Extended Health Benefit Plan. (a) All regular and probationary employees after three (3) months employment will be covered by a one hundred percent (100%) Extended Health Benefit Plan with the standard $100.00 deductible. The City will pay eighty percent (80%) of the costs and the twenty percent (20%) deduction for employees shall be made through payroll deductions. The extended health lifetime maximum will be
Extended Health Benefit Plan. The Board shall pay eighty percent (80%) of the premium cost of the Provincial Extended Health Benefit Plan.
Extended Health Benefit Plan. The Pacific Blue Cross Extended Health Benefit Plan shall be made available to all employees. This plan shall include an eyeglass option of five hundred dollars ($500.00) every twenty-four (24) months, a hearing aid option of three hundred dollars ($300.00), lifetime limit, and coverage of registered psychologist treatments, registered social worker or clinical counsellor up to two hundred dollars ($200.00) every twelve (12) months. Premium costs shall be fully paid by the Employer. Extended health care lifetime limit to be increased to $1,000,000.00 effective January 1, 2008.
Extended Health Benefit Plan. The Extended Health Benefit Plan shall include a travel rider and an eyeglass rider: This provides travel for three (3) out of town medical referrals per calendar year for either the employee or an insured dependent. This plan also provides for a maximum of $40.00 per day for commercial accommodation. When the employee is the one going out of town for a medical referral, the Board will pay a per diem of $75.00 per day in lieu of wages for full time employees and a prorated portion for part-time employees for a maximum of three (3) days per referral. Eligible expenses under this provision shall include charges for eye glasses or contact lenses and their fitting, up to a maximum of $140.00 and a maximum of one such expense in any 2 consecutive calendar years, for each insured person.
Extended Health Benefit Plan. The Board agrees to pay 80% of the premium for the current Extended Health Benefit Plan for all eligible employees. Subject to continuing eligibility, the surviving dependents of a deceased member may retain membership at no cost in the Extended Health Care Plan (provided that the member belonged to the Plan at the time of their death), for a period of two years after the death of the member.
Extended Health Benefit Plan. The Employer shall contribute 100% of the premium of this plan.
(i) Total lifetime coverage will be unlimited.
(ii) Hearing aid benefit claims will be to a maximum of $600 every five years.
(iii) The Employer will provide an eyewear program providing $250 per family member every two (2) years.
Extended Health Benefit Plan. The Employer shall contribute one hundred percent (100%) of the premiums of the recognized Extended Health Benefit Plan for all participating employees. (Refer to Addendum “A” for more details.) The lifetime maximum for extended health benefits is one million dollars ($1,000,000) effective the first month following signing of the agreement.
Extended Health Benefit Plan. (a) The Company will assume one hundred percent (100%) of the cost of an Extended Health Plan. It is understood by both parties that this participation by the Company will not be compounded by any compulsory Provincial or Federal Medical Plan, either in existence or introduced at a later date.
(b) It is further understood by both parties that the Company shall have full trusteeship of the Extended Health Benefit Plan and that the present benefit level will be maintained.
(c) The Extended Health Benefit Plan will provide coverage for corrective lenses in accordance with the provisions of the vision care rider. The maximum amount claimable during any consecutive twenty-four month period will be three hundred dollars ($300.00). The employee will be reimbursed for the cost of eye exam once per calendar year.
Extended Health Benefit Plan. The Company agrees to provide an E.H.B. Plan as outlined below:
1. Participation in the Plan by all full-time and part-time regular employees covered by this Agreement is a condition of employment.
2. Coverage for all eligible employees (and their dependants) shall commence on the first of the month following sixty (60) days' employment.
3. The Plan shall include benefits as follows:
(a) twenty-five ($25.00) dollars deductible annually for eligible expenses incurred.
(b) eighty (80%) percent co-insurance for eligible expenses after the deductible has been satisfied.
(c) a maximum one million ($1,000,000) dollars limitation for each person covered per illness.
(d) claim of eyeglass, frames and/or contact lenses up to one hundred and fifty ($150.00) dollars per person covered per each twenty-four (24) month period.
(e) Prescription Drugs: Flu Vaccine to be covered.
(f) Hearing Aids: amend to include adult at two hundred ($200.00)
4. The Carrier shall be Pacific Blue Cross.
5. The premiums for items 3(a), (b), (c) and (d) above are to be shared 50% by the employer and 50% by the employees hired after July 1, 2005. Items (e) and (f) are to be fully paid for by the Employer.
6. Employee eyeglasses, frames or contact lenses ...Effective July 1, 1996, the Employer agrees to self-insure an additional $150.00 per employee, but not dependants, per twenty-four (24) month period. Such self-insurance shall be by reimbursement upon presentation of receipts for amounts in excess of the Pacific Blue Cross coverage in 3. (d), above.
Extended Health Benefit Plan. The Pacific Blue Cross Extended Health Benefit Plan shall be made available to all employees. This plan shall include an eyeglass option of five hundred dollars ($500.00) every two (2) years, eye examination coverage at $75 per year for employees and their dependents, a hearing aid option of five hundred dollars ($500.00), lifetime maximum, and coverage of registered psychologist treatments up to two hundred dollars ($200.00) every twelve (12) months. Premium costs shall be fully paid by the Employer. Extended health care lifetime maximum limit $1,000,000.00. Increase the following paramedicals to $500 effective on the date of ratification of the 2013-2014 Collective Agreement. Acupuncture from $80 payable to $500 per calendar year Chiropractor from $160 payable to $500 per calendar year Naturopath from $160 payable to $500 per calendar year Podiatrist from $160 payable to $500 per calendar year Employer to contact Pacific Blue Cross about including an Employee Assistance Program under Extended Health Benefits.
