Extended Health Benefit Plan. The Pacific Blue Cross Extended Health Benefit Plan shall be made available to all employees. This plan shall include an eyeglass option of five hundred dollars ($500.00) every twenty-four (24) months, a hearing aid option of three hundred dollars ($300.00), lifetime limit, and coverage of registered psychologist treatments, registered social worker or clinical counsellor up to two hundred dollars ($200.00) every twelve (12) months. Premium costs shall be fully paid by the Employer. Extended health care lifetime limit to be increased to $1,000,000.00 effective January 1, 2008.
Extended Health Benefit Plan. The Extended Health Benefit Plan shall include a travel rider and an eyeglass rider: This provides travel for three (3) out of town medical referrals per calendar year for either the employee or an insured dependent. This plan also provides for a maximum of $40.00 per day for commercial accommodation. When the employee is the one going out of town for a medical referral, the Board will pay a per diem of $75.00 per day in lieu of wages for full time employees and a prorated portion for part-time employees for a maximum of three (3) days per referral. Eligible expenses under this provision shall include charges for eye glasses or contact lenses and their fitting, up to a maximum of $140.00 and a maximum of one such expense in any 2 consecutive calendar years, for each insured person.

Subject to continuing eligibility, the surviving dependents of a deceased member may retain membership at no cost in the Extended Health Care Plan (provided that the member belonged to the Plan at the time of their death), for a period of two years after the death of the member.

Extended Health Benefit Plan. (a) The Company will assume one hundred percent (100%) of the cost of an Extended Health Plan. It is understood by both parties that this participation by the Company will not be compounded by any compulsory Provincial or Federal Medical Plan, either in existence or introduced at a later date. (b) It is further understood by both parties that the Company shall have full trusteeship of the Extended Health Benefit Plan and that the present benefit level will be maintained. (c) The Extended Health Benefit Plan will provide coverage for corrective lenses in accordance with the provisions of the vision care rider. The maximum amount claimable during any consecutive twenty-four month period will be three hundred dollars ($300.00). The employee will be reimbursed for the cost of eye exam once per calendar year.

Items (e) and (f) are to be fully paid for by the Employer. 6. Employee eyeglasses, frames or contact lenses ...Effective July 1, 1996, the Employer agrees to self-insure an additional $150.00 per employee, but not dependants, per twenty-four (24) month period. Such self-insurance shall be by reimbursement upon presentation of receipts for amounts in excess of the Pacific Blue Cross coverage in 3. (d), above.
