. Employees reporting sick shall advise their supervisors fifteen (15) minutes before going on their regular shift in the case of day shift employees, and four (4) hours before going on shift in the case of employees on afternoon or evening shifts in the Civic Centre and one (1) hour in other Departments.

Notice of Sick Leave. ‌ Employees are required to provide as much notice as possible when calling in sick prior to their shift beginning. They should provide a reason as to why they are not coming to work as well as the time and date they expect to return. If an Employee is absent for three (3) days or more, they may be required prior to beginning their shift to present a doctor’s note to their supervisor, the Company or the supervisor will make this request prior to their return as it deems appropriate. The doctor’s note will clearly state the employee is able to return to work as a food handler or office staff employee. If the employee fails to present a note the Company may send the employee to get one at the employee’s cost before beginning their shift. The above note also applies to those employees who have been absent on Short Term Disability, medical leave of absence, or a work-related injury (WCB) where the employee is off the remainder of the injury day or beyond the date of the injury regardless of the length of absence. For Absences of less than three (3) days, the Employer will not require doctor’s notes from employees unless: 1. The employee has been formally advised that their attendance record is unacceptable and that a doctor’s note will be required in the future; or 2. The duration of the absence or circumstances surrounding the absence requires justification. 3. The absence is related to a work-related injury as noted above.

In the event notification is not possible through the Absence Management System, the employee shall notify his/her immediate supervisor, or authorized designee by email, text, or phone. At least one day prior to his/her expected return to work, the employee must notify his/her supervisor in order that any employee filling the position on a substitute basis may be terminated. b. In addition, the absence must be entered in the District Absence Management System. If help is needed, the employee can first refer to the site Office Manager; if further help is needed, the employee should contact their school’s assigned HR substitute help person. At the end of the month the employee shall sign the appropriate absence confirmation form.

Payments for unused sick days in amounts greater than $5,000 will be paid in equal amounts over five consecutive years beginning with the year of retirement. e. Payments for unused sick days in the amount at $5,000 or less or for the death of an employee will be paid in a lump sum to the employee or the employee’s estate in the year of retirement or death.

  • Extension of Sick Leave Unless an employee otherwise requests in writing, an employee who has used up her sick leave credits shall be granted advanced sick leave with pay for a period up to fifteen (15) days subject to the deduction of such advanced leave from any sick leave credits subsequently earned.
