Reporting Injuries. All on-the-job accidents or illnesses, no matter how minor in nature, shall be reported by the injured employee to the employee’s immediate supervisor or other person designated by the District, who will arrange treatment of the injury if necessary. Failure to so report an accident or illness may result in disciplinary action.
Reporting Injuries. If injured on the job, no matter how slightly, employees must report the incident immediately to a supervisor. Employees must complete the necessary workers' compensation forms following any injury. The Finance Director must be given the information as soon as possible. The Workers’ Compensation carrier must also be contacted prior to seeking medical attention (unless it is a life-threatening injury).
Reporting Injuries. 271 An employee shall not fail to report an injury immediately to their supervisor no matter how small it may seem. In case the supervisor is out of their department, the injured employee shall report the injury to the department xxxxxxx or designated employee. 272 If it is necessary for an employee to go to the First Aid Room, they will notify their supervisor. In case of an injury requiring emergency attention, the employee should go to the First Aid Room immediately. 273 Medical attention for industrial injuries must be authorized by the Company prior to receiving attention, except in cases of emergency.
Reporting Injuries. If an employee is injured at work, the employee is required to report it immediately or as soon as practicable to his supervisor and the Human Resources Department. The report must be filed immediately following the occurrence of an accident or injury. The employee will be paid the balance of their scheduled work hours on the day of injury if time is missed due to injury. The Company will provide transportation for medical treatment if necessary. Seniority will continue to accumulate during the period of the workers’ compensation leave. All benefits will continue from the start of the workers’ compensation leave of absence.
Reporting Injuries. 1. An employee shall report as soon as possible to his Supervisor, Department Head, or to the Human Resources Manager any on-the-job injury regardless of the extent of the injury. Without good cause, failure to report an injury immediately is a violation of City policy and the employee may be subject to disciplinary action.
2. The Supervisor/Department Head shall secure first aid and determine if the employee needs further medical attention. If any reasonable doubt exists, the employee should be examined by a medical doctor/medical facility selected by the City.
3. With the approval of the City, an employee may use his own personal physician. However, the City reserves the right to have the City’s physician examine the employee prior to and during the treatment except for emergencies. The employee and his Supervisor must obtain a medical authorization form from the Human Resources Director prior to such alternative treatment. Exceptions to this procedure may be granted in the case of an emergency but a telephone call must be made by the Department Head or his designee informing the Human Resources Director of the injury. Injured employee will be treated by physician selected by the City.

Reporting Injuries. (1) Employee shall report an on-the-job injury, or potentially hazardous exposure, as soon as possible, to his Supervisor, Department Head, or Human Resources Manager, regardless of severity or extent.
(a) Failure to immediately report such injury, without good cause, is a violation of City policy and the employee may be subject to disciplinary action.

(a) Employee will supplied with a form to be completed by the attending physician/health care provider in order for payment to be made to the physician and/or hospital.
(b) Employee shall be provided a copy of such injury report.
Reporting Injuries. Any employee who receives, or suffers, any injury during business hours while performing their official duties shall report such injury within three (3) business days on approved County reporting forms. Such notification will be made first to their direct supervisor, and then to the Administrative Assistant to the Deputy Chief.

1. Notification must be made by the injured member within forty-eight (48) hours of the injury.
