Scope of Role. The Forensic Mental Health in Community Health (FMHiCH) Program will be required to provide an efficient, high quality and comprehensive service to Adults 18 and up on Community Corrections Orders (CCO) residing in the Latrobe Valley. The FMHiCH Program is a comprehensive mental health and broader health evidence-informed service. The aim of the FMHiCH Program is to "to assist eligible individuals to access the mental health care they need and enable them to satisfy the requirement of their Mental Health and Treatment Rehabilitation (MHTR) condition while reducing their recidivism risk.” It is intended that the FMHiCH Program will assist with providing timely and appropriate assessment and treatment to those who require it, and potentially reduce risk to the community. Scope of Role. To determine eligibility and assess the needs of individual veterans for home care services in a timely way, using the phone based Veteran’s Home Care (VHC) standard assessment instrument; complete relevant documentation; approve services and allocate services to VHC contracted service provider; arranging follow-up referrals and review of services; maintaining knowledge of community based health and support providers and participation in quality activities. This role delivers a phone based service for eligible Veterans located in Western Australia from our office located in Ringwood, Victoria. Scope of Role. The Management Committee shall:

Scope of Role. Reporting to Manager Youth Services and Primary Prevention including headspace, the YCEW acts as an advocate for young people and is responsible for ensuring that the views of young people and their families and friends are embedded in the development and delivery of services. The incumbent will have a good understanding of the needs of young people and is primarily responsible for empowering them to become actively involved in decisions regarding their wellbeing, and in community education activities, in order to promote positive mental health. The YCEW will be adept in working as part of a multidisciplinary team and will be an experienced professional with considerable knowledge and experience in youth advocacy and mental health service provision. The YCEW will work collaboratively with all staff, located at headspace Morwell to promote youth-focussed services to young people and will oversee the activities of the headspace Morwell/LCHS Youth Reference Group and other volunteer groups. This position will also be responsible for the planning and implementing Freeza events. They will establish and maintain working relationships with key agencies in the community to establish integrated care pathways for young people.

Scope of Role. The role encompasses provision of physiotherapy services to the whole of the Latrobe Valley community. This includes service provision to clients eligible for services under funding streams such as Community Health, HACC, Private health, Medicare Benefits Schedule, Department of Veterans Affairs and other funding as it becomes available. There will also be opportunity to participate in quality improvement projects and develop research projects.
Scope of Role. The Primary Care Partnership Strategy is funded by the Victorian government to improve the health and wellbeing of community members. This is achieved by strengthening relationships between primary care providers across a catchment area so they are able to implement integrated health promotion and planning, improved service coordination, integrated chronic disease management, and joint programs. The Central West Gippsland Primary Care Partnership’s Executive Officer will provide leadership and support to the Partnership to facilitate the development, implementation and evaluation of PCP strategic directions. The Executive Officer will also provide advice to the Partnership on all matters related to the Primary Care Partnership Strategy and other relevant initiatives. The Executive Officer plays a key role in facilitating partnership and leadership development, planning and evaluation. This includes supporting collaborative partnerships and change management, facilitating strategic planning and impact measurement, and increasing organisational awareness and building capacity around PCP strategic directions. The Executive Officer is also responsible for the development and oversight of the budget and for the management of PCP staff and other resources.

The Continence Nurse Advisor will coordinate the specialist assessment and management strategies for clients who are referred with specific continence problems using evidence based decision making. Educational, supervisory responsibilities, evaluation and quality improvement activities will also be undertaken.
