Eligibility for Payment a) i) Except in the case of a disability arising out of an accident, an employee shall be eligible to receive an amount of disability benefit in accordance with Section 3 hereof, for a period not exceeding 52 weeks for any one illness, beginning after three (3) continuous days from the commencement of the disability, or beginning on the first day of hospital confinement as a bed patient, if earlier.
Eligibility for Payment. Employee(s) not scheduled to work on a paid legal holiday as listed above will be paid for that holiday provided that the employee is on paid status the work day immediately preceding and the work day immediately following the holiday. Payment shall consist of eight (8) hours straight time.
Eligibility for Payment. Whether a statutory holiday is worked or not worked, an eligible employee shall be paid statutory holiday pay equivalent to a normal day's time at straight-time rates. Employees shall be eligible for statutory holiday pay provided they are at work either the working day before or the working day after the statutory holiday but they shall not be eligible when a statutory holiday falls during any of the following periods of absence: • paid bereavement leave • absence due to sickness or non-occupational injury • WorkSafeBC Injury Compensation absences • approved leave of absence • any absence without Employer approval In no instance will an employee receive, for the same day, both statutory holiday pay and WorkSafeBC Injury Compensation, bereavement pay, or payments under the Wage Protection Plan.
Eligibility for Payment a) An employee shall be eligible to receive an amount of disability benefit after fifty-two (52) weeks of Weekly Indemnity/Salary Continuance entitlement for the same disability under the Company's Weekly Indemnity Disability Benefit Plan.
b) An employee absent from work and in receipt of an amount of disability benefit, shall continue to receive such benefit, even though a work shortage develops which would have resulted in his being laid-off had he been at work, provided that the employee remains disabled and continues to furnish evidence satisfactory to the Company and/or insurer, and verifies the continuance of disability.
c) An employee shall not be eligible for an amount of disability benefit under this Plan unless he is actively employed by the Company at the date that he becomes eligible or until he subsequently returns to active employment.
d) In the event of a lay-off, an employee who is insured under the terms of this Plan shall be considered as still employed for purposes of this benefit up to the end of the policy month next following the policy month in which the employee was laid-off.
e) An employee making a claim for an amount of disability benefit after lay-off or termination of employment, for disability established to the satisfaction of the Company and/or insurer as having occurred prior to his lay-off or termination, shall be eligible for an amount of disability benefit provided such disability was accompanied by a continuance of absence that commenced prior to actual lay-off or termination.
f) Successive periods of disability separated by less than three (3) consecutive months shall be considered one (1) period of disability, unless the subsequent disability is due to an accident or illness entirely unrelated to the previous disability and commences after return to active employment on full-time.
g) The monthly benefit will cease at the earliest of the following occurrences:
(i) The date at which the disability ceases,
(ii) The date at which the employee is eligible for an unreduced pension,
(iii) The date at which the employee reaches 65 years of age,
(iv) The death of the employee. Current provisions will not be reduced by the above-noted language.
h) An amount of disability benefit under this Plan shall not be paid in the event the absence is a result of:
(i) any injury arising out of or sustained while doing any act or thing pertaining to any occupation or employment for remuneration or profit, or;
(ii) self-destruction or any sel...
Eligibility for Payment. Payment will be subject to the following conditions:
(a) except with the approval of Australia Post, employees employed in classifications of Administrative Officer Level 6 or Postal Manager Grade 3 are not eligible to receive payment;
(b) the provisions of clause 17.5 will not apply where an employee is recalled to duty while restricted.
Eligibility for Payment. (a) Except as described in paragraph 4(b) and subject to paragraph 4(c) and paragraph 3, the Restrictions on the Performance Shares will end (your Performance Shares will vest) on the Payment Date described in paragraph 5, but only if you remain continuously employed
by a Cigna company until the Payment Date and comply with all the terms and conditions of this grant, including those contained in this document.
(b) Notwithstanding paragraph 4(a) and subject to paragraph 4(c) and paragraph 3, if your Termination of Employment is before the Payment Date:
(1) Your Performance Shares will vest upon your Termination of Employment if it is Upon a Change of Control (of Cigna Corporation). If your Performance Shares vest under this paragraph 4(b)(1), the Shares Earned Percentage shall be the greatest of:
(a) 100%;
(b) The Shares Earned Percentage for the Performance period that ended immediately before your Termination upon a Change of Control; or
(c) The average of the Shares Earned Percentages established by the Committee for the last two Performance Periods that ended before your Termination upon a Change of Control.
(2) Your Performance Shares will vest upon your Termination of Employment if it is due to your death. If your Performance Shares vest under this paragraph 4(b)(2), the Shares Earned Percentage shall be 100%.
(3) Your Performance Shares will vest upon your Termination of Employment if it is due to your Disability.
(4) Your Performance Shares may vest upon your Termination of Employment if it is due to your Early Retirement or Retirement and if the Committee or its designee (including Cigna’s senior human resources officer) approves the early vesting before your Termination of Employment. If you want to be considered for early vesting when you retire, you must ask your manager or human resources representative far enough in advance of your retirement so there is time to process your request.
(c) You must comply in all respects with the terms and conditions of this grant, including those contained in these Terms and Conditions.
Eligibility for Payment. When any of the following holidays occur on a standard work day on which the Employees would otherwise have worked, the Company will pay each eligible Employee his payroll rate for the number of straight time hours the Employee would otherwise have worked on such paid holiday without requiring the Employee to render service during the period observed. An employee eligible for this payment will have worked his regular scheduled shift upon the last working day preceding such a holiday and his regular scheduled shift upon the first working day succeeding such a holiday. However, payment for this holiday will be made if the Employee worked sometime within sixty (60) days prior to the holiday(s) but is absent on the above days due to death in the immediate family, or has obtained prior written permission from the Management to be absent on those days. The Company will pay the difference between the regular statutory holiday pay and the Weekly Indemnity payment for the statutory holiday if the absence on the holiday(s) was due to a verified illness (i.e.: Employee on Weekly Indemnity benefits), providing the entire duration of illness has not exceeded sixty (60) days at the time of the paid holiday.
Eligibility for Payment. 8.1 Any and all payments due hereunder may be denied if not already begun, or terminated if they have begun, if in the Company’s sole judgment Executive is either not eligible for such payments, or once such payments have begun is found to be or found to have been ineligible.
8.2 Executive shall not be eligible for any payments hereunder if the Company, in its sole discretion, finds that during or subsequent to his employment with the Company he:
(a) breaches, or has breached any term, provision or obligation enumerated herein;
(b) committed any act by commission or omission which materially and substantially adversely affects the Company’s business or reputation; or
(c) is convicted of any violation of the Federal Food, Drug and Cosmetic Act, or the violation of any other statute of material relevance to the Company’s business.
8.3 Should Executive be paid any benefits hereunder and thereafter be found ineligible, or to have been ineligible, he must return to the Company that portion of the benefit paid to him for the period of his ineligibility.

(ii) In the case of a disability arising out of an accident, an employee shall be eligible to receive an amount of disability benefit in accordance with Section 3 hereof, for a period not exceeding fifty- two (52) weeks for any one accident, commencing from the date of the accident.
Eligibility for Payment. 8.1 To be eligible for any portion of the bonus payment, the participant must be employed by the company at the scheduled payment date. A participant who terminates employment prior to the payment date will forfeit the bonus including all future payment schedules, except as otherwise provided in this article.
